[Effects of mesna (2-mercaptoethane sodium sulfonate) in children with malignant disease receiving oxazaphosphorine chemotherapy].

We studied the efficacy of mesna as a protectant for urotoxicity in pediatric patients receiving chemotherapy including oxazaphosphorines. Nineteen patients with malignant diseases (5 neuroblastoma, 3 acute lymphocytic leukemia, 4 acute non-lymphocytic leukemia, 2 non-Hodgkin lymphoma, 3 osteosarcoma and 2 rhabdomyosarcoma) were treated with a total of 106 courses of therapy between June of 1986 and May of 1989. Of these, no gross hematuria were seen. Microhematuria transiently occurred only in 2 courses (5%) of 1 patient (2%). These data indicated that mesna was highly effective for urotoxicity of oxazaphosphorines without any side effects, especially in pediatric patients.

[Selective injury of the vestibular and facial nerves following microvascular decompression procedures for hemifacial spasm: A case report].

Microvascular decompression procedures were carried out to relieve right hemifacial spasm in a 54-year-old man. His facial spasm was satisfactorily cured but he complained of severe vertigo, and slight facial palsy was also shown after the operation. Electronystagmograph disclosed spontaneous horizontal and rotatory nystagmus toward the non-operated (left) side. The postoperative caloric test showed profound dysfunction on the operated side. His hearing was well maintained postoperatively. These findings indicated, in addition to facial nerve injury, that the peripheral portion of the vestibular nerve was selectively damaged by the operative manipulations. It was shown that both the facial and vestibular nerves tended to be injured when the nerve trunk was shifted caudal-to-rostrally due to manipulations in the CP angle. It was stressed that caution should be paid to preserve not only the cochlear but also the vestibular nerve function in cerebellopontine angle manipulations.

[Malignant lymphoma of the central nervous system in a boy with immotile cilia syndrome].

Malignant lymphoma of the central nervous system in a thirteen-year-old boy with immotile cilia syndrome (ICS) is reported. He had frequent upper respiratory tract infections, chronic sinusitis and pneumonia during in childhood. Bronchiectasis was demonstrated by bronchography. The diagnosis of ICS was confirmed by the lack of dynein arms of cila in the nasal mucosa with electronmicroscopy. In 1987, he complained of headache and vomiting and a space occupied mass lesion in the left frontoparietal lobe was found by head CT scan, which was subtotally resected. Histological studies showed large cell type non-Hodgikin lymphoma of B-cell phenotype. He received radiotherapy (41Gy) to the whole brain and systemic chemotherapy consisting of adriamycin, cyclophosphamide, vincristine, prednisolone, L-asparaginase and intrathecal methotrexate, and the patient maintained complete remission for eight months. However, relapse occurred and the patient died twelve months after the initiation of treatment.

Fatty acid modification of cultured neuroblastoma cells by gamma linolenic acid relevant to its antitumor effect.

Two human neuroblastoma cell lines, NCG and GOTO, were used to study the cytotoxic effect of gamma linolenic acid (GLA). The cell growth inhibition of these culture cells by GLA was found to be associated with striking membrane fatty acid modification. When culture cells were exposed to 20 micrograms/ml and 60 micrograms/ml GLA for 48 hr, polyenoic acids in cell membrane phospholipids (PC, PE, PI, PS) and triglyceride significantly increased; 1.8-21.0 fold for NCG and 1.04-11.5 fold for GOTO, in association with decreased monoenoic acids. The most remarkable changes were; increase of C18:3, C20:3, C20:4 and decrease of C18:1. CoQ10 (50 micrograms/ml) and vitamin E (10 microM) shown to protect against the cytotoxic effect of GLA did not modify the incorporation of GLA into tumor cells. These results indicate that the antitumor effect of GLA is probably due to cellular dysfunction caused by fatty acid modification after GLA incorporation.

The value of 'Trapezoid Rotation Test' by Contraves computerized rotary chair system.

Trapezoid Rotation Tests using the Contraves computerized rotary chair system were performed in 60 normal subjects and 213 patients with peripheral vestibular disorder. In normal subjects, directional preponderance was not observed. The so-called 'response decline phenomenon' by repeating the tests was not detected and the test results showed good reproducibility. On the other hand, the test results of patients were variable, in close relationship with the state of vertigo, and reflected patient's labyrinthine pathology so accurately that the inspection of the treatment course was possible by this testing system.

OKT4 epitope deficiency in a patient with acute myeloblastic leukemia.

The association of OKT4 epitope deficiency and acute myeloblastic leukemia (M1 by FAB) in a 12-year-old girl is reported. The patient's peripheral lymphocytes lacked the antigen identified by the OKT4 monoclonal antibody, but showed normal OKT4A and Leu-3a reactivity. Functional studies of the lymphocytes revealed normal responses to mitogens, but slightly reduced helper T cell function. The patient responded well to antileukemia chemotherapy and has been in remission for 30 months. The possible link between OKT4 epitope deficiency and the pathogenesis of AML is discussed.

Hypercalcemia in a case of childhood acute lymphoblastic leukemia.

Severe hypercalcemia (serum calcium, 4.25-5.25 mmol/l), in association with osteolytic bone lesions, was found in a girl aged 2 yr 7 mo with common acute lymphoblastic leukemia (ALL). Hormonal studies excluded the possibility of the hypercalcemia being caused by primary hyperparathyroidism or ectopic parathyroid hormone secretion. Increased plasma prostaglandin E2 (PGE2) levels (130 ng/l), probably produced by leukemic cells, were considered to be one of the pathogenic mechanisms responsible for the occurrence of hypercalcemia in this patient. Both the hypercalcemia and the abnormal plasma PGE2 level returned to normal after chemotherapy.

[Therapeutic results in 22 cases of childhood non-Hodgkin's lymphoma treated by ACOP combination chemotherapy].

Twenty-two cases of childhood non-Hodgkin's lymphoma were treated during the past 8 years from 1979 to 1986. Median age was 7 years 4 months and the age range was from 2 years 4 months to 14 years 9 months, the male to female ratio being 18 : 4. Primary sites were common in the mediastinum (n = 7) and abdomen (n = 6) followed by submandibular lymph nodes (n = 3). According to Murphy's staging system, 7 were in stage I/II and 15 in stage III/IV. Histological studies of lymphoma tissues and blasts in the cerebrospinal fluid, pleural effusion or ascites revealed that 9 cases were lymphoblastic lymphoma while only 3 were defined as Burkitt's lymphoma. With our protocol of systemic combination chemotherapy (i.e. ACOP), intrathecal methotrexate and/or involved field irradiation, twenty-one out of the 22 cases (93%) attained complete remission and 14 patients are currently alive in continuous complete remission, with a median survival of 28+ months (range 6+ approximately 76+ months).

Antitumor effect of gamma-linolenic acid on cultured human neuroblastoma cells.

gamma-linolenic acid (GLA) was found to suppress the cell growth of 4 human neuroblastoma cell lines. GOTO was the most sensitive, followed by SK-N-DZ and NKP, while NCG was much less sensitive to GLA. In terms of GLA cytotoxicity, neither cyclo-oxygenase inhibitors nor a lipoxygenase inhibitor showed any effect. On the other hand, 4 antioxidants (Coenzyme Q, (D) alpha-tocopherol, (DL) alpha-tocopherol, butylated hydroxytoluene) reduced the growth inhibitory effect of GLA, but not in proportion to the decrease of GLA-stimulated lipid peroxidation. Accordingly, prostaglandins and leukotrienes probably do not play a role, and lipid peroxide may only be partly involved in the GLA effect.

[Improved results of treatment in 30 cases of acute nonlymphocytic leukemia in childhood].

Thirty cases of childhood acute nonlymphocytic leukemia (ANLL) have been treated with intermittent, intensive multi-drug combinations during the past 5 years. Three patients never attained complete remission (CR) and 3 died of complications after less than one month of treatment. Twenty-four (86.7 %) attained CR with a median CCR of 26 months. The median for disease-free survival (DFS) among the total 30 cases was 15 months. Twelve out of the 24 CR patients have been disease-free for longer than 16 months, indicating significantly improved therapeutic results. Among the factors affecting prognosis, we found that patients with certain clonal abnormalities of leukemic cells and initial leukocytosis (greater than 40,000/microliter) had poor prognosis. Further revision of treatment should be considered for patients with high-risk ANLL.
