[Clinical study of metastasis of thyroid differentiated carcinoma].

The Authors report 5 cases of distant metastases of well-differentiated thyroid carcinoma over a number of 114 patients operated for thyroid neoplasm (4.3%). In 101 pts was total thyrodectomy performed, in 13 pts underwent istmo lobectomy. Distant metastases were verified in 3 patients with follicular and in 2 patients with papillary carcinoma. In 2 cases the metastases were in lungs, in 2 cases are the bone, and in one patient in lungs and bones. In 3 patients istmolobectomy had been performed at first operation. All the patients were treated with radioiodine. Three patients were alive after 3, 5 e 10 years from the diagnosis of metastases, 2 of them are presently disease free.

[Reoperations in thyroid surgery].

The Authors examine etiology, incidence and hazards related to reoperation in thyroid surgery either for benign either for malignant pathology. The Authors present, then, 26 secondary operations performed in the Unit of Surgical Pathology of the Department of Surgical Sciences Chieti University: 15 for benign, 11 for malignant pathology. In the cases of thyroid malignant pathology, in 2 patients (operated in different hospitals) there was a parenchimatous recurrence, in 2 patients a lymphnodal recurrence. In all the patients a total thyroidectomy was performed, in one patient associated with lymphadenectomy. In the patients with lymphadenomatous recurrence a functional laterocervical lymphadenectomy was performed.

[Medullary carcinoma of the thyroid].

Medullary thyroid carcinoma represents 4-8% of thyroid malignancies; regional nodal involvement is commonly found, even at the time of first surgical approach. In these patients after surgical resection, other therapeutic choices are really limited. The Authors evaluated their experience in the period 1981-1994, concerning 14 patients, 9 females and 5 males, all affected with sporadic MTC. Diagnosis was preoperative in 10 patients and intraoperative in 4; as for the staging: in 8 patients the tumor resulted confined to the thyroid, while in the remaining 6 patients it was extra-thyroidal. In 5 patients a total thyroidectomy with regional lymphadenectomy was performed, with in 6 patients a total thyroidectomy with cervico-mediastinic total lymphadenectomy was achieved. Survival was evaluated with a short, follow-up (3 months) and calculated using the K-Mayer method. Average survival was: 5 years in patients undergoing only total thyroidectomy; 3 years and 1 month in patients undergoing total thyroidectomy and lymphadenectomy; 6 months in patients undergoing no treatment at all. Among the factors negatively affecting patients survival, the Authors pointed out: metastasis occurrence; incomplete resection and mediastinal metastasis. In conclusion surgical resection (total thyroidectomy) associated with radical lymphadenectomy resulted to be a good approach. However, prognostic factors require further investigation.

[Prognostic factors in gastric carcinoma].

A personal experiment conducted on 80 gastric carcinoma patients requiring radical surgery at the Special Surgical Pathology Institute is reported. The aim of the study was to identify and assess the factors capable of influencing prognosis e.g. sex, age, TNM, type of operation, symptoms etc and to make a comparison with data from the international literature.

[Autonomous adenoma of the thyroid].

Toxic goitre often referred to in the literature as "autonomous" adenoma or nodule actually signifies a number of different pathologies that are characterised by autonomy and hyperfunction so that it is now thought more appropriate to call it an autonomous follicle. Clinical features and the hormone profile suggest the diagnosis which is confirmed by scintiscan with curve as well as dynamic inhibition and stimulation tests using TRH, procedures that reveal both latent hyperfunction and autonomy. The most common current therapeutic approach is surgical since this provides a complete cure while leaving the surrounding thyroid tissue undisturbed.

[Diagnosis of thyroid nodules].

The basic features of the most common endocrine pathology, the thyroid nodule are analysed. After a brief assessment of the incidence of the condition the problem of clinical and instrumental diagnosis is tackled with the aim of demonstrating the value and possibility of sub-clinical diagnosis of micronodules. The present work aims to identify the fastest, most efficient and cheapest diagnostic procedure.

[Antibiotic prophylaxis in the implantation of permanent pacemakers].

Prophylactic antibiotics are prescribed frequently at pacemaker placements despite a paucity of data indicating effectiveness. Battery pack replacement procedures usually are not followed by infections and prophylactic antibiotics in this setting are controversial. Pacing wire and battery pack placement have higher infection rate (about 5%), thus antibiotics may be useful in the prevention of serious infections. In this study three different schemes of antibiotics prophylaxis (local prophylaxis, Short term prophylaxis, conventional postoperative prophylaxis) were compared in 72 patients receiving permanent transvenous cardiac pacemakers. No significant differences were found among these three groups also if S.T.P. showed a greatest simplicity.

[9 years of electric stimulation in Abruzzo. cognitive findings].

Data relating to 9 years of definitive electrostimulation in the Abruzzo region are presented. An account is given of the epidemiology, the indications for implantation, the stimulation and control technology, and the complications. It is pointed out that the implantability index rose from 1976 to 1978 to reach values of 130.5 implants/mil/inhab. This is in comparable with that in the literature. The technologies and modalities are rapidly developing, and a definitive picture cannot be drawn as yet. Further cognitive investigations are envisaged.

[Comparison between clinical and electrophysiological data before and after definitive pacemaker implantation for the sick sinus syndrome].

Personal observations of 22 patients with permanent inhibited ventricular PM for SSS during the period July 1976-February 1979 and re-examined with electrophysiological study after a follow-up made for an average period 25,5 months (14 to 49 months), after PM inhibition from the outside are presented. Patients were divided into 2 groups (group 1 with bradycardia and group 2 with brady-tachy arrhythmia). In spite of relative exiguity and uniformity of this casuistry, the conclusion is drawn that CSNRT and atropine test were more significant than SACT, in particular in forms with less pathological electrophysiological parameters.

[The sick sinus syndrome. Our experience with permanent electrostimulation].

Our study concerns 20 patients affected by "Sick Sinus Syndrome", these has been observed in our Coronary Care Unit during the period 1-7-1976 to 26-4-1979. All patients has been submitted to an electrophysiological study of the conduction system; of those only 16 has been treated with a permanent PMK, the other 4 with medical therapy. Our study's results underline the problematic nature of the "Sick Sinus Syndrome" and underline its etiopathogenetic, diagnostic and therapeutic aspects.
