[The thromboembolic syndrome in the surgery of rheumatic heart defects].

An experience with the treatment of 380 patients with rheumatic heart disease has shown that late stages of the disease are necessarily accompanied by the thromboembolic syndrome having exact clinical manifestations and definite stages. Pathogenesis of the thromboembolic syndrome is based on changes of the vascular bed, microcirculation and hemocoagulation parameters. So, control of factors of thrombus formation, prophylactics and therapy of thromboses and embolism are necessary in the process of surgical treatment of acquired heart diseases.

[Surgical treatment of postoperative ventral hernia using a modification of the Napalkov method].

A new modified method of plasty of practically all sizes of postoperative hernias in different areas of the abdomen has been developed on the basis of N. I. Napalkov's principle which is based on doubling aponeurosis and creating muscular defense of the aponeurotic suture. Clinical experience of the treatment of 125 patients with postoperative ventral hernias shows its use to be promising and expedient for a wide introduction into clinical practice.

[Immediate results of closed correction of recurrent mitral valve stenosis in the late stages of the disease].

Immediate results of closed surgical correction of recurrence of mitral stenosis in 127 patients at late stages of the disease performed in 1957-1986 are described. Closed re-commissurotomy proved to be possible in 98.5% and was effective in 80% of the patients operated upon. The complications were related with hemorrhage, acute cardiac failure. The intrahospital lethality was 11.8%. Immediate improvement was obtained in 70.1% of the patients after operation. A conclusion was made that closed commissurotomy could be used with sufficient effects in cases where the artificial circulation was impossible.

[Hemolysis indicators in the evaluation of the function of artificial heart valves after heart valve prosthesis].

Hemolysis of blood was studied in two groups of patients: the main group consisted of 75 patients with mitral, aortal and two-valves prostheses; a control group had 15 patients with rheumatic valve defects without prostheses. It was shown that hemolysis was minimal in patients with a satisfactory function of prostheses. It was higher after double prostheses and especially in patients with dysfunction of the valves. Lactate dehydrogenase is thought to be the most important and reliable test for the assessment of the hemolysis degree.

[Chronic cardiac insufficiency at the decompensation stage and the surgical treatment of mitral stenosis].

A classification of chronic heart failure was made on the material of surgical treatment of 290 patients with rheumatic mitral heart defect with prevailing stenosis. Three stages of chronic heart failure at the decompensation stage were found: I--unstable removable; II--stable removable; III--stable not completely removable. In the Ist stage the authors recommend operations by both closed and open techniques; in the IInd stage closed methods are thought to be preferable (open operations may be used in single cases); in the IIIrd stage only closed methods are possible.

[Determination of the input and output volume of the right ventricle based on cineangiocardiography data].

The authors have developed a method of separate determination of the volumes of the input and output portions of the right ventricle by findings of cineangiocardiography made in two projections. The volume of the input portion had been shown to be 1,23 times as great as that of the output portion. A comparison of measurements of the volumes of the input and output portions and the total right ventricle as a whole will reveal their participation in the compensation of congenital and acquired heart diseases.

[Microcirculatory disorders in mitral stenosis surgery].

The clinical investigation of microcirculation in 100 patients with rheumatic mitral stenosis performed by clinico-morphological and laboratory methods has revealed changes of microcirculation in 100% of cases. Three degrees of disturbances of microcirculation are shown. The authors believe microcirculation disturbances to be responsible for severity of the patient's state and effectiveness of surgical treatment.

[Pain as a stimulator of protective and curative processes (the theory of pain)].

The work deals with the theoretical approach to problem of pain. The mechanism of the appearance of pain is considered as lack of correspondence between functional capacity of the nervous system and the presented load. The function of pain as reparator and stimulator of defensive forces of the organism in pathological processes is disclosed. The possible employment of pain as a curative factor in practical medicine is discussed.

[Exposure to external pain in the treatment of bronchial asthma].

Immediate results of treatment of 55 patients with bronchial asthma of different severity by a method of the exposure to external pain are described. The method is shown to be efficient in bronchial asthma of any genesis, its use to be promising for overcoming any drug (even hormonal) dependence.
