Bronchogenic carcinoma staging: CT versus MR imaging. Assessment with surgery.

One hundred and twenty patients with bronchogenic carcinoma were prospectively studied by both computed tomography (CT) and magnetic resonance imaging (MRI) during the 2 weeks preceding thoracotomy or mediastinoscopy. MRI scans included contiguous axial and coronal slices. Results of CT and MRI studies were compared with the surgical and pathological findings on the basis of the TNM classification. Although no significant difference was found between the two imaging methods for the evaluation of tumour extent, MRI appears to be a valuable imaging technique for left upper lobe and apical neoplasms. Comparison between the two techniques for mediastinal node evaluation showed that sensitivity of MRI is superior to CT in the left paratracheal, aortopulmonary and subcarinal node areas.

[Contribution and limits of x-ray computed tomography and magnetic resonance imaging in evaluating the extent of primary cancers of the bronchi].

The authors report the contribution and the limits of CT and MRI in the thoracic and extrathoracic staging of bronchogenic carcinoma. From data of a personal study and of the literature, findings and performances of CT and MRI are reported and compared for the evaluation of mediastinal and parietal extension and in the detection of nodes and metastases. Their respective places and indications are underlined.

Penetration of the brain by nonionic water soluble tri- and hexaiodinated contrast media. Experimental autoradiographic study of two contrast media: Iotrol and Iopamidol labelled with iodine 125.

After suboccipital injection of Iotrol and Iopamidol labelled with iodine 125 in rabbits, we measured residual radioactivity in the whole brain and optical density on autoradiographs of brain sections obtained 2, 8 and 24 h after injection. Residual radioactivity is higher with Iotrol than with Iopamidol after 8 h and 24 h. At densitometry, while the penetration of the cortex is the same with both media at 2 h (although subcortical passage of Iotrol is greater), by 8 h the concentration of Iopamidol is twice that of Iotrol, and at 24 h it is three times as high. A similar pattern was seen in the subcortical region. These densitometric findings are in agreement with previous electrophysiological studies, in which changes were less severe and more transient with Iotrol than with Iohexol. There is nevertheless an apparent lack of agreement between the studies of radioactivity and the electrical findings. The lower neurotoxicity of Iotrol may be explained by: a longer half-life in the subarachnoid space; its larger molecules, which inhibit diffusion in the extracellular fluid, and its more hydrophilic nature, which reduces intracellular penetration.

[Left adrenal pseudomass. Apropos of a case of splenorenal anastomosis by the vein of the left pillar of the diaphragm].

A case of hepatocarcinoma on cirrhosis with portal hypertension is described. The anatomy of splenorenal anastomoses and their ultrasound and CT scan imaging characteristics are reviewed, and the interest of differential diagnosis from adrenal masses emphasized. Its importance is enhanced by the fact that the portal hypertension may be unrecognized as in the present case. Diagnostic effectiveness of ultrasound and CT scan imaging is discussed.

Adrenal Gland Neoplasms↗

[The lesser omentum. Normal aspect and tumor pathology in ultrasonography and x-ray computed tomography].

The lesser omentum is a peritoneal partition that can be divided into two portions: superior or gastrohepatic ligament and inferior or duodenohepatic ligament, although differentiation is sometimes difficult. Ultrasound and CT scan imaging allowed perfect definition of vascular and glandular elements contained in the omentum, and normal morphology is described. Masses developing between the two leaves of which it is constituted usually project in front of adjacent vascular structures and/or prolonged into the liver hilum, and a fatty layer separates them from large prevertebral vessels. These different guidelines allow them to be distinguished from retroperitoneal celiomesenteric adenopathies. These expansive processes are almost always of glandular origin secondary to primary digestive, retroperitoneal, pelvic and even bronchial tumors. In contrast, primary tumors are rare in this region.

[Posterior hernia of a calcified disk in children. Apropos of 2 cases].

Posterior herniation of a calcified disk was detected in two children. A 10-year-old boy presented with cervicobrachial neuralgia at the C7 level secondary to herniation of a calcified C6-C7 disk treated by surgery. A 13-year-old boy was seen with a post-traumatic painful stiffness in the thoracic region posteriorly, without neurologic complication, secondary to a herniated calcified T4-T5 disk. Surgery was not performed and the course was favorable with regression of clinical signs and spontaneous disappearance of the herniated calcified fragment. A literature review showed reports of 21 identical lesions, full details being provided in 9 cases. Epidemiologic data, clinical manifestations, diagnostic means and management of these patients are discussed.

[Thoracic sarcoidosis with pulmonary cavitation. Apropos of a case].

The authors report an unusual case of thoracic sarcoidosis in a 21 years-old man who presents pulmonary nodules turning to cavitation. This observations and the few similar cases of the literature are analysed regarding to their clinical and radiological sequential patterns.
