[Producing technical drawings using the feet--a special work site for a thalidomide-damaged technical draftswoman].

Persons with upper limb deficiency do not find it easy to lead a somewhat "normal" private, occupational and social life. True, society offers many kinds of assistance and support; it nevertheless is very much a matter of how the individual concerned reacts to the positive and negative influences and imponderabilities encountered. The present report sets out the efforts of a thalidomide-affected young woman who, in the face of numerous obstacles, left nothing untried to create a life basis of her own by obtaining a good vocational qualification as a technical draftswoman, regardless of her congenital upper limb amputation. Her success is proof enough that she chose the right course. This example, in more general terms, should perhaps be considered a prompting for society, on the one hand, to undertake the endeavour of providing quality assistance even in extreme situations and, on the other, to be prepared to have confidence even in those with very severe physical disablement to be able to accomplish goals which at the first glance may appear "nonsensical", such as a desire to be trained as a technical draftswoman even though one's hands are missing. The account not least shows the positive impact help and self-help may have in the rehabilitation of very severely disabled persons. The disabled person's reward primarily lies in the sense of independence achieved and in financial self-reliance, which in turn generate a sense of feeling free inwardly and outwardly.

[Genital infections and the course of pregnancy: a prospective study].

The reported study investigates the relationship of genital infections, pathobiochemical findings and demographic data to preterm labor, premature rupture of membranes (PROM) and premature delivery. The predictability of chorioamnionitis, puerperal and neonatal infections by these parameters was evaluated concurrently. 301 patients were included in this study between July 1985 and June 1986. 147 of these patients were studied longitudinally during pregnancy, delivery and puerperium (longitudinal group). A second group consisted of 154 women who presented themselves on start of labor to the labor and delivery unit of our Department (peripartal group). The incidence of preterm labor and of PROM was 26%. The incidence of premature delivery, chorioamnionitis, puerperal and neonatal infection was 11.4%, 5.5%, 7.6% and 3% respectively. Cervical colonization with Mycoplasma hominis correlated positively with PROM (relative risk 2.2), premature delivery (3.9) and neonatal infection (6.9). Chorioamnionitis, premature delivery and puerperal infection were also significantly increased in patients with positive vaginal Ureaplasma urealyticum cultures during pregnancy and delivery. Premature delivery (2.8) and puerperal infection (4.0) were associated with a vaginal group B-Streptococci (GBS) colonization during pregnancy, as was a positive GBS culture during delivery associated with puerperal infection. Bacterial vaginosis also correlated positively with premature delivery (5.6) and puerperal infection. Preterm labor correlated negatively with the socioeconomic level, PROM correlated negatively with the marital status, positively with age, a history of cervical cerclage, conization or PROM during former pregnancies. Sexual intercourse more often than once weekly during the last month of pregnancy was also associated with an increased number of PROM. Gardnerella vaginalis, Candida and Trichomoniasis during pregnancy and delivery were associated with preterm labor and puerperal infections. Levels of maternal plasma fibrinogen concentrations in patients with PROM were elevated 48 hours after delivery in accordance to the characteristics of acute phase proteins. In contrast, the maternal PMN-granulocyte-elastase concentration was significantly elevated at time of delivery and 24 hours thereafter in those patients who developed puerperal infections. The derived positive predictive value was 26%, the negative 94.7%, respectively. The overall accuracy of the prediction was 83.1%. Six out of seven mothers with neonates treated because of neonatal infection showed significantly elevated plasma concentration of PMN-granulocyte-elastase.(ABSTRACT TRUNCATED AT 400 WORDS)

[Peri-operative preventive use of antibiotics in abdominal hysterectomy].

In patients undergoing abdominal hysterectomy, the efficacy of antibiotic prophylaxis was compared to no treatment. Additional studies on the influence of risk factors such as D&C, conisation and IUD's on post-operative morbidity were evaluated. 144 patients were recruited into the study and prospectively randomised into one of four groups. Groups IV and II received 2 g i.v. Cefotetan and were comprised of patients with or without risk factors, respectively. Similarly, groups III and I, again with or without risk factors, received no treatment and served as the control groups. Infectious post-operative morbidity was significantly reduced in both groups of patients receiving antibiotic prophylaxis, the maximum benefit being observed in those patients with accompanying risk factors. The need for additional antibiotic therapy was highest in those patients receiving no antibiotic prophylaxis, i.e. groups III and I. Additionally if concomitant risk factors were present, the duration of hospitalisation was increased by a further 1.4 days.

[Functionally adapted office furniture for handicapped patients].

Individually adapted, functional office furniture is not only capable of making physically or sensorily handicapped persons more independent but also enhances their performance. These effects combine to bring about greater personal freedom, making it unnecessary for the handicapped individual to ask for help ever so often. Different types of disability may require totally different technical solutions. The present contribution deals with various office furniture programmes adapted specifically to the needs of certain types of handicap, such as those of wheelchair users, visually impaired or blind persons, hand amputees, or cerebral palsied persons. Examples are described of disability-adapted office designs both for training purposes and workplaces, including CAD or a specifically designed arrangement for technical drawing with the feet. The main objective is to show that individual functional losses can already be compensated for to a large extent by proper selection of commercially available furniture and workplace design.

[Perioperative antibiotic prevention with cefoxitin in cesarean section. Management and uses].

Antibiotic prophylaxis and febrile morbidity following non-elective Caesarean section were studied retrospectively. Febrile morbidity was found to be 5.4% in the treated group as compared to 20% in the untreated group. Percentage of endomyometritis was 2.2% in the treated group and 16% in the untreated group. Our results prove the efficacy of antibiotic prophylaxis in patients undergoing Caesarean section. Antibiotic application in 79% of all these patients led to a significant reduction in puerperal febrile morbidity and endomyometritis. Experience shows that positive results gained by the study do not have a positive feedback on the ward if there is lack of communication or reconfirmation.

Cefoxitin

[A computer-controlled telephone exchange for improving the occupational possibilities of blind patients].

Working conditions in telephone exchanges are undergoing major changes with the penetration of computer technology. Although overall telephone utilization continues to grow, more and more jobs are being lost by the impact of rationalization. Operator jobs in private exchanges, at the same time, rank high among the job opportunities for disabled people, specifically for the blind. Workstation adaptations, on the other hand, are a valuable working aid for all blind or visually impaired operators, in that they use blindness-adapted information output via the Braille terminal. Pins (dots) being used to represent written information on a braille keyboard, the device in this application is not used as an input but for output purposes. The information a sighted person reads from an LC (line) display, is literally "put at the fingertips" of a blind operator. Following an overview of technical details of both conventional and adapted exchanges, the present report deals with operator training and employment opportunities for blind persons in private branch exchanges.

[Bacterial colonization of the cervix and complications later in the course of pregnancy following cerclage].

In 107 patients undergoing cerclage operation, cervical bacterial flora was documented in a prospective study. Complications due to bacterial contamination were recorded. Postoperatively there was an increase in cervical bacteria, both in number and variety. The results were highly significant. The incidence of premature labor, premature rupture of membranes, puerperal and neonatal infections is higher than the level in our own institution and the overall level in Bavaria. Our results suggest that cerclage involves an increased risk of cervical contamination with subsequent complications during pregnancy, puerperium and newborn period.

[Accuracy and practicability of the Prevical curette for the early diagnosis of endometrial carcinoma].

.he Prevical curette is a new instrument for an early diagnosis of carcinoma of the endometrium. The good acceptance of this method, which can be practised on an outpatient basis without anaesthesia, has been described by other groups of researchers. To check its reliability, we analysed endometrium specimens of 105 patients both cytologically and histologically. The collection of the material by the cytological method was successful in 99% of the cases, whereas clinical interpretation was possible in 94%. With nine carcinomas of the endometrium in the examined women, no false negative result was obtained. Hence, the use of this curette seems to be useful in patients with special risk factors and patients who cannot be anaesthetised. It should, always be kept in mind that the Prevical curette is conceived as a screening method which cannot replace histological clarification.

[Results of the cytological examinations of cells from the uterine cavity (author's transl)].

A new method for obtained cytological material from the uterine cavity with the Exploret-Fatol is described. The results in 38 patients with atypical vaginal bleeding is reported. In 4 women the tight cervical canal necessitated general anaesthesia. In all cases adequate cytological material for examination was retrieved from the endometrium. It was not always possible to differentiate endometrium from endocervical cels by their morphology. Nevertheless all benign premalignant lesions and all malignant lesions of the endometrium were cytologically recognized. This method may find application in many high risk patients and therefore improve the early diagnosis of carcinoma of the endometrium. The methods success depends upon good knowledge of endometrial cytology by the examining cytologist.

[Integration of the handicapped into the work force using electronic data processing].

Successful settlement of disabled persons in gainful employment is closely contingent on both their training and the working environment present. For those disabled persons who cannot find jobs in the open market, it is possible to work in sheltered workshops. Founded on the initiative of parents of spastic children, the Saarbrücken Reha GmbH, a limited liability company for the sheltered employment of disabled persons, has for several years been employing disabled people in the field of text and data processing. This paper not only outlines some practical examples to illustrate suitable systems but also describes the types of tasks where good results can be achieved by the disabled employees.

Does the administration of the calcium-antagonist verapamil in tocolysis with beta-sympathicomimetics still make sense?

In tocolysis with beta-sympathicomimetics Verapamil is administered universally to reduce the subjective and objective cardiovascular side effects. The efficacy of the calcium-antagonist has not yet been substantiated. This has now re-emerged as an important issue due to the increasing occurrance of cardiopulmonary complications in tocolysis, some fetal. Nine mongrel German shepherds were anesthetized and relaxed, and their breathing was controlled at constant volume. After infusion of Fenoterol (0.06 microgram/kg/min) and subsequent administration of Verapamil (2 microgram/kg/min) a continuous record was made of the parameters of the pulmonary and systemic circulatory systems, the cardiac dynamics, the acid-base balance, the blood gases with O2 absorption and CO2 output, electrolytes and the colloid osmotic pressure. Furthermore, the intrapulmonary fluid volumes in the vascular system and the interstice were measured. During infusion with Fenoterol the changes expected from a beta-adrenergic were observed in the form of a hyperkinetic cardiac situation, metabolic acidosis and increased exchange of gases. Also pronounced fluid shifts in the lungs occurred from the intravascular space to the interstice. Subsequent administration of the calcium antagonist showed no influence on the beta-mimetic-induced changes demonstrated in the present work. The results agree with previous clinical investigations. According to the known dosage curve, the dosage of Verapamil would have to be increased multifold to achieve an effect, which is hardly possible due to the cardiodepression and peripheral circulation drop with hypotension that this would entail. The administration of Verapamil in tocolysis no longer appears expedient.

Adrenergic beta-Agonists

[Pharmacologic influencing of fetal phospholipid synthesis. Part IV: influencing the DPL(dipalmitoyl-lecithin) content of fetal lung by betamethasone (author's transl)].

Dipalmitoyllecithinic (DPL) is the quantitatively largest and most active portion of the surfactant system. Hence only a quantitative DPL determination allows a relevant statement of fetal lung maturity. The antenatal prophylaxis of RDS should result in an increase of the DPL-level in a fetal lung. A new gaschromatographical method for determination of DPL was developed. The results were secured by gaschromatography-mass-spectrometry-couplings. After treatment with betamethasone an increase of the relative DPL-amount in fetal rat lung lecithin species was found, but not an increase of the lecithin-level in the lung-tissue. Corticosteroids probably induce transesterification of the lecithins.

Animals
