The radiolabeled monoclonal antibody 791T/36 raised against a human osteosarcoma was injected into 20 patients with known or suspected bone tumors. Gamma camera images were acquired at 48 or 72 hours after injection, and assessed for antibody localization. Positive images were obtained in all five osteosarcomas and four other primary malignant sarcomas. Two of the four other primary bone tumors gave positive images. Three patients with trauma had negative images as did one patient with Paget's disease. Two patients with suppurative disease gave positive images. The antibody localized in the majority of malignant sarcomas tested. In one tumor where tissue was available, a tumor:non-tumor ratio of 2.8:1 was measured. Repeat imaging was performed in five patients. Immunoscintigraphy using the monoclonal antibody 791T/36 has shown tumor localization in patients with bone and soft tissue sarcomas.
Angiosarcoma is a malignant tumour of the vascular endothelial cell that rarely occurs in the head and neck. We present two cases arising in the nasopharynx after radiotherapy. The role of radiotherapy in carcinogenesis is considered with reference to the delay between treatment of a primary head and neck tumour and presentation with the second primary.

Mucosal biopsies from multiple sites in the stomachs of 21 patients with pernicious anaemia have been examined. The histological changes almost always involved the entire gastric mucosa, including that of the pyloric antrum. Metaplastic changes were almost universal and consisted of intestinal metaplasia in the body and antrum and pyloric metaplasia in the body. The severity of the pyloric metaplasia was such as to make the distinction between body and antrum on biopsy impossible. No relationship was found between serum gastrin activity and the histological appearances of the gastric antrum or body.

BACKGROUND: This study examined the validity of the early-late onset subtyping distinction in dysthymic disorder. METHODS: Participants were 340 out-patients meeting DSM-III-R criteria for dysthymia and a concurrent major depressive episode (MDE). The sample was drawn from a 12-site double-blind randomized parallel group trial comparing the efficacy of sertraline and imipramine in the treatment of chronic depression. All patients received comprehensive evaluations using semi-structured interviews and rating scales. RESULTS: 73% of the sample met criteria for the early-onset, and 27% for the late-onset, subtype. The early-onset patients had a significantly longer index MDE, significantly higher rates of personality disorders and lifetime substance use disorders, and a significantly greater proportion had a family history of mood disorder. The subgroups did not differ in symptom severity or functional impairment at baseline, nor in response to a 12-week trial of antidepressants. LIMITATIONS: Further work is needed to extend these findings to dysthymic disorder without superimposed MDEs. CONCLUSIONS: These results support the distinction between early-onset and late-onset dysthymic disorder.
Twenty-eight children whose diastematomyelia had been diagnosed at Our Lady's Hospital, Crumlin, were reviewed with respect to presenting symptoms, treatment, and outcome. Two children suffered significant neurologic damage during prophylactic removal of their bone spurs. Three children underwent spinal correction under traction with their bony spurs in situ without suffering any neurologic sequelae. The morbidity and mortality of surgical removal of bone spurs was considered and the traditional approach to this condition questioned.
Identical twins with spondylocostal dysplasia are reported. To our knowledge, this has not been reported previously. Other forms of short trunk dwarfism are discussed.
A 4-year old child with osteogenesis imperfecta developed high-output cardiac failure secondary to hypertrophic callus formation in four long bones. The association of hypertrophic callus formation with fracture and operative intervention is well known, but the role of infection in the aetiology of his condition is not well defined.
The presence of acute transient synovitis (TS, irritable hip) often warrants hospital admission not only to exclude underlying sepsis but also to aspirate when the intraarticular pressure might induce ischemia in the femoral head. Diagnostic doubt may affect this admission. We evaluated 68 patients in a prospective study comparing sonography with diagnostic aspiration and clinical examination. Sonography proved to be relatively accurate (81%) but was inferior to careful clinical evaluation in diagnosing hip effusions. Sonography may be helpful if diagnostic doubt exists. Aspiration under sonographic control will improve this accuracy.
Eighteen patients with slipped upper femoral epiphyses were treated by internal fixation using single pins in the centre of the femoral head. In 15 patients, prophylactic pinning of the unaffected side was performed; therefore, 33 hips were pinned. Results were encouraging in that no slip progressed following fixation, no pins broke, and no late reslips occurred. The optimum position for a pin is in the centre of the femoral head, with subsequent pins in suboptimal sites. The method of single pinning is simple and, theoretically, minimizes the risk of joint penetration.
