[Comparison of radiographs and magnetic resonance imaging in the detection of sacroiliitis in patients with ankylosing spondylitis].

The role of radiography and magnetic resonance (MR) imaging in the detection of sacroiliitis in patients with ankylosing spondylitis (AS) was compared in this study. Thirty-six sacroiliac joints in 18 patients with AS were examined with radiography and MR scan. MR images were performed with the sequence of the coronal T1 weighted image, T2 weighted image, and T2* weighted image. Images of all patients were graded according to the modified New York criteria. Statistical results showed that significant differences existed between MR imaging and radiography in the detection of sacroiliitis (P < 0.01). MR imaging was superior to plain film radiographs in visualising erosions (P < 0.01). Radiography cannot reveal the cartilage changes and bone marrow oedema, which can only be seen in MR images. In the absence of radiographic changes, MR imaging can provide objective and complementary findings of sacroiliitis in patients with AS. Due to the ability to image cartilage changes and bone marrow oedema directly, MR imaging may be particularly useful in early diagnosis of sacroiliitis. For patients without any changes on sacroiliac joint radiograph, further examination of MR imaging may be advisable.

Expression of HBV Pre S1 peptide in E. coli and product characterization.

HBV Pre S1 sequence is supposed to play an important role in the infection of HBV. Presence of Pre S1/anti-Pre S1 in serum has valuable clinical implications. In order to improve the study of Pre S1, Pre S1 sequence was overexpressed in E. coli as a fusion protein with MBP (Maltose-binding protein), and anti-Pre S1 antiserum was elicited in rabbits by Pre S1-MBP purified by affinity chromatography. The recombinant plasmid constructed from pMAL-cRI expressed the 106aa Pre S1 sequence at the C terminal of MBP by tac promoter. The resulting protein is about 54 kD in size. Western-blot analysis confirmed its reactivity with antiserum derived from synthetic Pre S1 peptide and serum from patients with acute hepatitis B (AHB). ELISA showed that Pre S1-MBP and Dane particles purified from AHB patient's serum reacted with antiserum against synthetic Pre S1 peptide, and this reaction was specifically inhibited by synthetic Pre S1 peptide. ELISA also demonstrated that antiserum against Pre S1-MBP reacted with synthetic Pre S1 peptide, but not with synthetic HCV peptide or HEV peptide.

Artificial neural network to assist psychiatric diagnosis.

BACKGROUND: Artificial Neural Network (ANN), as a potential powerful classifier, was explored to assist psychiatric diagnosis of the Composite International Diagnostic Interview (CIDI). METHOD: Both Back-Propagation (BP) and Kohonen networks were developed to fit psychiatric diagnosis and programmed (using 60 cases) to classify neurosis, schizophrenia and normal people. The programmed networks were cross-tested using another 222 cases. All subjects were randomly selected from two mental hospitals in Beijing. RESULTS: Compared to ICD-10 diagnosis by psychiatrists, the overall kappa of BP network was 0.94 and that of Kohonen was 0.88 (both P < 0.01). In classifying patients who were difficult to diagnose, the kappa of BP was 0.69 (P < 0.01). ANN-assisted CIDI was compared with expert system assisted CIDI (kappa = 0.72-0.76); ANN was more powerful than a traditional expert system. CONCLUSION: ANN might be used to improve psychiatric diagnosis.

[Phase I clinical study of a new anticancer drug boanmycin].

From August 1993 to March 1994, 36 patients were enrolled for a phase I study of Boanmycin (Bleomycin A6) a new anticancer drug to determine it toxicity and maximal dose. Of the 36 cases, 16 were male and 20 female, age 20-62. Dose escalation was performed if a minimum of threed patients were fully evaluable for toxicity (2 weeks following drug administration) and if severe or life-threatening toxicity had not occurred. Dose of Boanmycin were escalated from 1 mg (0.5-0.7 mg/m2) to 12 mg(6.7-7.5 mg/m2) i. m. three times every week for two weeks. Surveillance of serum concentration of Boanmycin was conducted in six cases by microbiological analysis, and the pharmacokinetics parameters were obtained. Phase I study of Boanmycin showed that Boanmycin had no myelosuppression and cardiac toxicity, and its major adverse reactions were fever, gastrointestinal reactions and hardening at injection (by i. m. route). Rather than dose-related, fever was individual-related. There were mild myalgia, alopecia, skin rash, pigmentation in some patients. All adverse reactions resolved after discontinuation of therapy. There was no Boanmycin-related lethal complication, and the maximum tolerated dose was not obtainable. If patients have renal or lung disease, Boanmycin aggravate their renal and lung functions. Therefore we recommend that dose of Boanmycin for phase I clinical trial should be is 8-10 mg(5-6 mg/m2) i m or iv (iv can decrease local side effect) two-three times per week.

[Etoposide combination chemotherapy for malignant lymphoma: a report of 91 cases].

From August 1984 to August 1994 ninety-one caese of malignant lymphoma (NHL 84 casese, HD 7 cases) were treated with Etoposide (VP-16) combinationn chemotherapy. In 70 evaluable patients, 18 achieved CR, 35 PR, total response rate was 75.7%, response time (RT) 1-79 months, medium response time (MRT) 9 months, survival time (ST) 2-79 months, medium survival time (MST) 12 months. The therapeutic efficacy was better than VP-16 alone. The major side effects were myelosuppression and gastrointestinal toxicity. In COPEP, CHEP, PEP and IEP regimens, best result was seen in COPEP (CTX + VCR + PYM + VP-16 + PDN), with a response rate of 93%. Authors suggest that COPEP protocol could be the first line treatment for patients with malignant lymphoma who did not tolerate anthracycline drugs and the second line treatment for anthracycline-resistant patients.

[A phase II clinical trial of pepleomycin].

From November 1993 to December 1994, a prospective multi-centre phase II clinical trial was done in 137 patients with advanced cancer on China-made pepleomycin (PEP). PEP was effective in patients with cancer of the head and neck, malignant lymphmo and lung cancer, with response rate of 66.7%, 50% and 30%, respectively. The major adverse reactions were fever and mild gastrointestinal reaction. In the controlled study of combination chemotherapy with China-made and Japan-made PEP, the response rates and side effects were similar. The results indicate that China made PEP can be used in lieu of that made in Japan.

[Studies on some characteristics of a strain of mink infectious enteritis virus].

A strain of mink infectious enteritis virus that was isolated from the east district of China can be bred on FK cell. Concentrated viruses were purified by using Sepharose-4B chromatography. The size of the virus was about 20-22 nm by electron microscope. Viral nucleic acid was extracted from pure virus by using SDS-protease K-Phenol. Tests with diphenylamine, acridine orange and the curve of thermal dentuation, etc. showed that the virus had a single-stranded DNA. The molecular weight of the ssDNA was from 1.5 x 10(6) to 2.0 x 10(6) determined by length of the virul nucleic acid.

[An experimental animal model study of HCMV].

This Paper is a study of animal model on the Kunmin and BALB/C mice infected HCMV-AD169. The death-rate showed a marked higher BALB/C mice (28.75%) than the kunmin mice. The Kunmin mice Percentage of diseased (94.74%) higher than BALB/C mice. Brian tissue of two groups mice that pathological changed was equal by HCMV infected.

Long-term effects of gingival fibroblast-coated hydroxylapatite graft on periodontal osseous defects.

This study was designed to examine the long-term effects of gingival fibroblast-coated hydroxylapatite (GF-HA) grafting on the healing of osseous defects caused by inflammatory periodontitis. Gingival fibroblasts were cultured from biopsy specimens of a healthy portion of gingiva in a periodontal therapy patient. Growth of cells on hydroxylapatite (HA) particles was established in vitro and the GF-HA complex was transplanted into the periodontal osseous defects. Clinical data of the gingival and plaque index, probing depth and periapical X-ray were monitored at baseline and at regular intervals up to 28 months after surgery. Postoperative plaque control and regular maintenance were meticulously performed. In addition to disappearance of gingival inflammation, results showed that the GF-HA treated sites had a marked pocket reduction and probing attachment gain at reentry and later recalls. Clinical bone fills of osseous defects were also demonstrated in periapical radiographs. These observations suggest that GF-HA grafting treatment may lead to regeneration of periodontal tissue in periodontitis-affected osseous defects.

Automated classification of encounter notes in a computer based medical record.

Harvard Community Health Plan is exploring emerging information technologies for means to use the text portion of its 25 year old computerized medical record system. The Center for Intelligent Information Retrieval is developing systems to answer the question: to what extent can automated information systems replace manual chart review of encounter notes? INQUERY, a probabilistic inference net information retrieval system, and FIGLEAF, an inductive decision tree text classifier are applied to the problem of classifying electronic encounter notes to identify acute exacerbations in pediatric asthmatics. Both systems achieve average precisions of greater than 80%, with a new enhancement to INQUERY's relevance feedback, the top performer. Refinement of the systems and plans for their integration are discussed.

[The clinical efficacy of CAC regimen in the treatment of advanced breast cancer].

Twenty seven Patients with advanced breast cancer were treated with CAC (carboplatin, adriamycin, cyclophosphamide) regimen. Eighteen patients had no prior chemotherapy. Carboplatin was given 300 mg/m2 intravenously (IV), on day 1 or 150 mg/m2 on day 1, 2, adriamycin 40 mg/m2 IV on day 3, cyclophosphamide 500 mg/m2 IV on day 3, 10. The treatment was recycled every 28 days. The overall objective response was 63% (17/27) with a CR rate of 18% (5/27). The median duration of response was 9 months and median survival time was 17 months. The response rate of soft tissue metastasis was 61% (11/18). Lung metastasis responded in 3/5, liver metastasis in 3/3, pleura metastasis in 4/4, bone metastasis in 1/10. The response rate of previously untreated patients was 72% (13/18). The gastrointestinal reaction was mild. No renal toxicity was observed. Leukopenia (WHO grade II, III) was seen in 89%. These results indicate that CAC is an effective regimen for advanced breast cancer.

Heterogeneous expression of alpha 1-adrenoceptor subtypes among rat nephron segments.

alpha 1-Adrenoceptor subtypes mediate many of the actions of the renal nerve, but their locations along the nephron are unknown. We investigated the distribution of alpha 1-adrenoceptor subtype mRNA and protein in rat proximal tubules and medullary thick ascending limbs (MTAL) using reverse transcription combined with polymerase chain reaction (PCR) and radioligand binding methods. Complementary primers were designed to span cDNA sequences in each of the third intracellular loops of the rat alpha 1B- and alpha 1D-adrenoceptors. Expression of the mRNA of alpha 1B- and alpha 1D-adrenoceptors was first detected in total RNA from whole rat kidney, and the PCR product identity was confirmed by sequencing. Endogenous expression of alpha 1B- and/or alpha 1D-adrenoceptor mRNA was then investigated in microdissected segments of the rat proximal convoluted tubule (S2 segments) and the MTAL. mRNA was reverse-transcribed directly from permeabilized microdissected segments and the resulting cDNA was subjected to PCR with the alpha 1-adrenoceptor primers. In proximal convoluted tubules, amplification of both alpha 1B- and alpha 1D-adrenoceptor mRNA was observed. In MTAL segments, only alpha 1D-adrenoceptor mRNA was detected. We also measured receptor protein using [3H]prazosin in saturation and competition binding experiments. Proximal tubular membranes contained 3.3-fold more alpha 1-adrenoceptor than did MTAL membranes (163 +/- 21 versus 49 +/- 3 fmol/mg of protein). When the alkylating agent chloroethylclonidine (CEC) (10 microM, 10 min) was used to define alpha 1-adrenoceptor subtypes, proximal tubules were found to contain primarily CEC-insensitive (alpha 1A) sites (68 +/- 4%) and MTAL primarily CEC-sensitive sites (75 +/- 3%). Most [3H]prazosin binding sites (72 +/- 2%) in MTAL segments were also sensitive to the alkylating agent SZL-49, consistent with their identification as alpha 1D-adrenoceptors. In competition studies with the antagonists WB4101, 5-methylurapidil, and (+)-niguldipine, both high and low affinity sites were observed in proximal tubules. WB4101 interacted with only one site in MTAL membranes, intermediate in affinity between those sites found in proximal tubules. We conclude that reverse transcription-PCR is a useful method for demonstrating the expression of alpha 1-adrenoceptor subtypes in small amounts of tissue. Results from our experiments suggest that alpha 1A-, alpha 1B-, and alpha 1D-adrenoceptors are all expressed in proximal tubules and that alpha 1D-adrenoceptors are the primary alpha 1-adrenoceptor subtype expressed in MTAL. The distinct anatomical distribution of each of these adrenoceptor subtypes suggests that they serve different functions in the kidney.

Treatment of osseous defects with fibroblast-coated hydroxylapatite particles.

Several techniques of periodontal regeneration have been used, including hydroxylapatite (HA) grafting, demineralized free-dried bone allografts and guided tissue membranes, for which clinical merits still need further scientific study. The purpose of this pilot study was to examine the healing of periodontal defects using HA grafts coated with fibroblasts isolated from the periodontal ligament (PDL). We cultured fibroblast-like cells either from a clinically healthy site of the PDL or gingival tissues of the subject receiving the cell transplantation. We added HA to the cultures and allowed the cells to migrate onto the HA surface. Then the HA particles coated with cells were harvested and transplanted into the periodontal osseous defects of four patients after phase I treatment. Three HA grafts without coating cells were used as controls. Periapical x-ray and clinical parameters were monitored for up to six months. Scanning electron microscopy demonstrated that fibroblast-like cells had proliferated on the HA particles in vitro. Our results showed that the experimental group had greater pocket reduction and clinical attachment gain, and less gingival recession than did the control group at six months postoperatively. Periapical films revealed good filling of osseous defects in both groups. This study introduces a new biological approach for bringing PDL cells into intimate contact with root surfaces, in order to facilitate earlier repopulation of root surfaces with regenerative PDL cells.

Regional distribution of alpha 1-adrenoceptor subtypes in rat kidney.

alpha 1-Adrenoceptors are expressed in high density in the rat kidney. Recent studies have shown that alpha 1-adrenoceptors are heterogeneous and can be subtyped based on their affinity for the antagonists WB 4101 (alpha 1A greater than alpha 1B) and chloroethylclonidine (alpha 1B-selective). We therefore investigated the distribution of alpha 1-adrenoceptor subtypes using [3H]prazosin binding in membranes prepared from cortex, the outer stripe of the outer medulla (OSOM), the inner stripe of the outer medulla (ISOM) and inner medulla dissected from rat kidney. [3H]Prazosin binding was detectable in each region with the following maximum binding site values for [3H]prazosin (femtomoles per milligram of protein): cortex, 186 +/- 20; OSOM, 76 +/- 14; ISOM, 34 +/- 2; and inner medulla, 8 +/- 2. Pretreatment of membranes with chloroethylclonidine (10 microM for 10 min) reduced maximum binding sites to 41 +/- 4% (75 +/- 9 fmol/mg) of control in cortex, 41 +/- 9% of control in OSOM (29 +/- 8 fmol/mg) and 15 +/- 3% of control in ISOM (5 +/- 1 fmol/mg). In competition studies, WB 4101 labeled both high and low affinity sites in cortex (respective pKi values = 10.01 +/- 0.3 and 8.23 +/- 0.1) and OSOM (pKi values = 9.6 + 0.3 and 8.3 +/- 0.5), but only low affinity sites in ISOM (pKi = 8.41 +/- 0.1). The relative prevalence of high:low affinity sites revealed by WB 4101 was 53:47 for cortex, 52:48 for OSOM and virtually all low affinity for ISOM. Prior treatment with chloroethylclonidine greatly reduced the low affinity component of the WB 4101 competition curve in cortex and OSOM. We conclude that: 1) the density of alpha 1-adrenoceptors is highest in the cortex and decreases from cortex to papilla and 2) the alpha 1A and alpha 1B subtypes are approximately equally distributed in the cortex and OSOM, but the alpha 1B subtype predominates in ISOM.

[Biochemical metabolism and oxygen free radical changes following ischemic and reperfused injured limbs. An experimental study].

Oxygen free radicals may contribute to skeletal muscle damage associated with ischemia and reperfusion. This study utilized rabbit right hindlimb ischemic model to investigate the biochemical metabolism and oxygen free radicals change in limbs subjected to periods of ischemia and reperfusion. The results showed that the levels of malondialdehyde (MDA) as the oxygen free radical product in the plasma from ischemic and reperfused limb was significantly higher than that from the single ischemic limb, and varied with intracellular enzymes (CPK, LDH, GOT) which reflected the injury of skeletal muscle cells after ischemia. It Suggests that increased free radical product might be one of the important causes of ischemic and reperfused injury in limbs.
