Wave packet study of the UV photodissociation of the Ar2HBr complex.

The photodissociation dynamics of the Ar2HBr van der Waals molecule is studied using the multiconfiguration time-dependent Hartree method. Standard Jacobian coordinates are used to describe the molecule. Two four-dimensional calculations are carried out where the rotation of the Ar2 molecule and, in addition, either the vibration of Ar2-Br or that of Ar2 are frozen. The time-evolution of the probability density in the different modes and the calculation of the dissociative flux show that the dissociating hydrogen atom preferentially moves out of the plane defined by Ar2 and Br. A comprehensive study of the cage effect in the process is presented.

Cumulative isomerization probability studied by various transition state wave packet methods including the MCTDH algorithm. Benchmark: HCN-->CNH isomerization.

The 3D cumulative isomerization probability N(E) for the transfer of a light particle between two atoms is computed by one time-independent and two time-dependent versions of the transition state wave packet (TSWP) method. The time-independent method is based on the direct expansion of the microcanonical projection operator on Chebyshev polynomials. In the time-dependent TSWP methods, the propagations are carried out by the split operator scheme and the multiconfiguration time-dependent Hartree (MCTDH) algorithm. This is the very first implementation of the TSWP method in the Heidelberg MCTDH package [G. W. Worth, M. H. Beck, A. Jackle, and H.-D. Meyer, The MCDTH package, Version 8.2 (2000); H.-D Meyer, Version 8.3 (2002). See http://www.pci.uni-heidelberg.de/tc/usr/mctdh/]. The benchmark is the HCN-->CNH isomerization for zero total angular momentum. Particular insights are given into the tunneling region. In larger systems, the time-dependent version of TSWP making use of the MCTDH algorithm will permit to treat more and more modes quantum mechanically, for very accurate results. Therefore, it was important to calibrate the implementation. Besides, we also assess the efficiency of a reduced dimensionality approach by comparing the new exact 3D calculations of N(E) for the HCN-->CNH isomerization with results obtained via 1D or 2D active subspaces. This suggests that, it should be possible to take directly benefit of the present 3D approaches, adapted for triatomic Jacobi coordinates to compute N(E) for H-transfer in larger systems, via three active coordinates. The prerequisite is then the simplification of the reduced 3D kinetic energy operator with rigid constraint to take the form corresponding to a pseudo triatomic system in Jacobi coordinates with effective masses. This last step is checked in the methoxy radical and malonaldehyde. Finally, different ways to obtain reliable eigenvectors of the flux operator associated with a dividing surface are revisited.

Decrease of replicative capacity of HIV isolates after genotypic guided change of therapy.

A longitudinal study of the replication capacity of HIV strains isolated from 18 patients failing highly active antiretroviral therapy (HAART) was undertaken at the time of genotypic guided change of therapy and after 12 months. Patients were divided in two groups according to the response to therapy: immune responders (12 patients with immune recovery defined as having more than 100 CD4 cells compared to baseline value), and failing patients (six patients without immune recovery). At enrollment no significant difference in terms of CD4 cell count and HIV plasma viremia was detected between the two groups. One year after change of therapy, all patients experienced a decrease in the replication capacity of HIV strains. The HIV replication capacity of the failing and of immune-responder patients decreased from 60% (range 14-96%) to 26.4% (range 0.4-74.5) and from 46.8% (range 15-98%) to 3.6% (range 0.1-26.8%), respectively. At month 12, the difference of HIV replication capacity between the two groups reached a statistical significance (P<0.03). After the change of therapy, an increase in the number of drug resistance mutations in the protease gene was detected in both groups with a higher prevalence of M36I mutation in immune responders. The HIV strains of patients failing HAART showed a progressive impaired replication capacity. The degree of the impairment in viral replication correlated with the viro-immunological discordant response to HAART and with the acquisition of new drug resistant mutations in the protease gene. In patients failing HAART, the impaired replication capacity of HIV strains could justify the persistence of an immune recovery.

Interaction between levothyroxine and indinavir in a patient with HIV infection.

Drug interactions are an important and emerging problem in the treatment of HIV-infected patients. Protease inhibitors, like nonnucleoside reverse transcriptase inhibitors, are metabolized by the cytochrome P-450 enzyme system and each of these antiretroviral agents may interact with other drugs metabolized by this system. Some protease inhibitors may also interact with glucuronosyl transferase activity affecting plasma concentrations of drugs metabolized through this pathway. We describe a case of an HIV-infected patient, taking levothyroxine for hypothyroidism and clinically stable, who, after the introduction of an antiretroviral regimen containing indinavir, developed a pharmacological hyperthyroidism.

Fertility in postpartum dairy cows in winter or summer following estrus synchronization and fixed time AI after the induction of an LH surge with GnRH or hCG.

In this study, the fertility of postpartum dairy cows after a sequence of treatments with GnRH (Day 0), PGF2alpha (Day 7) and GnRH (Day 9) (GnRH group; n = 164) or hCG (Day 0), PGF2alpha (Day 7) and hCG (Day 9) (group hCG; n = 166) was investigated in summer and winter seasons. All cows were artificially inseminated without estrus detection, 16-18 h after the end of treatment. Control cows (CONT; n = 226) were not treated and were inseminated at natural estrus. The pregnancy rates at Day 90 (46% versus 33%; P < 0.05) and at Day 135 (76% versus 62%; P < 0.05) postpartum were significantly lower in CONT cows in summer compared to winter months but this effect was not observed in the two treated groups. The number of days from calving to conception was significantly lower in GnRH and hCG treatment groups compared to CONT cows in cold months (102 +/- 3.2, 106 +/- 4.2, 126 +/- 3.1, respectively; P < 0.001) and in hot months (112 +/- 3.2, 114 +/- 4.2, 139 +/- 3.1, respectively; P < 0.001). The concentration of insulin was significantly higher in winter (P < 0.001). There were no differences in average plasma concentration of glucose (P = 0.474), GH (P = 0.441) or IGF-I (P = 0.190). In conclusion, we have shown that veterinary supervision combined with a program of estrous synchronization and fixed time insemination can improve fertility of cows suffering heat stress.

Limits on the existence of heavy neutrinos in the range 50-1000 eV from the study of the 187Re beta decay.

We analyzed the spectrum of the 187Re beta decay, obtained with a cryogenic microcalorimeter, searching for heavy neutrinos in the mass range 50-1000 eV. No evidence has been found for them and the upper limits on the mixing angle with a zero-mass neutrino are reported. Upper limits of 9x10(-3) at 1000 eV/c(2), 1.2x10(-2) at 500 eV/c(2), 4.4x10(-2) at 200 eV/c(2), and 0.116 at 100 eV/c(2) at 95% C.L. have been obtained. These upper limits are a factor of 2 to 4 lower than the current limits reported in the literature.

The use of PC based VR in clinical medicine: the VREPAR projects.

Virtual reality (VR) is an emerging technology that alters the way individuals interact with computers: a 3D computer-generated environment in which a person can move about and interact as if he actually was inside it. Given to the high computational power required to create virtual environments, these are usually developed on expensive high-end workstations. However, the significant advances in PC hardware that have been made over the last three years, are making PC-based VR a possible solution for clinical assessment and therapy. VREPAR - Virtual Reality Environments for Psychoneurophysiological Assessment and Rehabilitation - are two European Community funded projects (Telematics for health - HC 1053/HC 1055 - http://www.psicologia.net) that are trying to develop a modular PC-based virtual reality system for the medical market. The paper describes the rationale of the developed modules and the preliminary results obtained.

Fluvoxamine alone in the treatment of delusional depression.

OBJECTIVE: The aim of this study was to evaluate the efficacy of fluvoxamine in the treatment of delusional depression. METHOD: Fifty-nine inpatients who met the DSM-III-R criteria for major depression with psychotic features were treated with fluvoxamine for 6 weeks. Patients were assessed at baseline and weekly thereafter with the Hamilton Depression Rating Scale and the Dimensions of Delusional Experience rating scale. RESULTS: Of the 57 subjects completed the trial, 84.2% (N=48) recovered. The index episodes of the patients who did not respond to fluvoxamine were of significantly longer duration than those of the responders. CONCLUSIONS: Fluvoxamine has a response rate similar to that of the currently most efficacious treatments for delusional depression, including antidepressants plus antipsychotics and ECT.

Nimesulide as a downregulator of the activity of the neutrophil myeloperoxidase pathway. Focus on the histoprotective potential of the drug during inflammatory processes.

Neutrophils, recruited to tissue sites of inflammation, release a variety of oxidants and enzymes, which are responsible for tissue damage. Among the oxidants released are potent chlorinated compounds, such as hypochlorous acid and chloramines, which induce tissue cell damage and inactivate protease inhibitors, particularly alpha 1-antitrypsin, the specific inhibitor of neutrophil elastase. In studying a rational approach to the pharmacological control of neutrophil-mediated tissue injury, we investigated the activity of the anti-inflammatory drug nimesulide. This agent reduced the function of the myeloperoxidase pathway (which generates hypochlorous acid), by exerting a cell-directed inhibitory activity, as shown by measurement of superoxide anion and hydrogen peroxide production. Nimesulide also inactivated hypochlorous acid directly and protected alpha 1-antitrypsin from the neutrophil-mediated oxidation. Thus, neutrophil elastolytic activity may be attenuated by nimesulide-spared alpha 1-antitrypsin. The prevention of oxidative inactivation of alpha 1-antitrypsin by nimesulide strictly correlates with the drug's ability to suppress the extracellular availability of hypochlorous acid. Taken together, these data suggest that nimesulide may prevent tissue injury at sites of inflammation by maintaining natural host protective systems.

A comparison between delusional and non-delusional depressives.

Delusional depressive episodes may represent more sever degrees of depressive manifestations or a distinct subtype of depressive illness. To test these two alternative hypotheses, characteristics on demographic, clinical and symptomatic variables, presence of personality disorders and familial loading were compared in 57 delusional and 57 non-delusional depressive patients. The delusional group did not differ on symptomatological intensity degree, clinical and familial loading characteristics. They did differ on higher distribution of Cluster 1 personality disorders. In the delusional sample the presence of mood incongruent psychotic features and hallucinations were evaluated as possible indicators of different subtypes of delusional depression.

Perspectives in clinical psychopharmacology of amitriptyline and fluvoxamine. A double-blind study in depressed inpatients.

The efficacy of fluvoxamine was compared to that of amitriptyline in a double-blind 6-week fixed-dose trial of 56 inpatients with major depressive episode. The two drugs were comparable in their antidepressant efficacy. We tested the percentage of improvement in Hamilton-D scores during the first and the second weeks of treatment as predictors of efficacy for the last week. Improvement rates during the second week significantly predicted the outcome. We also investigated whether or not some symptomatological characteristics would permit prior prediction of the outcome with amitriptyline or fluvoxamine, dividing our sample into responders and nonresponders to the two drugs. The four groups showed differences in their symptomatological profiles.

Effectiveness and tolerability of nimesulide in the treatment of osteoarthritic elderly patients.

One important point in the treatment of osteoarthritis is to control the pain that accompanies the persistent inflammatory conditions. Because of the chronic nature of the disease, drugs with a good ratio of effectiveness to tolerability must be used. In the following double-blind study the effects of nimesulide, a non-steroidal antiinflammatory drug, were compared with those of placebo in 40 elderly osteoarthritic patients, during and after a treatment cycle of 90 days with two 100-mg tablets a day. The reduction of spontaneous and motion-induced pain and of morning stiffness seen at the various check-ups attested the good effectiveness of nimesulide. Functional status as a whole was improved, and it was found that the concomitant weekly intake of an analgesic (paracetamol) treatment could be decreased during the 3 months of the study. The tolerability of nimesulide was good.

[Use of ultrasound-guided needle aspiration in the diagnosis and treatment of breast cysts].

The authors evaluated the advantages obtained utilizing FNAB with ultrasound in the needle aspiration of breast cysts. Echography can guide the aspiration up to its end in an effective and localized way. Cytologic examination of the cystic fluid confirmed the benign nature of the cyst. The association of needle aspiration and guided echography was related to a low number of relapses (19.8%). Among these relapses some risk factors have been found as well as a correlation between cystic fluid cytologic examination and relapses incidence has been determined.

[Breast secretions].

Nipple discharge, a rare clinical sign, is more frequently determined by benign breast diseases, but it can be associated to breast cancer; for this reason such clinical sign shouldn't be ignored. Cytologic examination together with breast examination and thermography are the correct oncologic approach for nipple discharge, because such way it is possible to select the patients for whom mammography or galactography are recommended as well as and breast biopsy depending on the galactographic data.

[Diagnostic use of the cell culture technique in a case of postoperative saccate lymphorrhea].

The Authors demonstrated the presence of tumour cells by an in vitro culture of the cells from the pellet of a postoperative saccate effusion, when no tumor cells were evident by the cytologic examination of the effusion. The ultrastructural features and the immunocytochemical characteristics of the cell line were analyzed and these data confirmed its malignant nature. It allowed a firm diagnosis, a correct therapy and prognosis.
