[The surgical treatment of knee joint extensor muscle contractures].

It is reported about the results after operative mobilization of knee joint in 19 patients with contractures of extension muscles of knee joints. After operative arthrolysis and/or Payr-Plasty the range of motion has increased on average from 23 degrees to 92 degrees (a relative gain of 52%). Patients with traumatic knee contracture of extension muscle achieved an improvement in the range of movement on average by 76 degrees, whereas those with contracture due to infection improved on average by only 43 degrees. The etiology, the duration, and the preoperative loss of movement are important for success of treatment.

[Specialized and interdisciplinary team work in rheumatology].

The large number of rheumatic diseases demands a clear differentiation between the individual forms of diseases. For this purpose are at first necessary knowledge concerning clinical criteria and in the second place knowledge about serological and radiological examinations. The serological programme shall be relevant to practice, consist of a basis diagnostics and for particular differential-diagnostic problems shall include specialized laboratory investigations. Necessary X-ray pictures must be performed with a special aim and comparison of the sides. For therapy the interdisciplinary team work, consisting in the cooperation of rheumatologists, orthopedic surgeons, specialized surgeons and physiotherapists stood the test. An important prerequisite is also the cooperation of the outpatient and inpatient rheumatological institution. For a complex rehabilitation programme apart from the physicians experienced coworkers for tasks of welfare, ergotherapy and physiotherapy are necessary.

[Liver glycogen of the Wistar rat in experimental chronic cadmium intoxication under seasonal conditions].

Male Wistar-rats were used to investigate the liver glycogen pattern quantitatively under normal conditions and after oral application of CdCl2 (dosage: 50 ppm or 150 ppm) solved in drinking-water. The duration of the experiment was 1,3,6, or 12 month. The control animals have the lowest liver glycogen content at the beginning of the spring. During the summer, the liver glycogen content is increased and reached the highest level at the ending of the year. Under cadmium influence the liver glycogen content was diminished. The cadmium-effect is influenced by seasons. There is a small cadmium-effect in the time of a low liver glycogen content at the beginning of the spring. With the increasing liver glycogen content in the June, temporary the cadmium-effect is increasing too. The strongest cadmium-effect was obtained in the phase of the cadmium-effect is increasing too. The strongest cadmium-effect was obtained in the phase of high liver glycogen accumulation at the ending of the year.

[Heavy metal pattern in the liver of Wistar rats in experimental chronic cadmium poisoning].

Male young and adult Wistar-rats were used to investigate the heavy metal pattern of the liver after application of CdCl2 (dosage: 50 ppm or 150 ppm) solved in the drinking-water. The duration of the experiment was 1, 3, 6, or 12 month. Under these conditions, an increasing accumulation of heavy metal storages in the liver was obtained. The most conspicuous structure of the normal heavy metal pattern is a fine network of heavy metal storages accompanying the bile capillaries. The stellate cells (Kupffer) react positive too. Under cadmium poisoning mainly quantitative changes of this ground pattern was obtained. Additional small little storages may be scattered in the nuclei and in other regions of the liver cells. There are storages in the epithelium of small bile ducts and in the inner and the middle layer of blood vessels too. The increase of storages was medium in the first 3 month of the experiment. Fast storages were obtained after 6 and 12 month.

[Bone mineral content determination by the photon absorption technic in chronic non-occupational fluoride exposure].

Bone mineral content and bone width of radius were examined by use of bone mineral analyser in persons with long-term nonoccupational fluoride exposition. Investigations took place in 275 female and 194 male without professional fluoride influence. Comparison with normal values showed no significant differences after fluoride exposition. The meaning of these findings is discussed.

[Evaluation of the mineral content of peripheral bones (radius) by photon-absorption technique in normals as well as in patients with various types of bone diseases (author's transl)].

The evaluation of the mineral content of peripheral bones by measuring the photon absorption of the radius has proven to be a valuable method for routine clinical work: for diagnosis, follow-up and control of therapy. While there was a significant difference in the findings of normal persons compared with those of patients suffering from osteoporosis, renal osteodystrophy, osteogenesis imperfecta and skeletal fluorosis, there was no difference between normals and these patients suffering from Bechterew, Scheuermann, coxarthrosis, spondylosis, skoliosis and rheumatoid arthritis. Normal values for the mineral content and width or the radius at the junction of the middle and lower third--based on 8000 examinations--are mentioned.

[Bone changes in hemoblastoses and malignant lymphomas].

In haematological systemic diseases such as acute and chronic leukoses, malignant lymphomas, lymphogranulomatoses, osteomyelofibroses, polycythaemias and aplastic anaemias with a different proportion changes of the bones in form of osteoporoses, osteolytic processes and deformations of the vertebral bodies are to be found. The proof may be performed radiologically and histologically. In 461 patients with different haematological diseases absorption measurings of monoenergetic rays of a J-125-source were performed at the distal third of radius and ulna. It was shown that the bone mineral content of patients with proved bone destructions did not significantly differ from the normal group. The too peripherally located place of the measuring and also the late inclusion of the compacta into the changes is regarded as cause for the negative result.

Physical properties of fluorosis bone.

The bones of two patients, one with a moderate and one with a severe chronic industrial fluorosis (stage I-II and stage III), and the bones of three control persons were examined. The following parameters were determined: the fracture load, the fracture load/unit area (resistance to pressure) of the body of the first lumbar vertebra, the bending strength of the neck of the femur and of the lower third of the femur, the fracture load/unit area and the modulus of elasticity of femoral slices 2 cm thick and of precisely defined cylinders from the femoral cortex. The microhardness according to Vickers on the cross section of the femur was also determined. The results obtained are discussed with regard to fluoride therapy of osteoporosis.
