[What is the opinion of Spanish internists on osteoporosis?].

OBJECTIVE: To conduct an opinion survey on osteoporosis in Spanish internists. METHOD: Survey sent by mail and by personal visit to members of the Spanish Internists Society. Collection of data on opinion on the disease, diagnostic and therapeutic attitude and means available (general laboratory analyses, conventional radiology, biochemical markers of bone remodeling, densitometry and ultrasounds) and preference when choosing a certain treatment. RESULTS: A total of 538 internists answered. More than 90% of those surveyed consider that osteoporosis is a disease that should be treated by internists. A total of 93% consider that osteoporosis is a prevalent disease. More than 80% have access to densitometry. CONCLUSIONS: The majority of Spanish internists consider that osteoporosis is a disease that should be treated by internists and that it is a disease that enters into their action scope. In general, they have the means necessary for its study and treatment. Bisphosphonates constitute the drug of choice and calcium and vitamin D supplements are indicated in almost all the cases.

[Usefulness of bone densitometry in the evaluation of primary hyperparathyroidism].

BACKGROUND: In recent years, the clinical profile of primary hyperparathyroidism (PH) is predominantly characterized by mild or asymptomatic forms. In this context, the effects of the PH on bone metabolism reach a growing importance in the adoption of therapeutic decisions. PATIENTS AND METHODS: 116 patients with PH were studied, 95 women (25 premenopausal, 70 postmenopausal) and 21 males. In all cases parathyroidectomy criteria were evaluated. Bone mineral density (BMD) was determined in 71 patients in lumbar spine (LS) and femoral neck by dual-X-Ray absorptiometry (DXA) and the influence of this measurement in the decision of surgical treatment was analyzed. RESULTS: The patients with PH showed a significant reduction of BMD at all sites (p < 0.001) and 71.8% met osteoporosis densitometric criteria. The most frequent parathyroidectomy criteria was the presence of specific clinical manifestations (51.7%) followed by decrease of the BMD in LS (49.3%). The probability of meeting criteria for surgical treatment was significantly higher in patients in which BMD was measured (odds ratio: 3.09 [1.03-9.22]; p = 0.036). CONCLUSIONS: In its current presentation, PH presents a significant decrease of bone mass. The systematic performance of bone densitometry has a decisive influence in its appropriate management.

[Evolution of diagnostic and therapeutic criteria in Graves' disease in Spain. Comparison of the results of 2 surveys in 1987 and 1995].

BACKGROUND: To assess the new diagnostic and therapeutic trends for hyperthyroidism due to Graves' disease in Spain and the differences with respect to a previous study performed in 1987. METHODS: A questionnaire about a typical clinical case of hyperthyroidism due to Graves' disease and 10 variations to it, in which different diagnostic and therapeutic options are exposed. These questionnaires were mailed to 70 Spanish units of endocrinology during 1995, and 51 participated finally in the study. The results are compared with those obtained in Spain with a similar study in 1987. RESULTS: Thyrotropin (98%) and free thyroxine (88%) were the most used tests for diagnosis of Graves' disease, with a significant decrease (p < 0.001) in the use of total T4 and total T3 in comparison with the results of the questionnaire performed in 1987. The measurement of antibodies against thyrotropin receptor (TSH-R-Ab) was the most frequently used immune marker for the diagnosis (78%), with significant differences (p < 0.001) with respect to questionnaire in 1987. The use of anti-thyroperoxidase antibodies (anti-TPO-Ab) (36%) in diagnosis of this disease, significantly increased (p < 0.05) with respect to 1987. Antithyroid drugs were the most frequent initial treatment (98%) with significant differences (p < 0.001) in use of radioiodine (24%) as treatment of choice in elderly patients respect to 1987. Surgery was mainly used for large-size goiters (33%) and radioiodine for recurrences after medical (61%) or surgical (80%) treatment. Antithyroid drugs were the most frequent treatment for children and for recurrences during gestation. CONCLUSIONS: In Spain, the measurements of TSH, FT4 and TSH-R-Ab are the main diagnostic test of hyperthyroidism. Antithyroid drugs are still the treatment of choice in typical case of hyperthyroidism due to Graves' disease, in recurrences during gestation and children. Surgery is only used for large goiters and radioiodine is the treatment of choice in recurrences after medical or surgical treatment.

[Evaluation of cardiac sympathetic innervation with 123I-metaiodobenzylguanidine in patients with diabetes mellitus].

OBJECTIVES: To evaluate the usefulness of iodine-123 metaiodobencylguanidine (123I-MIBG) in the study of the cardiac autonomic neuropathy in insulin-dependent diabetic patients, by means of: a) analysis of heart to mediastinic ratio and tomographic images with 123I-MIBG; b) comparison with conventional non-invasive cardiac reflex test, and c) analysis of left ventricle ejection fraction. POPULATION AND METHODS: Ten patients submitted for cardiac evaluation with 123I-MIBG were compared with a control group of 11 patients. In both groups we excluded the presence of coronary pathology by means of an exercise test. We carried out planar and SPECT studies, using 123I-MIBG, and the calculation of the ejection fraction by equilibrium ventriculography. RESULTS: The uptake of 123I-MIBG in diabetic patients was significantly smaller than the control group in the calculated index (heart to mediastinic ratio: 1.64 +/- 0.20 vs 2.00 +/- 0.26; p < 0.001; SPECT index: 44.87 +/- 8.37 vs 55.54 +/- 3.96; p < 0.001). In polar images we noted a more reduced uptake in the diabetic group in the inferior wall (p = 0.020). Patients with cardiac sympa-thetic dysinnervation demonstrated less uptake in both indexes (p < 0.05 and 0.005, respectively), essentially in the inferior wall and in basal and medium territories (p < 0.05). No differences in respect to the ejection fraction parameter were found. CONCLUSIONS: Despite the small sample population, insuli-dependent diabetic patients in show a significant reduction of the uptake of 23I-MIBG, more accentuated in the base. Scintigraphy using 123I-MIBG can be an approach to investigate the pattern and the natural history of the sympathetic innervation in these patients.

[Multicenter clinical, double blind, and randomized study to assess the effectiveness and safety of binifibrate versus gemfibrozil in type IIa, IIb, and IV hyperlipidemia].

A multicentric, double-blind and randomized clinical study was conducted on a total of 54 patients with hyperlipidemias type IIa, IIb and IV in order to compare the effectivity and tolerance of two hyperlipemiant drugs: Binifibrate and Gemfibrozil. The treatment had a length of six months, performing clinical and analytical controls at: baseline, 1 month, 2 months, 4 months and 6 months. Both drugs showed a very good tolerance and the following results were obtained: Total cholesterol: Binifibrate -21.9%, Gemfibrozil -20.5%; Triglycerides: Binifibrate -48.6%, Gemfibrozil -43.27%; HDL: Binifibrate +18.2%, Gemfibrozil -12.8%; LDL: Binifibrate -24.3%, Gemfibrozil -22.8% VLDL: Binifibrate -48%, Gemfibrozil -43.2%; Apo A: Binifibrate +133.5%, Gemfibrozil +108.1%; Apo B: Binifibrate -59.7%, Gemfibrozil -46%. Both treatment were very effective, although the greater increase produced by Binifibrate on Chol-HDL, with a statistically significant difference versus the increase produced by Gemfibrozil, must be highlighted.

[Primary hypothyroidism associated with changes in the sella turcica and the hypophyseal function. Description of 5 cases].

We report 5 clinical cases of primary hypothyroidism or premixedema with associated pituitary lesion. Before treatment was started with L-thyroixine (L-T4) pituitary hormones were examined and an X-ray of the silla turca as well as a CT scan of the hypophysis (CTH) was done. Once supplementation treatment was started hormone levels normalized. In three out of the 5 cases, CT scan done after establishing therapy, showed empty silla turca. Therefore, me stress the importance of closely examining the pituitary gland in patients with untreated primary hypothyroidism.

[Serum osteocalcin in insulin-dependent diabetic patients].

Serum osteocalcin (BGP) is the currently most reliable marker of osteoblastic activity and of the bone formation index. To investigate its state in patients with insulin-dependent diabetes mellitus (IDDM) we measured the BGP level in 82 individuals divided in two groups. The control group (CG) consisted of 26 healthy volunteers (16 males and 10 females: mean age 25.6 +/- 2.8 years). The diabetic group (DG) consisted of 56 patients with IDDM (36 males and 20 females; mean age 23.2 +/- 6 years). The DG was, in its turn, subdivided in four subgroups (A, B, C, D) depending on the time from the diagnosis (TE) (less than 2, 2-5, 5-10, and greater than 10 years). The DG had a significantly lower BGP than CG (3.01 +/- 1.5 vs 3.9 +/- 1.7 ng/nl; p = 0.024). BGP levels were also lower in the subgroups C (2.4 +/- 1.0 ng/ml; p = 0.006) and D (1.7 +/- 0.9 ng/ml; p = 0.000) than in CG. When they were compared between them, the subgroups A (3.6 +/- 1.1 ng/ml) and B (4.5 +/- 1.4 ng/ml) had BGP levels significantly higher than subgroups C (2.4 +/- 1.0 ng/ml; p = 0.009 and p = 0.000) and D (1.7 +/- 0.9 ng/ml; p = 0.000 and p = 0.000). There were no differences between subgroups A and B, on the one hand, and C and D, on the other. There was a highly significant lineal correlation between TE and the BGP levels (r = 0.5196; p = 0.000). The patients with IDDM have reduced BGP levels; this reduction becomes apparent after 5 years of clinical evolution and shows a correlation with the time since the diagnosis.

[Biochemical profile of primary hyperparathyroidism. Comparative study with hypercalciuric renal lithiasis].

Primary hyperparathyroidism (PH) is now considered a common condition. Its frequency and the deleterious long-term effects of hypercalcemia make a correct diagnosis mandatory. We attempted to evaluate the usefulness of the indexes of parathyroid function and hormone measurements more commonly used in the diagnosis of PH. To this end we studied 64 patients, distributed in three groups: group with PH, group with hypercalciuric renal lithiasis (HRL) and control group (CG). The results were evaluated with a test of comparison of means and a stepwise discriminating regression analysis. The 8 most useful measurements to differentiate PH from HRL and CG were serum calcium, corrected serum calcium, serum phosphorus, fasting calcium excretion (FCE), maximal tubular calcium reabsorption (MTCR), maximal tubular phosphate reabsorption (MTPR), osteocalcin, PTH half molecule (PTH-HM) and 1,25-dihydroxyvitamin D. The 3-variable and 4-variable groups with a highest discriminating ability were: serum calcium, FCE and PTH-HM, and serum calcium, FCE, PTH-HM and MTPR. We think that the measurement of these four variables is the most adequate strategy for the diagnosis of PH.

[Parathormone, cyclic AMP, 1,25 dihydroxyvitamin D and osteocalcin in hypercalciuric renal lithiasis].

A study was undertaken in 46 subjects; 21 patients diagnosed as having HRL and 25 volunteers patients. Biochemical and hormonal analyses were performed in the study population, including determination of Ca, P, Mg, Cr in blood and urine, phosphate tubular resorption (PTR), maximum tubular phosphate resorption (MTPR), fasting calcium secretion (FCS), alkaline phosphatase (AP), hydroxyprolinuria (HPR), osteocalcin (BGP), parathormone (PTH), cAMP, and 1-25(OH)2D. The stone formers showed lower calcemia values (p less than or equal to 0.005d), higher phosphaturia, and magnesiuria (p less than or equal to 0.0005), higher FCS (P less than or equal to 0.005) and higher values for PTH (p less than or equal to 0.01) and cAMP (p less than or equal to 0.0025). No significant differences were observed for the other parameters evaluated. Classification of the patient group into 2 subgroups (renal SbR and absorptive SbA) according to FCS values greater or lower that 0.16 mg/dl, the SbR patient group revealed a higher PTH and 1-25(OH)2D values (p less than or equal to 0.05). There appears to be no increase of bone resorption since AP, HPR, and BGP values in our patients fell within normal ranges. The 1-25(OH)2D levels were also normal and, with respect to the control group, were only elevated for the SbR patient group, whose PTH levels were also observed to be elevated. These increments appear to be related and may result in intermediate forms between renal and absorptive hypercalciuria.
