Alternative pathway therapy for urea cycle disorders.

In man the major pathway for the disposal of waste nitrogen is the urea cycle; in inborn errors of this pathway, nitrogen flux is reduced. As a result there is accumulation of ammonia and glutamine with disordered metabolism of other amino acids. Nitrogen homeostasis can be restored in these patients with a low-protein diet combined with compounds that create alternative pathways for nitrogen excretion. The introduction of these compounds has been a major advance in the management of these inborn errors and as a result the outcome, particularly for those treated early, has improved.

Lipoprotein metabolism in marasmic children of northern Mauritania.

Lipid metabolism in marasmus has been poorly studied compared with that in kwashiorkor. Published studies show normal or high plasma triglycerides and low cholesterol concentrations. We studied some plasma lipid indexes, including total, HDL, and LDL cholesterol; triglycerides; apolipoproteins A-I, A-II, B, C-III, and E; and lipoprotein particles containing apo A-I, apo A-I and A-II (Lp A-I:A-II) Lp C-III:B, and Lp E:B in Mauritanian marasmic children compared with a control group. We found in patients low total, HDL, and LDL cholesterol and normal Lp A-I concentrations, which suggest that the cholesterol reverse transport system is not altered. We observed normal triglyceride concentrations and significantly increased Lp C-III:B and Lp E:B, which are triglyceride-rich particles. These data suggest that the marasmic state led to quantitative and structural modifications of the triglyceride-rich lipoproteins, defined by their apolipoprotein composition, and was probably related to undercatabolism.

Effect of sympathetic nervous system on cerebral blood flow in the newborn piglet.

The role of the sympathetic nervous system on cerebral blood flow (CBF) autoregulation was evaluated in newborn piglets. Six animals were studied after ablation of the right superior sympathetic ganglion and compared to 6 control animals. Mean arterial blood pressure (MABP) was decreased by successive blood withdrawal and CBF was measured by radioactive microspheres. In denervated animals, MABP and CBF correlated positively according to a parabolic curve showing an absence of autoregulation when MABP is above 50 mm Hg (y = 0.079x2 - 5.9x + 154, p less than 0.01). In control animals, CBF remains stable throughout the experiment (y = 0.28x + 5). These data suggest a shift to the left of the upper limit of the autoregulation range in denervated animals and consequently a poor adaptation to increased MABP.

[The current practice of prevention of rachitis in France. A survey of 600 physicians].

French legislation prohibiting vitamin D food supplementation, rachitism prophylaxis is based on specific infantile calciferol administration, according to an old ministerial recommendation published in 1963. Out of 1000 questioned physicians, 600 completed the present inquiry. Ninety-nine per cent of them indicated that they systematically prescribe vitamin D for infants, but 18% do not propose it to breast-fed babies; 36% begin prophylactic administration only after 2 weeks and 5% after one month of age; 16% stop prevention between 12 and 18 months, 72% between 2 and 6 years and 12% after 8 years. Expressed on a daily basis, vitamin D dosage is usually rather high, generally slightly higher than the officially recommended intake (1100 to 1400 versus 1000 IU/24 h) and can reach excessive levels, especially in coloured people for which the already large proposed dose (2500 IU/24 h) is exceeded by 13% of inquired physicians. Daily rhythm of supplementation is the most frequent (94% of physicians) before 2 years, semestral or annual afterwards. The mode of administration, diluted in milk (40%), water (27%) or fruit juice (14%) depends on the physicians. In free commentaries, none of them speaks about maternal supplementation during pregnancy. Very few propose special dosage for premature babies. These defects in rachitism prophylaxis are partly due to the obsolescence of official recommendations which postpone the beginning of vitamin D prevention until the age of 6 weeks, do not mention maternal supplementation during pregnancy and do not specify the necessity for vitamin D intake in breast fed infants.(ABSTRACT TRUNCATED AT 250 WORDS)

[Lesions of the basal ganglia in mumps. Clinical and neuroradiological development in a case].

Among mumps complications, encephalitis is the most frequent. Involvement of the basal ganglia has been previously reported associated to some other encephalic lesions. In one case, changes were confined to basal ganglia and occurred at the 10th day of evolution of mumps. Clinically a bilateral extrapyramidal syndrome was present without other neurological disorders. CT scan showed bilateral lesions in the area of the basal ganglia. The course was favorable. The neurological examination and CT scan were normal 6 months later. Such complications should be prevented by vaccination.
