[Unusual pathology of the popliteal space. Popliteal artery entrapment].

After reviewing the main anatomical anomalies responsible for the popliteal artery entrapment syndrome, the author emphasizes the need for early diagnosis prior to onset of complications. Whenever confronted with hamstring pain due to sports-related physical stress in a youngster, loss of posterior tibial and pedal pulse should be checked for during static contraction, and then during forced passive stretching of the gastrocnemius muscle; modifications in arterial Doppler tracings during these maneuvers will confirm the diagnosis. Should this fail to occur at this early stage, severe complications will develop in time, manifested primarily by thrombosis of the popliteal artery; ectasia of this artery in itself carrying a risk of further peripheral embolism, thrombosis and rupture.

[Colonic cancer. Indications and results apropos of a continuous series of 223 patients].

The authors report a retrospective study of 223 surgical colonic carcinoma (1978-1987), the rectal carcinoma left out of them because of disparity in technical aspects. The total post-operative mortality rate was 9.4 per cent, made of great mortality in emergency (31 per cent) and in palliative surgery (17 per cent). Out of emergency, the mortality of curative surgery is 3.7 per cent. Surgical procedure was mechanical in 96 per cent with incidence of leakage in 3.9 per cent. The total survival rate is 53 per cent after 5 year follow-up, 70 per cent after curative surgery and 89 per cent for the first level (A) in DUKE'S classification. There is only a problem for the choice in size of resection for the sigmoid side (50 per cent of the cases). Contrary to recent studies, in our experience the post-operative mortality rate and survival rate are different with sigmoidectomy or left colectomy. Then the authors think that every model of resection has to be choice according age of patient and classification of carcinoma.

[Mechanical anastomosis on the second duodenum. Value, technics and indications].

The authors present 50 anastomoses on the second part of duodenum after gastrectomy, that were done for gastro-duodenal ulcer in 24 patients, gastric carcinoma for 20 patients and post-gastrectomy syndromes in 5 patients. The mechanical procedure provide excellent security (without leakage) and was easy to perform. When it is necessary to make the restoration of duodenal continuity, this device is a good technical solution.

[A pre-wirsungal portal vein. Apropos of a case].

Portal vein anomalies are rare. One of the more frequent is pre-duodenal portal vein (50 cases). The case report is describing a pre-pancreatic but retroduodenal portal vein discovered in an operation for chronic pancreatitis. No case report has been found in the literature. This paper presents embryological discussion and surgical consequences in pancreatic surgery.

[Common bile duct lithiasis. Reflections apropos of 1000 cases].

One thousand case reports of patients treated in eleven French surgical departments for stones in the common bile duct between 1975 and 1982 were analyzed. One in three cases were asymptomatic and detected by peroperative radiology during treatment of a simple or complicated gallstone, most forms were painful, with jaundice in 9 p. cent, an angiocholitis in 12 p. cent, and biliary pancreatitis in 2 p. cent. The most reliable exploratory procedure for gallstones is ultrasound imaging, as positive results were obtained in 90 p. cent of cases, but it enabled the diagnosis of choledocholithiasis in only one out of five patients. Intravenous cholangiography is a useful diagnostic tod for painful forms (60 p. cent). The failure of these two methods to establish the diagnosis in forms complicated by jaundice, pancreatitis, or angiocholitis has to be compared with the good results (85 p. cent) observed with endoscopic retrograde cholangiography. Treatment was surgical in 99 p. cent of patients, peroperative radiology being performed in 95 p. cent of these cases and endoscopy in 30 p. cent. In one out of two cases a single large stone was present while multiple stones were present in 8 p. cent. A "ideal" choledochotomy was carried out in 77 patients (7,7 p. cent) without mortality. External biliary drainage in 702 cases resulted in a 1.7 p. cent mortality rate, this increasing to 8 p. cent after 192 biliodigestive shunt operations, and 7 p. cent after 146 sphincterotomies, including 18 under endoscopic control. Unrecognized lithiasis, detected during follow-up radiography two weeks after external biliary drainage, accounted for 2-8 p. cent of cases. Of the 30 patients with residual stones and open biliary pathways, 6 were treated by mechanical removal, 6 by endoscopic sphincterotomy, and the others by repeat surgery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Interposition of dacron meshes through a Pfannenstiel approach for bilateral hernia (author's transl)].

The use of a dacron mesh interposed between the deficient abdominal wall and the peritoneum has proved to be a reliable means of preventing a recurrence. For inserting the material in bilateral hernias, until 1979, the authors used a median supra-pubic incision, in more than 150 patients. Since then they turned to the Pfannenstiel approach in 68 patients. This fashion has the following advantages: -- a better exposition -- a need for less prosthetic material -- no risk of hernia on the incision -- a better post-operative comfort for the patient.
