[Trends in the specialty of internal medicine: specialization and/or integration].

The successes of modern medicine are results of the specialisation in medicine due to the technical development. With increasing specialisation several problems for education and training, for research and medical care develop, so, for instance, a total investigation of patients is not always given on a sufficient scale. Therefore it should increasedly be orientated to an education and training to a generalist both in the training of students of the subject human medicine and during the time of specialisation in the field of internal medicine. At the same time it appears necessary to discuss more intensely the integration and/or the specialisation with the internal specialists working in practice and in research, in order to develop at first a corresponding sense for the problems and to integrate the professional status of the generalist increasedly into the self-appreciation of all specialists of internal medicine.

[Enteral resorption kinetics in the aging process].

With growing age functional and morphological changes in the alimentary tract appear. The age dependence of the intestinal absorption was investigated with the help of the modified D-xylose absorption test. By means of a graphic-mathematical evaluation of the blood level kinetics of D-xylose after oral and intravenous application the absorption rapidity (constant k12), the temporary position of the relative maximum of the D-xylose concentrations in the blood serum after oral application (time tm) and the size of the relative absorption of the small intestine were established depending on the time RR (t). Within the age range of 3 to 96 years it has been proven that a slowing down of the enteral absorption occurred with growing age. By prolonging the period during which the food components are absorbed in the small intestine (shift of tm towards later times) the old organism is just as capable of complete absorption as is the young one, even though the absorption processes take place at a slower rate. Absorption disorders in old age are due to morbidity and not, sui generis, to involution.

[Alcohol induced damage to the pancreas in patients with increased blood alcohol levels detected in road traffic].

50 patients, driving motor-cars, having increased blood alcohol levels, were registrated and prosecuted. They voluntary underwent catamnestic researches for early diagnosis of alcohol induced lesions of the pancreas. In relation to the habitual drinking systems the study concluded 28 (56 per cent) healthy persons, 13 (26 per cent) alcohol endangered cases, 9 (18 per cent) alcoholics. Various biochemical parameters were changed, liver histomorphology resulted in about 50 per cent of cases a fatty degeneration of the liver and hepatitis. Special diagnostic methods (CCK-test, ERCP, ultrasound echography, computer-tomography) submitted in 24 per cent of cases suspect of alcohol induced pancreatitis. Results, which promote alcoholic pancreatic damage, are being discussed.

[Diseases of the digestive tract in old age (author's transl)].

Diseases of the digestive tract follow a changed pattern in old age. Their particularities as well as the geriatric aspects of diagnosis and therapy are discussed. Inflammations, neoplasms and circulatory disorders in the alimentary tract are discussed, while affections of the stomach, colon and the biliary tract are described in more detail because of their frequency and the propensity to complications and increased mortality rate involved. Although the therapeutical means and methods available for elderly patients do not differ substantially from those available for younger ones, certain guiding lines should be taken into consideration. The author shows by means of examples that certain difficulties in treating old people must not be allowed to lead either to an overrealous or nihilistic attitude.

[Investigation of the excretory function of the pancreas in later life by means of secretin and pancreozymin (author's transl)].

The investigation of the excretory function of the pancreas in subjects of various age groups has shown that digestive activity can be assessed only by means of secretin and pancreozymin. Stimulation with secretin (dose: 2 international units per kilogram of body weight of the subject) done twice within 60 minutes or a twofold combined secretin-pancreozymin test yield the most reliable data on the excretory functional capacity of the pancreas and its secretory reserve in elderly people. The data from subjects of various age groups--young, old and very old--can be used as a guideline for setting up standards for various age groups. They are also important for functional diagnosis and for differentiating between age-dependent functional changes and pathological conditions of the pancreas.

[Digestion and absorption in elderly man (author's transl)].

Morphologically first indications of an age-related alteration of pancreatic microscopic structures can be proved an age of fourty. In healthy elderly man functional capacity of exocrine pancreas is lowered. It can be proved by means of pancreatic function tests. Normally the digestive capacity will be sufficient provided a reasonable nourishment. Usually the intestinal absorptive capacity tends to decrease in the elderly. But following delayed intestinal transit of nutritients absorption to a certain degree can occur sufficiently. Considering the nutrition of elderly people recommendations are given.

[Initial results of investigations into the proliferation kinetics of enterocytes (author's transl)].

The changes of absorption appearing in man and caused by aging are probably due to disturbances of the proliferation and exfoliation kinetics of the enterocytes of the small intestine mucosa. The relevant experimental examinations available are insufficient because the results were obtained from a few animals only. Our own systematic autoradiographic examinations of rats are designed to contribute to a clarification of this problem which is important for practice.

[Involutive specialities of the water-- and electrolyte metabolism (author's transl)].

The involutive restriction of the renal, pulmonary, endocrine and buffer mechanisms also diminished the range of efficacy and possibility for adaptation of the water-- and electrolyte metabolism of old people. Under geriatric aspects many causes for the endangering of the water-- and electrolyte metabolism and the therapeutic consequiences are discussed. It is founded that in old patients the quicker metabolism imbalance means a stronger threat of life that the disease itself. Often the consideration of this problem in much cases of diseases can save the life of the old patient.

Peculiarities of digestion in senescence.

It is established, that involutional changes of digestive tract limits digestion in old patients. In the age digestive capacities mostly are sufficient, but there is a great tendency to maldigestion. The decrease of functional capacity doesn't show the clinical features of a manifest digestive insufficiency in old healthy probands. But it was shown that the digestive efficiency is exhausted by hyper-alimentation more quickly in elderly. The pancreas of elderly is able to digest small meals taken frequently.

Adult

[Aging and resorption].

The enteral absorption in dependence of age has been tested by D-xylose. From the additional investigation of the D-xylose reaction in the blood plasma one can detect statistically ascertained age relation in comparison between young and old healthy test persons. Though in old age the absorption process is retarded a sufficient absorption at a longer time at diminished speed of passage of the ingesta in old age.
