[Key words, essential tools for bibliographic research: analysis of usage in Archivos de Bronconeumología for respiratory system knowledge areas].

OBJECTIVE: To analyze key word usage in Archivos de Bronconeumología, by comparing words used in the journal to those used in the Index Medicus database within various respiratory system knowledge areas, and to determine whether usage has changed over time. MATERIAL AND METHODS: Original articles published in Archivos de Bronconeumología from 1994 through 2001 were reviewed manually to gather the key words used. The list was translated to English and then compared to the medical subject heading (MeSH) terms used in the PubMed Browser. RESULTS: Seven hundred six original articles published in the study period used a total of 1163 key words. Matches with MeSH terms were found for 62% (n=46) of the key words in smoking research, 48% (n=52) in asthma, 39% (n=82) in respiratory insufficiency and sleep disorders, 60% (n=49) in diagnostic and treatment techniques, 61% (n=35) in tuberculosis, 65% (n=87) in nontuberculous infections, 61% (n=121) in oncology, 60% (n=37) in circulation, 55% (n=47) in pleural diseases, 48% (n=21) in pathophysiology, and 64% (n=68) in interstitial diseases. We did not see a clear tendency in the evolution of the journal's key word usage for the knowledge areas analyzed during the study period. The percentage of matching key words held steady around 50% over the last 3 years. CONCLUSIONS: Respiratory system key words in the knowledge areas we investigated are used correctly in Archivos de Bronconeumología only about 50% of the time.

[Spanish scientific production in diagnostic and therapeutic research area of respiratory system in international journals from 1990 to 2002].

OBJECTIVE: The aim of this study were: to analyze the spanish production in diagnostic and therapeutics research area of respiratory system in international journals from 1990 to 2002. MATERIAL AND METHODS: Papers published in diagnostic and therapeutics area of respiratory system during this period of time were selected by the PUBMED system. We delimited the production with key words: respiratory system and the one that is used by the diagnostic and therapeutics research area. RESULTS: We obtain a total of 67 document published in international journals by spanish authors. The scientific production in the diagnostic and therapeutics area have stabilized in this 12 years period. The distribution of articles by the institutional affiliation and province of authors also showed a wide dispersion: Barcelona and Clinic and Provincial Hospital of Barcelona were responsible for 43.1% and 23.8% of all the production. Bronchoalveolar lavage and the use of telescopic catheter were the type of diagnostic and therapeutic procedure more productive. CONCLUSIONS: The scientific production in the diagnostic and therapeutics area of respiratory system have stabilized in this 12 years period, this evidence contrast with the high growth detected in respiratory system in general.

[Use and comprehensibility of statistical analysis in Archivos de Bronconeumología (1970-1999)].

OBJECTIVES: To describe the type of statistical analysis used most often in original research articles published in Archivos de Bronconeumología, and the evolution of statistical analysis over time in terms of complexity. To determine comprehensibility, taking bivariate analysis as the reference threshold. MATERIAL AND METHODS: All articles published in the original research section of Archivos de Bronconeumología from 1970 through 1999 were reviewed manually. For each article we recorded the category or categories of statistical analysis and its comprehensibility (with a reference threshold set at category 7). We studied the following factors: year of publication, type of analysis, comprehensibility, maximum category achieved, subject area and number of authors. RESULTS: Eight hundred sixty original articles, with a mean 5 2 authors per article were examined. The maximum category reached was a mean 4.15 4.61. The three types of analysis used most often in all articles were category 1 (descriptive only) at 49.4%, category 2 (t and z tests) at 26.4% and category 3 (bivariate tables) at 19.1%. Among the more complex analytical categories, the most often used were analysis of variance (category 8) at 9%, survival analysis (category 16) at 6.2%, and non-parametric correlations at 3.4%. Comparing results by decade, the proportion of articles with only descriptive analysis fell from 74% in the seventies to 63.9% in the eighties and to 36.1% in the nineties (90s vs. 80s, p < 0.05; 90s vs. 70s, p < 0.05; 80s vs. 70s, p < 0.05). The use of statistics from categories 3 and 6 increased (p < 0.05), and the use of the more complex categories 8, 10, 11 and 16 increased (p < 0.05). Comprehensibility decreased significantly to 68.1% in the 90s in relation to increased complexity of analysis. CONCLUSIONS: The complexity of statistical analysis in articles published in Archivos de Bronconeumología increased over time, while comprehensibility decreased.

[Analysis and evolution of bibliometric indicators of productivity and readership of articles on smoking appearing in Archivos de Bronconeumología from 1970 to 2000. A comparison to others topics in respiratory medicine].

OBJECTIVES: To analyze the evolution of bibliometric indexes for research on smoking published in Archivos de Bronconeumología (AB) from 1970 through 2000, to compare indexes for each of the three decades under study, and to compare the indexes for smoking research with those of other topics in respiratory medicine. MATERIAL AND METHODS: We reviewed all articles published by AB between 1970 and 2000, recording data on 13 characteristics. RESULTS: Of 2,198 documents reviewed, 45 (2.04%) were about smoking; 71% of them were published in the last five years. The research came mainly from the most productive four provinces and five hospitals, which produced 68.1% and 50% of the reports, respectively. The first authors were pneumologists in 78.4% of the articles. The productivity index was 1.65, and the mean number of authors per document (collaboration index) was 4 3. The total number of references was 1,230 and the number of references per paper was 27.3 26. AB was the most cited journal. We found no change in obsolescence indexes. Insularity and self citation indexes tended to increase. Topics with productivity indexes that were higher than the index for smoking research were asthma (p < 0.05), respiratory insufficiency and sleep disorders, non-tuberculosis infection, oncology and pleural and interstitial diseases (p < 0.001). The insularity index was higher for tuberculosis than for smoking (p < 0.05). CONCLUSIONS: Smoking research increased considerably during the period studied. Pulmonologists predominated among the authors of studies on smoking. Bibliometric indexes for smoking evolved as did the indexes for other research. The most frequently cited journal was AB.

[Analysis of the impact of Anales Españoles de Pediatría through the Science Citation Index from 1997-2000].

OBJECTIVES: To quantify the impact factor of Anales Españoles de Pediatría from 1997 until 2000 and to identify the journal's citation patterns and the topics with the greatest impact. METHODS: SCISEARCH was used to locate citations of articles published in Anales Españoles de Pediatría between 1995 and 1999. The following data were collected for each article: year of publication, authors, journal, country of publication, language, specialty or specialties, institution(s), residence of the first author and topic. The impact factor was calculated as the ratio between citations received over 1 year by articles published in Anales Españoles de Pediatría in the two previous years and the total number of articles published by Anales Españoles de Pediatría over the 2 years under study. RESULTS: The impact factor of Anales Españoles de Pediatría was 0.052 in 1997, 0.080 in 1998, 0.101 in 1999, 0.089 in 2000 and 0.064 in 2001. Citations were found in a wide range of source journals. The greatest proportion (35.6 %) were found in Spanish medical journals. Citations were made mainly by Spanish authors (62.8 %) and self-citation was moderate (14.3 %). Topics related to neurology (16.9 % of the citations received), infectious diseases (16.2 %) and neonatology (14.8 %) had the greatest impact. CONCLUSION: The impact factor of Anales Españoles de Pediatria is modest, although higher than that of some other biomedical publications included in Journal Citation Reports.

[Usefulness of induced sputum cytology in the study of central masses in elderly patients].

OBJECTIVE: To analyze the diagnostic yield of cytology obtained by means of sputum induction in elderly patients with central masses. METHODS: A total of 23 patients older than 75 years with centrally located pulmonary masses identified by computerized tomography were studied. All patients collected three spontaneously produced sputum specimens and underwent three sputum-induction sessions. Induction was considered satisfactory when the patient expectorated within the following 15 minutes. All sputum specimens were processed by May-Grünwald-Giemsa and Papanicolau staining techniques. The definite diagnostic was considered that obtained by means of bronchial biopsy or chest percutaneous aspirative puncture (PAP). RESULTS: With sputum induction, 19 patients (83%) collected satisfactory specimens and in 14 cases cytology was positive for malignancy (sensitivity 74%) and negative in five. No false-positive cases were recorded and in no patient did complications occur in association with sputum induction. The analysis of agreement between induced sputum and definite diagnosis showed a kappa index of 0.66. CONCLUSIONS: Among elderly patients with central pulmonary masses, the cytology of induced sputum should be considered as the first diagnostic procedure because of its harmlessness, remarkable sensitivity and high histologic correlation.

[Evolution of Spanish scientific production in international respiratory system journals from 1987 to 1998].

BACKGROUND: A relevant increase in the clinical relevance of respiratory system area has occurred in the last few years. The aims of this study were: to analyze the evolution of production and impact of Spanish scientific activity in the respiratory system area between 1987 to 1998; and to assess the participation of the different Spanish autonomic communities in this activity. METHODS: Papers published in 38 international journals of respiratory system during this period of time were selected by the MedLine system, delimiting Spanish production. Geographical and institutional distribution of the production, specialist of authors, main publication journals and type of publication were recorded. Production and impact bibliometric indicators were estimated. RESULTS: The number of Spanish documents from 1987 to 1998 multiplied by eight, reaching the 1.49% of the total published documents in 1998. However, the growth model of scientific production during this period was lineal. The increase in the scientific production was associated with a increase over time in the expected impact factor. In 1998, expected impact factor was 2. 206. Forty-two percent of the papers were signed by pneumologists. The highest contributor were hospitals. The most productive autonomous communities were Catalonia, Cantabria and Madrid. The communities with the highest expected impact factor were Balears and Catalonia. CONCLUSIONS: A relevant increase has occurred in the scientific production and impact of Spanish biomedical activity in respiratory system area.

An evaluation of tissue polypeptide antigen (TPA) in the two bronchoalveolar lavage fractions of lung cancer patients.

BACKGROUND: It has been proven that cytokeratins (CKs) are useful tumor markers for the follow-up, treatment monitoring and prognosis evaluation of lung cancer and among these, tissue polypeptide antigen (TPA) plays an important role. Nevertheless, only a small number of studies have been reported about their diagnostic capacity. Bronchoalveolar lavage (BAL) can be divided into two fractions: bronchiolar (BF) and alveolar (AF). For the above reasons, our aims were (1) to analyze the diagnostic usefulness of TPA in the BAL of lung cancer patients and (2) to observe if, in lung cancer patients, TPA levels in the two BAL fractions are different. This should mean that the study of tumor markers in the BAL should be carried out in both fractions to increase their diagnostic capacity. METHODS: We studied 289 BALs divided into two phases. In phase I, TPA was analyzed in the BAL of six groups of subjects (healthy persons, chronic bronchitis, asthma, respiratory infections, diffuse interstitial pulmonary diseases and lung cancer). In phase II, TPA was studied in both BAL fractions of a group of patients with lung cancer. RESULTS: We observed that TPA levels were significantly higher in the BAL of patients with bronchogenic neoplasias. In these patients, TPA was increased in the BF of the lavage in relation to the AF. In smoker patients with pulmonary carcinomas, TPA was higher in the AF of the BAL than in the lavage of non-smokers. This did not occur in the BF. We found no relation between the TPA concentrations and cancer histology. CONCLUSIONS: We believe that TPA is a useful tumor marker with diagnostic capacity and this capacity is increased when it is studied in the two BAL fractions. Smoking habit may play a role in the secretion of tumor markers by the tumor cells.

[The sleep apnea syndrome as a health problem. An estimation of its prevalence and morbimortality].

The sleep apnea syndrome (SAS) is a frequent health problem and has severe consequences. In some reports, their main symptoms like habitual snoring, excessive daytime sleepiness and nocturnal respiratory pauses can affect up to 65, 22.2 and 27.9% of men, respectively, being less frequent in women. The incidence of SAS ranges between 4 and 7% of adult general population. Patients without treatment have an increased mortality (37% to 8 years). Between the complications that made this adverse outcome are cardiovascular problems (arterial hypertension, coronary artery disease, sudden death), cerebral infarctions and other derived from sleepiness like traffic crashes.

[Estimate of bibliometric indicators of the impact of +Archivos de Bronconeumología].

The main bibliographic impact factors of ARCHIVOS DE BRONCONEUMOLOGIA for the past two years are estimated. Citations of ARCHIVOS DE BRONCONEUMOLOGIA in 1996 and 1997 by Medicina Clínica and all citing journals grouped under the heading "Respiratory System" in the Science Citation Index were counted. The year the citation appeared and the year of publication of the article in ARCHIVOS DE BRONCONEUMOLOGIA were recorded. The total number of articles published by ARCHIVOS DE BRONCONEUMOLOGIA between 1994 and 1996 were also counted. Based on these data we calculated our journal's impact factor for each year studied. Relative impact factor was also figured. In 1996 the exposure factor achieved was 1.602, the half-life of articles was 2.00 years, the impact factor was 0.069 and the relative impact factor was 1.371. In 1997 the exposure index was 1.477, the citation half-life was 3.00 years and the impact factor was 0.030. In conclusion, the international impact of ARCHIVOS DE BRONCONEUMOLOGIA is modest, although our relative impact factor is greater than that of some other internal medicine publications or specialized journals listed in Journal Citation Reports.
