The limitation of primary excision of cicatricial alopecia: a report of 63 patients.

This 11-year retrospective study (1985-1996) of 63 patients demonstrates that cicatricial alopecia is almost always distressing for the patient and is considered as a social handicap, which makes repair a necessity. Two major therapeutic methods for remedying this condition were used in the study: suture excision and tissue expansion. The optimal use and limitations of each method are defined on the basis of treatment quality, as evaluated by the patient and the surgeon. Suture excision often ends with disappointing results for the surgeon as well as the patient and seems limited to alopecias no greater than 10 mm in width. Tissue expansion is considered generally satisfactory, but has the problem of scar enlargement, which reduces the quality of the result.

[Isolated palsy of the flexor pollicis longus in anterior interosseous nerve syndrome].

Anterior interosseous nerve palsy are relatively rare, specialy in traumatic cases. Two case of incomplete anterior interosseous nerve palsy with isolated total loss of function of flexor pollicis longus are reported. Partial palsy of the A.I.N. have been previously reported, but their mechanism is unclear. The incomplete anterior interosseous nerve palsy has to be known. They can be mistaken with tendon rupture, specialy in traumatic cases, and lead to unnecessary surgery.

Desmoid tumours of the hand.

We report a case of a 3-year-old child with a desmoid tumour of the hand, which is an exceedingly rare location. Desmoid tumours of the hand are difficult to treat because of the many important structures concentrated in the area as well as the infiltrative and recurrent character of these tumours.

Malignant blue nevus: three new cases and a review of the literature.

Three patients with malignant blue nevus are reported-one on the right side of the body, one on the right arm, and one on the face. The criteria and difficulty of histopathological diagnosis are considered as well as the differential diagnoses for this tumor. The therapy is described, and the possible relations between malignant blue nevus and certain other tumors (e.g., atypical or locally aggressive cellular blue nevus) are explored. A review of the literature reveals that there is current disagreement about the exact prognosis for these tumors and indicates the need to collect data for all patients observed.

[Iatrogenic intra-arterial injection during sclerotherapy of the external saphenous vein].

Inadvertant intra-arterial injection is a rare complication of sclerotherapy of varicose veins. We report one case of accidental popliteal injection complicated by acute ischemiae of the posterior compartment of the leg who needed fasciotomy and Achillus tendum elongation. There is no practical "step-by-step" guide of these complications and the best treatment is the prevention by respect of contraindications and the injection of sclerosing agent under ultrasonographic control.

[Hemifacial atrophy. A case report associated with linear scleroderma].

The authors report a case of facial hemiatrophy, secondary to linear scleroderma, and review the various possible causes of facial hemiatrophy. The various treatments proposed to correct facial hemiatrophy are described. The advantages and disadvantages of each technique together with their indications as a function of the severity of the lesions are then discussed. Free flap currently appears to be the treatment of choice in severe forms and the main question concerns the choice of flap. The authors prefer an inverted dermal fat flap because of its advantages (absence of long-term ptosis, better facial contours) and the absence of laparotomy.

Aneurysm of the coeliac artery.

A case of coeliac artery aneurysm associated with multiple splanchnic artery aneurysms is reported. This abnormality involved the gastroepiploic and hepatic arteries. The coeliac artery was ligated at laparotomy and the liver revascularized by direct anastomosis of the common hepatic artery to the aorta. Arteriography on the fifth postoperative day showed a satisfactory result. The surgical problems associated with aneurysms of the coeliac artery are reviewed.

Mycotic aneurysm of the left subclavian artery presented with hemoptysis in an immunosuppressed man: case report and review of literature.

We report the case of a 32-year-old man with a mycotic aneurysm of the left subclavian artery. This patient had immunosuppression caused by chemotherapy administered for treatment of leukemia. This aneurysm was revealed by two episodes of hemoptysis caused by a lung parenchyma fistulization. The patient was treated successfully by simple ligation and exclusion via a thoracotomy with partial lung resection. Histologic examination confirmed the presence of aspergilloma filaments in the false aneurysm. We suspect that aspergilloma could have been the cause of the mycotic aneurysm in this particular case. The literature on subclavian artery mycotic aneurysms is reviewed.

[Metacarpal aneurysmal cysts. Report of two cases and review of the literature].

The authors present two cases of aneurysmal cyst, one primary and one secondary. The first case was treated surgically by resection and bone graft. The second case, associated with a giant cell tumors, was initially treated surgically by resection and bone graft, but, because of a rapid recurrence of the giant cell tumor, the middle finger had to be amputated. The clinical, radiological and histopathological features of aneurysmal cyst are recalled. The various therapeutic modalities recalled and their value in this particular site are discussed.
