[Total gastrectomy extended to the head of the pancreas and the liver with D4 lymphadenectomy for well-differentiated gastric carcinoma].

Adenocarcinoma of the stomach is the second most common cancer worldwide. Early diagnosis and an adequate surgical approach could save live. Surgical approach is conditioned by the extension of the disease and the presence of metastases. Extended lymph-node dissection (D2) has been generally accepted as a standard treatment modality. However, the role of the super-extended lymphadenectomy (D4) for gastric cancer has not been established.

Association between hepatitis C virus and mixed cryoglobulinemia [see comment].

The prevalence of hepatitis C virus (HCV) RNA and of antibodies to HCV in an unselected series of 42 mixed cryoglobulinemia patients was investigated in this study. HCV RNA was detected by the polymerase chain reaction technique, and HCV antibodies by two enzyme-linked immunosorbent assays (Chiron ELISA HCV, Second Generation, Emeryville CA, USA; Wellcome Diagnostic, England). HCV RNA was found in 86% of the mixed cryoglobulinemia patients. Using either the Chiron ELISA or the Wellcome ELISA, HCV antibodies were present in 90% of the same samples; anti-HCVAb seropositivity was confirmed in all cases by immunoblot assay (Chiron RIBA HCV, Second Generation Assay). A striking correlation between HCV RNA and anti-HCV antibody seropositivities was recorded. HCV RNA in mixed cryoglobulinemia patients was not correlated with the presence or absence of biopsy-proven liver involvement. These results suggest that the association between HCV and mixed cryoglobulinemia is not fortuitous, and therefore that HCV may have an etiopathogenetic role in this disorder.

[Evaluation of fracture healing with the computerized analysis of radiographic images].

The radiographic evaluation of fracture healing in the long bones utilizes subjective and hardly analytic standards. Therefore, to solve this problem, the authors suggest using a computerized image processing system which increases radiographic informative content and allows an objective evaluation of fracture healing. We analyzed the radiographs of 15 patients suffering from femoral fracture and treated by means of Küntscher nailing. We processed the radiographs taken both before and 1, 2, 4, and 6 months after surgical treatment. It was thus possible to select the regions of interest, to graph brightness levels and to measure both distances and areas. Computerized image processing provides an objective and quantitative analysis of radiographs and allows an increase in informative content. It also allows the comparison of pre- and postoperative radiographs with those taken at different times. Our results may confirm the utility of computerized image analysis of radiographs to evaluate bone remodelling during fracture healing.

Death rate in patients submitted to the surgical treatment of fracture of the proximal femur.

The death rate 6 and 12 months after surgery in 389 patients who had been treated for fracture of the proximal femur between 1989 and 1993 (1st semester) was studied. All of the patients submitted to surgery were traced through the official records on the population of the City of Padova. The overall death rate after 6 months was 16%, that after 12 months 23%. The best survival rate was observed in patients who had been submitted to gamma nail arthroplasty (p < 0.01).

[Liver abscess: is drug therapy only possible?].

We describe a case of liver abscess, whose favourable evolution, after the initial medical treatment, started when we didn't know the diagnosis, persuaded us to continue the begun treatment, based on localization of disease and on clinical improvement, instead of carrying out on evacuative puncture. At moment, according to our knowledge, this is the third case of liver abscess which has recovered by on antibiotic rational treatment without surgical punctures. Can we treat the liver abscess like the pneumonia according to features of localization and clinical history?

[Hypertrophic cardiomyopathy (probably secondary) in an infant with Stickler's syndrome. The physiopathological aspects].

Stickler syndrome is a hereditary connective tissue disease transmitted by AD pattern. It is characterized by peculiar facies with bone and ocular anomalies. Heart anomalies (except mitral valve prolapse) are an uncommon features that may be associated. The authors report on a 4 months patient affected by this syndrome who had hypertrophic cardiomyopathy. Pathogenetic mechanism of this anomaly is discussed.

[A case of type II achondrogenesis].

We describe a rare case of type II achondrogenesis (gestational age = thirty-two weeks) dead forty-five minutes after birth. This congenital skeletal dysplasia is classified among the lethal osteochondrodysplasias. Clinical features were enough for diagnosis and autopsy added nothing to our clinical knowledges.

Etiopathogenetic role of hepatitis C virus in mixed cryoglobulinemia, chronic liver diseases and lymphomas.

Hepatitis C virus (HCV) infection has been found in the majority of patients with mixed cryoglobulinemia (MC) in studies conducted in different countries. In our series of 110 MC patients the frequency of HCV markers was significantly high (91%) compared with other rheumatic diseases (6.4%) and with healthy Italian controls (1.2%). Moreover, HCV RNA was detected in 81% of the peripheral lymphocytes from MC patients. Comparable percentages of HCV infection were detectable in other disorders, i.e. porphyria cutanea tarda (77%) and autoimmune hepatitis type 1 (77%). The HCV infection of peripheral lymphocytes suggests that this virus could be the triggering factor for the lymphoproliferation underlying MC. In a number of patients with MC the evolution from a benign lymphoproliferation to frank B-cell lymphoma was observed. In these subjects HCV RNA in the sera and in fresh and cultured peripheral lymphocytes was constantly detected. The same phenomenon has been observed in patients with long-lasting type C chronic hepatitis. Interestingly, HCV infection has also been recorded in 32% of idiopathic B-cell non-Hodgkin's lymphomas. Taken together, the above findings suggest that HCV can cause benign B-cell proliferation with the consequent production of various autoantibodies, including rheumatoid factor and mixed cryoglobulins. These serological abnormalities characterise different clinical disorders, including the appearance of lymphoma in a not negligible number of individuals.

[Early-onset hyperthyroidism with muscle involvement: a report on a patient].

A case of Graves' disease in a 2 years child old is reported. He presented tachycardia, loss of weight, anxiety and gait abnormality. The serum free T3 and T4 and thyrotropin receptor antibodies values were elevated, TSH was not measurable. The EMG was abnormal with short duration motor unit potentials.

[Sandifer's syndrome: a rare form of torticollis in childhood. A report of a patient].

Sandifer syndrome is an uncommon clinical entity characterized by gastroesophageal reflux, torticollis and paroxysmal dystonic postures. For the wide variability in clinical expression it is diagnosed as neurological disease. We report on a 3-year-old patient who presented sudden extensions of the head and neck with tilting of the head one side and severe arching of the spine. It is presented a review of the related literature.

[Melnick-Needles syndrome. Report of a case].

Melnick-Needles osteodisplasty is a X-linked dominant syndrome characterized by generalized bone dysplasia and abnormal facies. The Authors report on a 13 year old girl affected by severe valgus. At clinical examination the patient showed exophthalmos, full cheeks and micrognathia. Radiological examination revealed sclerosis of the skull base and mastoids, bowed radius and ulna (S-shaped), coxa valga and abnormal vertebral.
