[Cancer of the gallbladder].

Of 21 patients with primary carcinoma of gallbladder the diagnosis has been considered before operation in three. Nine patients underwent laparotomy only, six a palliative resection, and six a curative resection. In these three groups, 17 patients died. The mean survival time was respectively 1.5.4.3, an 8.6 months. Four patients are alive. Three of them had a resection for a carcinoma limited to the wall of gallbladder. Two patients had radiation therapy, one after palliative resection (died 11th month), the other one after biliary and digestive by-pass (alive with a follow-up of 15 months). Three patients had dosages of carcino-embryonic antigen. Our experience suggested that: diagnosis may be suggested by echography, The usefulness of carcino-embryonic-antigen levels, radiotherapy may increase the effectiveness and length of palliation.

[Pseudocysts of the pancreas simulating a renal lesion. Apropos of 2 cases].

The authors report two cases of pancreatic pseudocysts simulating a left renal pathology. Although arteriography and sonography may visualize a cyst, the key to the diagnosis is the high amylase content of the fluid collected by translumbar puncture of the pseudocyst. Correct conservative treatment implies a precise preoperative diagnosis, as abdominal exploration is apt to be confusing and often leads to the performance of an unnecessary nephrectomy.

[Sclerosing cholangitis following the surgical treatment of hydatid cyst of the liver].

The authors report a case of sclerosing cholangitis following surgical treatment of a hydatid cyst of the liver. The review of the literature demonstrates the rarity of this serious complication. The authors speculate on the mechanism of this disease: postoperative infection of the common bile duct, or chemical trauma associated with a cysto-biliary fistula. No single hypothesis seems to be able to account for all of the signs of this condition.

[Colo-anal intussusception of the adult. A new case (author's transl)].

Report on a case of intestinal obstruction due to a sigmoido-rectal and afterwards anal intussusception of a malignant tumor : review of the literature. Colo-anal intussusception remain very rare as, since Lataste in 1975 related six cases, this one is unique. On the contrary seven new cases of colo-rectal intussusception have been reported. Diagnosis can be difficult. The variability of the mass revealed by successive examination and the intussusception pictures sometime provided by the barium enema may give the correct direction. In this case, treatment consists in the perineal resection of the sigmoid intussusception and its causal tumor followed by an end to end primary anastomosis with an iliac colostomy. This treatment seems to be more suitable for old and deficient people where carcinologic problem is not of first importance.

[Subclavian-carotid anastomosis: a new technique for treating obliterations of the proximal subclavian artery (author's transl)].

A new technique for treating obliterations of the proximal subclavian artery is described. It consists of anastomosing the cervical part of the artery with the common carotid artery. The authors have used this technique in 4 patients. The operation and post-operative course were uneventful. The functional disorders regressed and velocimetric tracings returned to normal. The technique has the advantage of treating the lesions without having recourse to thoracotomy or insertion of foreign material.

[The surgical treatment of impotence of arterial origin (author's transl)].

26 subjects underwent surgery for impotence of arterial origin between January, 1976 and January, 1979. The mean age of these patients was 49.6 years. The post-operative course was followed up with a mean period of 19.6 months. The 3 epigastrico-cavernous bypasses gave no good results beyond the third month. The best results were seen in lesions situated around the ostium of the internal iliac (9 cases out of 10). The treatment of extensive aorto-iliac lesions in arterial impotence gave satisfactory results in 9 out of 13 cases of the series.

[Treatment of perforated duodenal ulcer. Reflections concerning 110 cases. (author's transl)].

Based on the analysis of 110 perforated duodenal ulcers, and the revue of the literature, the authors present the following comments: 1) The mortality in the acute phase depends mainly on the general condition of the patient; that is the reason why the Taylor's method, which is the least aggressive, is indicated in the debilitated patients, but only if it proves quickly its efficiency. In all the other cases, this method is not suitable because of its numerous early and late failures. 2) The immediate and late results of the simple suture of the ulcer are not always beneficial to the patent. 3) The good results of the elective surgical treatment of the peptic ulcer disease are also found in the patients operated on for acute perforation. The authors propose a therapeutic protocole in which the indications depend more on the general state of the patients than on the classical criteria, and in which a surgical approach to the peptic ulcer disease is emphasized.

[Conservative surgical treatment in stage 4 arteritis of the lower extremities. Apropos of 26 cases].

The authors report 26 cases (average age 69 years) of stage IV atheroma of the leg arteries for which a conservative surgical operation was attempted. Aortography (18 times) or femoral arteriography (8 times) were performed without difficulty before operation. During the same operation, a single procedure was performed in 15 cases, 2 or 3 procedures were performed in 11 cases. Thus were carried out: sympathectomy in 7 cases, endarterectomy in 8 cases, a by-pass operation in 23 cases. There were 4 deaths (15%) during the postoperative period. In 18 of the 22 patients who survived the operation, trophic disorders completely disappeared, and the functional result assessed with an average follow up of 15 months (1 to 25 months) on the degree of intermitent claudication was judged good or very good in 16 cases, mediocre in 2 cases. In 4 of these 22 patients, the trophic disorders persisted or reappeared in the short term and amputation was then carried out in 3 of these 4 cases. Conservative surgery remains possible and useful in stage IV arterial disease of the lower limbs. It is however important to operate early in the course of the disease after failure of adequate medical treatment.

[Inflammatory tumors of the cecum].

The authors study 19 personal cases of inflammatory tumour of the caecum and 60 cases found in the world literature over the same period. Without disputing the definition and classification of these inflammatory tumours which were perfectly drawn up by Bremen and Flamand in 1958, they emphasize the difficulty in determining the precise etiology. Struck by the large number of right hemicolectomies carried out for often benign lesions, they propose in order to reduce their number, the use of new investigations, such as selective arteriography, colonoscopy, and attempt whenever possible, to reduce the inflammation by medical treatment.
