New technique for optical control of the first trocar insertion.

BACKGROUND: In laparoscopic surgery serious complications caused by the blind insertion of the first trocar have been reported even after the pneumoperitoneum is established by means of a Veress needle. As a consequence, some techniques to safely insert the first port have been developed. Many surgeons advocate a minilaparotomy to position the first port. However, especially in obese patients, the dissection to insert the Hasson's trocar may be difficult and time consuming. As an alternative, optical disposable trocars have been studied and produced to be safely positioned under direct visualization. METHODS: A new technique to insert, under adequate optical control, a non disposable standard port is described. Personal experience in 126 consecutive patients is reviewed. Data were recorded prospectically and insertion time was recorded in every case. RESULTS: Medium time of trocar insertion was 55 sec (range 30-160 sec). Identification of distinct layers of the abdominal wall was always possible. There were no complications related to the insertion of the first reusable cannula under visual control. CONCLUSIONS: This technique may be considered a safe, simple and cost effective method to insert the first trocar.

Malignant duodenocolic fistula. A case report.

Malignant duodenocolic fistula is a rare condition in which radical surgical treatment is seldom possible. A personal case treated by pancreaticoduodenectomy is presented with a review of the literature.

[The role of cytology by needle aspiration in the diagnosis of thyroid nodules. Examination of 179 cases].

Since May 1981, fine needle biopsies have been used for cytological examination of thyroid nodules that were barely or not recorded by scintiphotography. By February 1984, a total of 179 patients had been examined. The biopsies were repeated on 12 patients where the first sample produced no diagnostic material. In 57 cases, a histological control existed in the form of material removed surgically by enucleoresections, partial and total thyroidectomies. Histological examination confirmed the cytological diagnosis in 7 out of 8 cases (88%) of malignant neoplasias with varying degrees of differentiation. A cytological diagnosis of adenoma was reached in 80% of cases, and of nodular struma in 82% of cases. Microscopic aspects of the cases are described together with the consequent cytological diagnostic criteria, while the characteristics and importance of the technique are also discussed.

[Value of modern diagnostic technics in rupture of the spleen].

Personal experience is reported of the latest instrumental techniques used to diagnose ruptures of the spleen following a closed abdominal trauma. Particular interest was taken in patients with unclear clinical symptoms who were given a series of surgical and/or non-surgical tests in order to diagnose ruptured spleen in the asymptomatic phase. Tests analysed included: peritoneal injection-douche, laparoscopy, angiography, scintiphotography, echography and CAT scanning.

[Value and limitations of the gastric secretion function test with 99mTc-pertechnetate].

The technique and results of the gastric secretion test using 99mTc-pertechnetate to investigate functional organic gastropathies are described. The test is fairly simple, well tolerated and an alternative to gastric probing. If performed with a computerised gamma chamber it can indicate the volume of the secretion. A rapid scintigraphy will also provide information on intestinal morphology and is useful if such information it not otherwise obtainable. However the real value of the test is in checking the results of pharmacological or surgical treatment.

[Echography and percutaneous transhepatic cholangiography in the evaluation of obstructive lesions of the bile ducts].

Among diagnostic techniques proposed for the study of obstructive jaundice, the initial use of echotomography clearly distinguishes between medical and surgical forms. If surgery is not indicated, further diagnostic investigations, (transhepatic percutaneous cholangiography followed if necessary by drainage of the bile duct) are conducted. Used in conjunction, echography and TPC will solve clinical doubts in most cases.

[Rehabilitation of patients after colostomy. Personal experience].

Personal experience of aiding 58 colostomised patients is reported. Certain basic aspects of aid in the immediate and later postoperative period are emphasised: diet, stools, irrigation, psychological support. Follow up showed that most patients returned successfully to normal life, returning to work and in some cases to the practice of sports.

[Usefulness of sequential isotopic cholangiography in biliary surgery follow-up].

Personal experience in the follow-up of 39 patients subjected to bile duct surgery is reported. The results indicated that sequential isotopic cholangiography is less effective as a supplementary examination in the postoperative period by comparison with its use in preoperative diagnosis. It does, in fact, constitute the only possibility of visualising the main biliary pathway in subjects who have been operated for biliodigestive anastomosis, with the exception of invasive methods, such as PTC, itself out of the question in the execution of periodic postoperative controls.

[Use of single-layer anastomoses in intestinal surgery; experimental and clinical studies].

Reference is made to an experimental and clinical investigation of single-layer intestinal anastomoses. The techniques of Gambee, D'Onofrio, Lembert, Halsted Connel were applied in six groups of 10 rabbits. The survival and weight data, gross and microscopic inspection, and the stenosis index showed that the best results were obtained with Halsted's, D'Onofrio, and Gambee's techniques. An account is also given of a clinical series of 124 patients in which Halsted's technique was employed.

[Galactography: an indispensable diagnostic supplement in the study of the secreting breast. Evaluation in 132 cases].

Personal experience with 132 galactographic examinations carried out on all types of breast secretion is reported, in 7 patients in the absence of secretion. Radiology showed 38 endoductal productive lesions which did not always reveal themselves with loss of blood from the nipple. Features of the ductal lesions in relation to the type of secretion are compared. No complications connected to the examination were ever encountered even in cases in which the contrast medium spread.

[Value of thermography in the postoperative control in direct arterial surgery].

21 patients suffering from arteriopathy of the lower extremities and treated with a variety of surgical techniques and arterial recanalization have been studied by means of pre- and post-operative angiography and thermography. After analysing the results, the value of thermography, particularly as regards assessment of the surgical results and subsequent long-term controls, is confirmed.

[The surgical therapy of normally functioning multinodular goiter: our experience].

Surgical treatment of multinodular goiter is still now discussed: total versus subtotal thyroidectomy. Advantages of partial thyroidectomy are the low incidence of laryngeal nerve injury and the low risk of hypoparathyroidism. Disadvantages are the increased recurrent disease, the risk of carcinoma, the elevated possibility of laryngeal nerve lesions and permanent hypoparathyroidism after reoperations. Own experience regarding 401 thyroid operations is reported; 60 patients were operated for multinodular goiter; 48 subtotal and 12 total thyroidectomy were performed. Among those 1 (1.65%) inferior laryngeal lesion and 1 (1.65%) permanent hypoparathyroidism occurred. Anatomic knowledge and correct surgical procedure give a reduction in the mobility of total thyroidectomy which in selective case can be considered the treatment of choice.
