Comparative study of two non-steroidal anti-inflammatory eyedrops, 0.1% indomethacin versus 0.1% diclofenac in pain control post photorefractive keratectomy.

We evaluated the efficacy and safety of 0.1% indomethacin and 0.1% diclofenac solutions, in controlling pain post excimer laser photorefractive keratectomy (PRK). After written consent, 61 informed patients (23 males, 38 females; mean age = 33.5 +/- 8.4 yrs) were enrolled in a double-masked, randomized, comparative study and assigned to either indomethacin or diclofenac treatment. Subjective preoperative evaluation of individual susceptibility to pain evoked by topical application of 1% tetracaine vs saline served as reference for further post-operative pain measurement using a visual analog rating scale. Ocular and cephalic pain, itching, foreign body sensation, insomnia, photophobia, blepharospasm as well as systemic analgesic medication and alcohol intake were monitored for 3 days following photoablation as well as the re-epithelialization process. Both solutions significantly reduced pain on the first day following excimer laser PRK, and this activity was maintained until the end of the observation period. At Day 0 the first measure of pain level was slightly higher in the indomethacin group (p < 0.05) and could be related to a possible anaesthetic effect of Diclofenac. During the follow-up the oral intake of analgesics was higher in the diclofenac group, however this difference was not significant. Wound healing rate was not affected by indomethacin or diclofenac administration. These data suggest that both 0.1% indomethacin and 0.1% diclofenac ophthalmic solutions may help to control the pain induced by excimer laser PRK without any deleterious effect on corneal wound healing.

Levocabastine eye drops in the treatment of vernal conjunctivitis.

The efficacy and tolerability of levocabastine eye drops in vernal conjunctivitis (VC) were evaluated in a double-blind, placebo-controlled trial involving 46 patients over a period of 4 weeks. After 1 week of treatment, therapeutic efficacy was considered to be excellent or good for 70% of the levocabastine-treated patients compared with only 33% of patients in the placebo group (p < 0.009). Levocabastine patients experienced significantly greater relief of their individually severest symptom than placebo-treated patients both after 1 week and at the end of the trial (p < 0.04). The reduction in symptom severity was significantly greater in the levocabastine group than in the control group for photophobia (p < 0.003) after 1 week, and for photophobia (p < 0.008), irritation (p = 0.05) and itchy eyes (p = 0.05) at the end of the trial. The percentage of days on which patients were completely symptom-free was significantly higher in the levocabastine group than in the placebo group (28% versus 4%; p < 0.02). Eight placebo-treated patients withdrew from the trial due to treatment inefficacy compared with only four levocabastine-treated patients (p = 0.013). Two of the three levocabastine, and all five placebo patients who elected to continue on open-label levocabastine had an excellent or good overall response after 1 to 3 weeks of treatment. All reported adverse reactions were mild and their incidence was equal in the two treatment groups. Levocabastine eye drops are effective and well tolerated in the treatment of VC.

A rare case of chronic papillary conjunctivitis diagnosed after several years of evolution. Clinical and pathological findings.

A man with a 2.5 year history of chronic papillary conjunctivitis was diagnosed as having "floppy eyelid syndrome". This syndrome, first described in 1981, is characterized by loose upper eyelids that evert readily on elevation of the lid, a chronic papillary conjunctivitis and a soft rubbery tarsus. The condition is resistant to any form of medical treatment but responds well to a horizontal lid shortening procedure. The etiology is unknown but the present report suggests that an abnormality of the structure of the orbicularis muscle may play a role.

Peribulbar regional anaesthesia for anterior eye segment surgery.

The technique, the results and the complications for local peribulbar anaesthesia for anterior eye segment surgery are discussed. A personal series of 3,350 cases is analysed. The rare local complications could have been avoided by improving the technique. There were no general complications.

Short term results with excimer laser-photo-refractive keratectomy.

The results at three months after photo-refractive keratectomy surgery (PRK) with the Meditec Mel 60 laser are discussed. A total of 86 eyes (18 hyperopes and 68 myopes) were followed. The predictability of +/- 1D in the myopia group of less than 6D was 86% and it was 68% in the myopia group of more than 6D. The predictability of +/- 2D was 96% in the group below 6D, and in the group of more than 6D it was 85%. In hyperopia, the mean refractive change was 2.7D and 44% of eyes were between +/- 1D at three months after Excimer laser surgery.

Personal results with the 3M diffractive multifocal intraocular lens.

The results of implanting 269 3M multifocal intraocular lenses are analyzed and compared with those of monofocal lenses. Visual acuity results were excellent. In the best case group, 98% of eyes obtained a distance visual acuity of 20/40 or better and 96% of best cases obtained a functional reading capacity without supplementary reading correction. On contrast sensitivity testing, eyes with the 3M multifocal intraocular lens performed slightly less than eyes with a monofocal lens using the Regan low contrast charts and the Vistech charts for near vision.

Malignant melanomas of the choroid and ruthenium applicators.

Fourty patients with a malignant melanoma of the choroid were treated with ruthenium applicators. Depending on the height of the tumour, the patients were divided into 3 groups. In the first group with a tumour height superior to 6 mm the results were poor as well from a functional point of view as considering tumour regression. In a second group with a tumour height between 5 and 6 mm the results were favourable concerning tumour regression, but only in 50% of the cases did the eye remain functional. The results in the third group with a tumour height between 2 and 5 mm were satisfactory. Tumour regression was obtained in 96% of the eyes; 86% of the eyes remained functional. Tumour related death (7.5%) was not correlated with initial height of the tumour at the time of diagnosis.

Personal results with bicanaliculonasal intubation.

Indications, peroperative and postoperative complications and results of bicanaliculonasal silicone intubation are presented in a series of 80 eyes. Almost 100% of treated children were cured and 87% of adults were improved.

Two-year postoperative results of Galand lens implantation.

The Galand lens is a closed J-loop implant for capsular-bag fixation. We present a retrospective study of 214 eyes implanted with this lens. Eyes were examined at three months, six months, one year, and two years after surgery. Results indicate that 82.2% of eyes obtained a visual acuity of 20/40 or better at two years after implantation. If patients with other ocular pathology were excluded, the percentage of those whose visual acuity was 20/40 or better at two years was 93.7%. Although this implant avoids "spring action" within the capsular bag, the rate of dislocation into the posterior chamber was low (0.93% in this series). Early stability, before sealing of the capsular leaflets, was obtained by limiting the anterior capsulectomy to the pupillary area. No late zonular rupture was observed. Secondary posterior capsulotomy has been performed in 9.8% of the eyes.

Melanocytoma of the optic disc.

On the occasion of the observation of 4 cases of melanocytoma of the optic disc, the authors stress the relative rarity of this pigmented tumor, its benignity and its frequent association with a choroidalnevus. The melanocytoma is mostly stationary and produces no functional symptoms. It is generally diagnosed after the age of 30, and its pathogenesis is unknown.
