Evolution of DNA content in small and well-differentiated hepatocarcinoma.

DNA ploidy of 10 well-differentiated hepatocarcinomas was evaluated by densitometry on smears obtained by fine needle aspiration biopsy (FNAB) with ultrasound guidance. Because of diagnostic difficulties or an increase in tumor size, FNAB was repeated once in six cases and twice in four. All the material collected was then analyzed to study the variation of DNA content over time in these neoplasms. The study was performed on Feulgen-stained slides using an IBAS image analyzer system. DNA ploidy was evaluated by visual interpretation of the corresponding histograms and by calculating the 5c exceeding rate (5cER), 2c deviation index (2cDI) and DNA malignancy grade (DNA-MG). In the first samples seven cases were euploid (four diploid and three polyploid), with 5cER < 5; three cases were aneuploid, with 5cER > 5. In the second samples, taken after four or six weeks, eight cases became aneuploid, with 5cER > 5, and two cases were polyploid, with 5cER > 5. Four cases in which a third FNAB was performed resulted in aneuploidy, with 5cER > 5. A progressive increase in aneuploidy, 5cER and DNA-MG values was observed in six cases in the second FNAB and in one case in the third FNAB along with the increasing size of the nodules. In only one case we observed a paradoxical reduction in the 5cER in the third FNAB; the corresponding histogram remained aneuploid. Limitations could have occurred because of sampling small nodules under ultrasound guidance and because of the relatively small number of cells measured. DNA evaluation seems to be useful in the diagnosis of hepatocarcinoma and in the study of its biologic features.

[Calcifying odontogenic epithelial tumor or Pindborg's tumor. Apropos of a case].

Calcifying odontogenic epithelial tumor is a rare affection, representing only 1% of all odontogenic tumors. Histology shows polyedral epithelial cells, organised in layers, strands or nests or, more rarely present as individual cells. The cell nuclei are polymorphic and hyperchromatic with projecting nucleoli. An amorphous substance presenting specific immunohistochemical characteristics surrounds the cellular elements. It appears to be related to amyloid and is also considered to be the product of degeneration of the cell basal lamina and/or the product similar to the dentine or cement of the tumor cells. Surgical treatment varies from simple enucleation to more or less partial resection of the affected bone.

Fine-needle aspiration biopsies of breast masses. An additional experience with 1153 cases (1985 to 1988) and a meta-analysis.

A series of 1153 fine-needle aspiration biopsies (FNAB) of breast masses obtained during the period 1985 to 1988 in one institution was examined. The following diagnostic accuracy values were found: sensitivity, 93.0%; specificity, 92.4%; predictive positive value, 93.7%; and efficiency, 92.8%. These results were compared with a previously published series for the period 1976 to 1984. The diagnostic accuracy values did not differ significantly between the two series.

Ovarian serous tumor with mural nodules of carcinomatous derivation (sarcomatoid carcinoma): report of a case.

An ovarian serous cystic tumor of low malignant potential with solid mural sarcoma-like nodules is reported. This tumor metastasized to the ileal wall. The nodules in the ovary and in the ileal wall were made up of highly malignant cells with admixed osteoclast-like multinucleate giant cells. Immunohistochemical reactions demonstrated the epithelial differentiation of the tumor. We propose that the term "sarcomatoid carcinoma" of the ovary should be adopted for these lesions.

Comparative morphometric analysis of aggressive and ordinary basal cell carcinoma of the skin.

A morphometric analysis was performed on histologic sections of 11 cases of aggressive basal cell carcinoma that recurred and/or metastasized after the first diagnosis (BCC2) and on 11 ordinary basal cell carcinomas (BCC1). The considered parameters were as follows: nuclear area, perimeter, maximum diameter, feret x, feret y, form perimeter, and form area. The results show numeric differences between the two groups for the first five parameters. A statistical analysis was performed between BCC1 and BCC2 for all the considered parameters using a two-sided t test for independent samples. The test showed significant differences between the first five parameters except for the two form factors. A multivariate analysis was performed using area and perimeter values of the compared groups that amplified the discrimination threshold between the two groups. The authors conclude that a morphometric assessment may bring significant contribution in the knowledge and in the outcome prediction of basal cell carcinoma.

Percutaneous intralesional brushing of cystic lesions of bone: a technical improvement of diagnostic cytology.

A modified bronchial brush is presented as a new cytologic method to improve the value of fine-needle aspiration biopsy in the diagnosis of cystic lesions of bone. Four female and six male patients ranging in age from 10 to 44 years were studied. In all cases intralesional brushing was percutaneously performed under fluoroscopic control with local anesthesia. Fine-needle aspiration biopsy preceded intralesional brushing and was performed as described elsewhere. The brush was driven into the walls and septa of an osteolytic lesion through a 14-G needle; material obtained was fixed and processed for cytological and/or histological examinations.

Fine needle aspiration cytology of medullary thyroid carcinoma: a review of 18 cases.

A series of 18 consecutive medullary thyroid carcinomas (MTC) diagnosed by fine needle aspiration cytology (FNAC) is described. The most important diagnostic cytologic criteria were the dispersed cell pattern, the polygonal appearance of the cells, binucleated cells and the presence of amyloid. Other less common cytologic features are reported and the variable microscopic appearance of MTC is pointed out. The possibility that this cytologic variability of MTC may be by itself an important diagnostic feature is proposed.

Hodgkin's disease mimicking suppurative lymphadenitis: a possible pitfall in fine-needle aspiration biopsy cytology.

We report one case of nodular sclerosing Hodgkin's disease with uncommon pseudosuppurative presentations in fine-needle aspiration biopsy (FNAB) samples and on histopathologic material. The criteria for cytologic diagnosis include atypical mononuclear cells and diagnostic SR cells in the purulent exudate. This lesion is considered a possible pitfall in FNAB cytology of Hodgkin's disease.

Fine-needle aspiration biopsies of breast masses. A critical analysis of 1956 cases in 8 years (1976-1984).

A series of 1956 fine-needle aspiration biopsies (FNAB) of breast masses is described. The diagnostic accuracy of this series (sensitivity, 95.7%; specificity, 89.6%; predictive value of positivity, 95.9%; and efficiency, 94.0%) was compared with that published in the literature. Statistically significant differences were found between the level of diagnostic accuracy in series published by pathologists who diagnosed smears prepared by clinicians and surgeons (Group A), and those published by pathologists who performed palpation and aspiration, and made the cytologic diagnosis by themselves (Group B). These differences mainly consisted of a lower number of false-positives and "unsatisfactory" samples in Group B series.

The role of morphometry in the cytology of well-differentiated hepatocarcinoma and cirrhosis with atypia.

A morphometric study was performed on 200 nuclei per case in six well-differentiated hepatocarcinomas and in six cirrhoses with cytologic atypia, using samples obtained by fine needle aspiration (FNA) biopsy of the liver. The parameters measured were the nuclear area, the nuclear perimeter and the maximum nuclear diameter. The nuclei of well-differentiated hepatocarcinomas could be distinguished from those of cirrhoses on the basis of the larger size and greater anisonucleosis of the former. A statistical analysis (using a two-sided t-test) of the means of the parameters showed significant differences between the two diagnostic groups. These results suggest that morphometric analysis can help in the differential diagnosis between well-differentiated hepatocellular carcinoma and cirrhosis with cytologic atypia in FNA biopsy samples.

Malignancy-associated cellular markers (MAC) in oral and bronchial epithelial cells in sputum specimens: possible morphologic marker for high-risk groups (asbestos-exposed workers).

To verify a possible role of malignancy-associated cellular markers (MAC) in high-risk groups, the authors reviewed a total of 291 consecutive sputum specimens from 97 workers exposed to asbestos. The asbestos workers were matched according to smoking habits and cellular changes. Twelve subjects (12.3%) had MAC in epithelial cells; eight were smokers, four nonsmokers. Among MAC+ smokers, three sputum specimens contained cells of squamous metaplasia and one had cells from carcinoma in situ. Two MAC+ nonsmokers had cells of squamous metaplasia, too. In addition, MAC+ cells were also identified in four inflammatory samples, belonging either to smokers or nonsmokers. Two MAC+ subjects had a negative sputum specimen. In keeping with these results, the authors believe that MAC evaluation in sputum specimens might be of help in the oncologic follow-up of asbestos-exposed workers.

Mucoid carcinoma of the breast on fine-needle aspiration biopsy sample: cytology and ultrastructure.

16 cases of mucoid carcinoma of the breast were diagnosed in specimens obtained by fine-needle aspiration biopsy. The subsequent tissue examination confirmed the cytologic diagnosis in all 16 cases but revealed in 1 case an association with an endocrine component and in 3 cases the presence of an invasive ductal component. The diagnostic and prognostic necessity of distinguishing pure forms of mucoid carcinoma from those associated with invasive ductal carcinoma or endocrine differentiation means that investigations other than fine-needle aspiration biopsy are also required. In every case, the cytologic diagnosis of mucoid carcinoma should be considered as an aspect of the tumor and not representative of the entire neoplasm.

Application of plastic embedding to fine needle aspiration biopsy.

OBJECTIVE: To explore the possible advantages that plastic embedding offers to fine needle aspiration biopsy (FNAB). STUDY DESIGN: A series of 54 FNABs was fixed in Kryofix (30 cases) and acetone (24 cases), plastic embedded and cut using a rotative microtome. The 2-micron sections obtained were May-Grünwald-Giemsa and Papanicolaou stain in all cases; mucicarmine, Alcian Blue, periodic acid-Schiff (PAS), PAS-diastase and Feulgen stain for DNA cytometric evaluation were also applied in five cases. Immunocytochemical staining using the antibodies cytokeratin 1-8, epithelial membrane antigen, vimentin, Ki-67, lysozime, calcitonin and carcinoembryonic antigen was carried out with the immunoalkaline phosphatase method. RESULTS: The fragments in the sections were perfectly two dimensional; the architectural features of the original tissues were preserved. Morphologic details of nuclear membranes, nucleoli and mitotic figures were excellent. Cytochemical and immunocytochemical stains were successful in all cases. Immunostains showed the precise location of the signal (nuclear, nucleolar or cytoplasmic), a clear background and preservation of morphology. CONCLUSION: Plastic embedding can be applied to FNAB to solve the problem of dense cellular groups, to study structural and cellular details, and to capitalize on the diagnostic material available for serial cytochemical, quantitative and immunocytochemical purposes.

Apoptosis and cytologic differentiation in hepatocellular carcinoma on fine needle aspiration samples.

OBJECTIVE: To evaluate possible alterations of the mechanisms leading to apoptosis in hepatocellular carcinoma by studying bcl-2 expression on fine needle aspiration biopsy (FNAB) samples using immunocytochemistry. STUDY DESIGN: The study was performed on a series of 84 hepatocarcinomas aspirated under ultrasound guidance. A Papanicolaou-stained smear for each case was destained and restained for bcl-2 by using the immunoperoxidase technique. bcl-2 Expression was then correlated with cytologic grading and the size of the tumor. RESULTS: In 16 cases (19%), bcl-2 immunostaining gave a specific cytoplasmic signal. Fourteen of these positive cases were well differentiated, and two were pleomorphic tumors. Six positive cases were smaller than 5 cm, 3 were larger than 5 cm, and 7 were diffuse. CONCLUSION: A significant percentage of hepatocellular carcinomas produce and accumulate bcl-2 protein in cell cytoplasm. bcl-2 Expression can be detected on destained cytologic smears. bcl-2 Expression seems to correlate with the cytologic degree of differentiation but not with the size of the tumor.

Fine needle aspiration biopsy diagnosis of giant-cell tumor of bone. An experience with nine cases.

The cytologic findings in nine cases of giant-cell tumor (osteoclastoma) of bone diagnosed by fine needle aspiration (FNA) biopsy are described. The aspirates contained a dual population of cells, consisting of mononucleated tumor cells and multinucleated giant cells (osteoclasts). The tumor cells were usually evident in the FNA smears as cohesive perivascular clusters; some were present as sheets or single cells. The most striking feature, which may best suggest the diagnosis of giant-cell tumor in an FNA smear, was the attachment of the osteoclasts to the cohesive groups of tumor cells. Even when the cells were more dissociated, there was a rather regular distribution of the two types of cells. This series further demonstrates that FNA biopsy permits an accurate cytopathologic diagnosis of giant-cell tumor and that its use can clarify the clinical and radiologic impressions of these bony lesions preoperatively. The differential diagnosis between osteoclastoma and other giant-cell containing bone lesions is discussed in detail.
