[Detection of opportunities for improvement of the quality of parenteral nutrition in patients subjected to gastrointestinal surgery].

The nutritional support team must justify its role by proving that it provides an adequate quality control and supervises the administration of the nutritional support to avoid its inappropriate use. The measures based on the process reported on the improvement opportunities while those based on the results allow an evaluation of the quality. The objective of this study is to present the results of a systematic search for improvement opportunities in two fundamental activities of the nutritional support team: the evaluation of the patient needs, and the adequation of the caloric supply to these needs. The data corresponding to nutritional support and nutritional assessment of 217 patients who initiated central parenteral nutrition during the perioperative surgery period for a laparotomy were registered between January of 1996 and June of 1997. These data were used to calculate 8 selected quality indicators to report on the quality of the activities subject to the evaluation. Moreover, the initial and final values of the nutritional assessment parameters of a sub-group of patients were compared with the aim of obtaining a measure of the result of parenteral nutrition. The final average levels of albumin, prealbumin, transferin, and the Nutritional Prognostic Index were significantly better than the initial data in the subgroup in which these data were available. The analysis of the process indicators allowed the detection of the need to reduce the caloric supply in relation to the protein supply and to promote the use of programs with a caloric supply that was better adjusted to the BMI and/or the patients' weight. It was also shown that it is necessary to increase the number of patients assessed from a nutritional point of view at the beginning and at the end of parenteral nutrition.

Allergy to Anacardiaceae: description of cashew and pistachio nut allergens.

Some members of the Anacardiaceae family, such as cashew nut, pistachio nut and mango, have been reported to cause immediate allergic reactions. We report three cases of anaphylaxis to cashew nuts. With the aim of describing the allergens existing in cashew and pistachio nuts, patients were prick tested with cashew and pistachio extracts. Specific IgE against both nuts was studied by CAP and SDS-PAGE/immunoblotting. It was found that skin tests and specific IgE to cashew and pistachio nuts were positive in the three patients. Both nuts showed several protein bands in SDS-PAGE. The strongest IgE-binding bands had similar molecular weights (15, 30 and 60 kDa) in cashew and pistachio nuts. These main bands were found to be sensitive to reducing agents. It was concluded that these three patients suffered immediate reactions to cashew nut due to an IgE-mediated mechanism.

[Immediate results in percutaneous mitral valvuloplasty with the Nucleus balloon].

UNLABELLED: We present the immediate results of mitral valvuloplasty in 10 patients using Nucleus balloon. Several publications show highly successful results obtained with the Inoue balloon and double balloon technique. There are no publications of Nucleus balloon. We consider that this device could offer several advantages according to its physical and technical characteristics. We performed the procedure in 10 female patients, with severe mitral valve stenosis, with an average age of 44.8 years (23-70) and an average Wilkins score of 7.5 (6-9), using a combined technique. The increase in mitral valve area with Gorlin equation was 1.03 +/- .13 to 2.6 +/- .67 cm2, the decrease in transmitral gradient from 15 +/- 4.1 to 3.42 +/- 2.6 mm Hg, and decrease in systolic pulmonary pressure from 54.1 +/- 18.8 to 24.9 +/- 5.1 mm Hg. One patient developed grade I mitral insufficiency after the procedure, and another non significant interatrial shunt. CONCLUSIONS: Good results are obtained with this balloon, the valvuloplasty technique is more simple than with double balloon, it is much cheaper than Inoue balloon and we consider it could be useful in moderately calcified valves.

[Biliary calculi in th golden hamster (M. a. auratus). XXXV. Effect of the hydrogenation of Polifat KA-02 on its preventive activity against pigment cholelithiasis].

It has been previously reported that Polifat KA-02 (fatty acids derived from safflower oil) prevents the pigment cholelithiasis produced by vitamin A in the golden hamster. In order to establish whether the saturation or the unsaturation of Polifat is directly related with its preventive action, in the present study the effect of the hydrogenation of the derivative on this preventive action was tested. Two experiments were carried out in which the animals received the different experimental diets ad libitum during 70 days. In the 1st. experiment the effect of the additions to the lithogenic diet (Rodents Chow + 25,000 UI% of retinol acetate) of 18% of crude safflower oil, or 15% Polifat, or 15% hydrogenated Polifat (approx. 80% saturation) were assayed. In the 2nd. experiment the effect of a more hydrogenated Polifat KA-02 (greater than 99%) was tested. The results of the 1st. experiment showed that the lithogenic diet produced a high incidence (90.5%) of animals with gallstones, which was very similar (100%) to that produced by the crude safflower oil, whereas the additions of normal and hydrogenated Polifat completely prevented the formation of gallstones. The 2 forms of Polifat produced a considerable hepatomegaly. The absorption of vitamin A, measured as total hepatic vitamin A, was higher in the 3 groups that received extra lipids in their diets. The results of the 2nd. experiment fully confirmed the findings related to the hydrogenated Polifat. It is concluded that hydrogenated Polifat KA-02 prevents, as normal Polifat does it, the pigment cholelithiasis in the golden hamster produced by vitamin A.

[Myxoma of the right ventricle: report of a clinical case and review of the literature].

We studied a seventeen year old female patient with clinical manifestations of pulmonary embolism. A Two-Dimensional echocardiography showed the presence of a right intraventricular mass. It was surgically removed and the histopathological study showed it to be a myxoma. The recovery of the patient was uneventful. This is the first case of a right ventricular myxoma diagnosed during life in our hospital. The medical literature describing this unusual location is reviewed.

[Preliminary results of indirect isotopic lymphography with colloidal rhenium marked with Tc 99m in urinary tract pathology].

The authors have carried out a study of a new method for studying the lymphatic system in patients with a neoplastic, urological pathology, by using lymphogammagraphy with radionuclids. It involves the comparative analysis of the lymphatic ganglion chains. This method consists in the subcutaneous injection into the interdigital space of a radionuclid, Colloidal, Rhenium Sulphide, marked with Tc-99m and with external detection by means of a gammacamera and with lymphatic channeling not being necessary.

[Conjunctival ophthalmomyiasis caused by Oestrus ovis].

BACKGROUND: Microbiologic diagnosis of conjunctival ophthalmomyiasis. MATERIAL AND METHODS: Material extracted from three patients' conjunctiva who complained of sudden presence of foreign body and were diagnosed as conjunctival ophthalmomyiasis has been studied. Patients were recruited over two months from the Emergency Department. Diagnosis was performed through observation using the light microscope. RESULTS: Oestrus ovis first-instance larvas were observed in all the samples studied. CONCLUSIONS: This observation of similar cases in a short period of time and the low frequency of diseased people who lived in a urban environment makes necessary to think in this agent as cause of ophthalmomyiasis in our area and therefore perform an adequate differential diagnosis.

[A pathological study of valvular dysplasia and attachment in Ebstein's anomaly].

Forty hearts with Ebstein's disease of the tricuspid valve were studied in order to correlate dysplasia and attachment of tricuspid valve leaflets at the ventricular walls, atrialization of the right ventricle and fibrosis in this chamber. The correlation was positive. The right ventricular atrialization was directly proportional to leaflet attachment; the correlation in severity between leaflet attachment; the correlation in severity between leaflet attachment and dysplasia was positive in 45%. The right ventricular fibrosis was very frequent in the group with patent pulmonary valve (90%), and the contrary was in the group with pulmonary valve atresia (10%). The structural damage in the right ventricular wall was directly proportional to leaflet attachment in hearts with patent pulmonary valve (90%); in all of these hearts the anatomic annulus of the tricuspid valve was enlarged but in hearts with pulmonary valve atresia this feature only occurred in 10%. The specimens with atrioventricular discordant connexion, the anatomic annulus size, the degree of leaflet attachment, the dysplasia of the tricuspid valve as well as the fibrosis of the wall of the right ventricle were less severe compared with those hearts with concordant atrioventricular connexion. Ebstein's disease of the tricuspid valve is characterized by a disorder in the histodifferentiation of the right ventricle and the tricuspid valve which originates in great part from it.

[Allgrove syndrome (achalasia-alacrima-adrenal gland insufficiency): report of a case].

Achalasia is very uncommon in children. It is still more when it is associated with alacrima and adrenal insufficiency, a disorder known as Allgrove's Syndrome (Triple-A-Syndrome). This paper describes a 21-years-old man with lack of lacrimation, achalasia and glucocorticoid deficiency. Additional features included: Hyperreflexia, pes cavus, muscle weakness, and nasal speech. Indicating that the disorder has a wide spectrum of clinical manifestations. This case has been diagnosed at adulthood.

Plasma sulfobromophthalein disappearance in Gilbert's syndrome.

BACKGROUND/AIMS: We studied the metabolism of sulfobromophthalein and its relationship with serum bilirubin levels in 40 patients with Gilbert's syndrome (type I 30; type II 6; type III 4). MATERIAL AND METHODS: Plasma sulfobromophthalein disappearance studies were carried out and 72 hours later, serum bilirubin concentrations (total and unconjugated fraction) were determined at baseline and after 24 and 48 hours of dietary restriction to 400 calories/day. RESULTS: The fractional transfer rate of sulfobromophthalein from plasma to liver was significantly higher in types I (14.7 +/- 3.4 ml/min) and II (14.9 +/- 2.7 ml/min) than in type III (8.7 +/- 1.5 ml/min). The fraction of the plasma sulfobromophthalein pool irreversibly cleared per min was significantly higher in type I (12.2 +/- 2.6 ml/min) than in types II (9.5 +/- 1.5 ml/min) and III (9.3 +/- 3.8). In all patients, serum bilirubin concentrations were significantly higher after fasting as compared with baseline. There was a significant correlation between the increments of serum unconjugated bilirubin levels after the fasting test and the transfer rate of sulfobromophthalein from plasma to liver (F = 9.8411, r = -0.4535, p = 0.003). CONCLUSION: These findings indicate the presence of an active uptake system shared by bilirubin and sulfobromophthalein.
