[Improved method of dermatoplasty].

For restoration of dermal integument on hands, feet, major joints, face and neck in 104 patients special device was used which provided creation of split dermal flaps of new types--with fenestrated gaping perforations or entire flaps with areas of various thickness which enabled to preserve islands of full thickness skin at donor's sites or superficial dermal layers. The thickness of the transplants made up 0.4-0.6 mm in adults and 0.25-0.5 mm in children. Perforated flaps were transplanted in 128 cases, the flaps of variable thickness--in 31. 151 operations (95.0%) resulted in complete recovering of the transplants, all wounds at donor's sites have closed spontaneously. Through 3-17 years after the operation the scars at perforations' sites looked hardly out of the ordinary, the areas of smaller thickness are unnoticeable, skin-grafts are soft and loading-proofed. The decrease in complications' rate and improvement of functional and aesthetic results of treatment after the application of the device were stated.

[Classification and selection of treatment of chronic gastroduodenal ulcer complicated by hemorrhage in elderly and senile patients].

The classification of chronical bleeding gastroduodenal ulcer in elderly and senile patients, considering the blood loss severity and concurrent pathology, was proposed. The tactics of treatment--conservative, operative, or actively-waiting one--was determined according to the classification adduced. The proposed classification application and the tactics of treatment also have permitted to reduce mortality from 15.7 to 11.2%.

[Intratissue electrophoresis in patients with appendicular colic and a preliminary diagnosis of non-destructive acute appendicitis].

The possibility of the use of intratissue electrophoresis in patients with presumptive diagnosis of acute appendicitis was studied. Use of the method suggested contributes to enhancing the effectiveness of the treatment performed, prevents unsubstantiated performance of appendectomy, shortens time patients stay in a hospital. The long-term results of treatment with the use of intratissue electrophoresis are better than those in use of the existing methods of dynamic observation (parenteral administration of spasmolytic and desensitizing agents).

[Foreign bodies of the stomach and intestines].

The experience with surgical treatment of foreign bodies of the stomach and intestine in 127 patients is summarized. In presence of acute foreign bodies, the use of drugs increasing intestinal peristalsis, or causing vomiting is not recommended.

[Programmed laparotomy in the diagnosis and therapy of pathology of the abdominal cavity].

The authors analyse the performance of programmed laparotomy for dynamic visual observation over the condition of the abdominal organs in 3 patients when (as they claimed) laparoscopy would have produced no effect. The article reports on the efficacy of programmed laparotomy conducted by the method suggested by the authors in 15 patients with purulent peritonitis for repeated cleansing of the abdominal cavity.

[Closed thoracic trauma and concomitant diseases].

Examined were 340 patients with closed thoracic trauma and posttraumatic pneumonia. Often, this complication was revealed in the elderly and senile. Chronic non-specific diseases of the lungs, pneumothorax predisposed to occurrence of posttraumatic pneumonia. The methods for treatment of a given complication have been developed. Diseases of the heart, vascular system in thoracic trauma can cause the aggravation of a patient's state up to lethal outcome.
