[Most common cardiovascular diseases and sleep apnea syndrome. A general population study].

BACKGROUND: The sleep apnea syndrome (SAS) is a frequent disease associated with significant morbidity. The aim of our study was to investigate diseases associated with the sleep apnea syndrome (SAS) in general population. METHODS: We selected a random sample of 110 people from the electoral census. These people were invited to the clinic where medical history, physical examination and monitoring for sleep-disordered breathing was done. RESULTS: Twenty two subjects were diagnosed of SAS. The prevalence of arterial hypertension in the SAS group was 36.4%, and coronary artery disease 13.6%. CONCLUSIONS: Although the prevalence of this diseases was increased in the SAS group, we do not see significant association with this disease.

Trismus and pain after removal of impacted lower third molars.

PURPOSE: This study evaluated trismus and pain after removal of impacted lower third molars and investigated whether these responses were related to difficulty of surgery. PATIENTS AND METHODS: A consecutive series of 104 patients, all of whom underwent removal of an impacted lower third molar under local surgery, was studied. Difficulty of surgery was evaluated on a modified version of the Parant scale: I, extraction with forceps only; II, extraction by ostectomy; III, extraction by ostectomy and coronal section; IV, complex procedures. Trismus was evaluated in terms of maximum interincisal distance (MID) 1 and 5 days after surgery. Pain was evaluated on the basis of reported analgesic use 1 and 5 days after surgery. RESULTS: Among group I subjects, mean day 1 MID did not differ significantly (P > .05) from mean presurgery MID, whereas mean day 1 MID in groups II, III, and IV was significantly lower than before surgery. In groups II, III, and IV, mean day 5 MID remained lower than before surgery. The proportion of group I patients using analgesics was significantly lower on both days 1 and 5 than the proportion of patients using analgesics in groups II, III, and IV. In all groups, the proportion of patients using analgesics dropped significantly between days 1 and 5. CONCLUSION: Trismus is less severe after simple (forceps-only, grade I) extractions than after surgical extractions (grades II to IV). However, trismus severity after surgical extraction does not depend on difficulty of surgery. Pain, as revealed by reported analgesic use, is likewise less severe after simple extractions. Regardless of extraction type, pain declines between days 1 and 5 postsurgery.

[Chronic morbidity and self-perception of health in the elderly of a rural community].

OBJECTIVES: To find chronic morbidity, self-perception of health status and its relationship with different socio-economic and health variables, among the elderly in our health area. DESIGN: A crossover study using a home questionnaire. SETTING: A rural area in Galicia. PARTICIPANTS: 408 people over 65, chosen by simple random sampling from the census. INTERVENTIONS: Data referring to socio-demographic features, chronic morbidity, self-perception of health, use of health services and drugs consumption, were extracted from a previously validated questionnaire containing 100 items. RESULTS: A high prevalence of chronic pathologies was found. 72% of those surveyed stated that they had some chronic illness, with eye disorders (71%), arthrosis (71%) and hypoacusia (44%) being the most common. 73% said they had taken medicine during the previous fortnight. 32% had consulted a health professional in the previous two weeks. Almost a quarter (23.8%) of those surveyed had self-perception of poor health, which was associated with feminine gender (p = 0.001), drug consumption (p = 0.0002), considering their economic resources insufficient (p = 0.02) and getting on badly with their family (p = 0.008). However, we found no association with living alone, nor with consulting health professionals. CONCLUSIONS: There is high prevalence of chronic illnesses among the elderly. Self-perception of poor health is related not only to health problems, but also to economic and family problems.

[Activity of leukocyte elastase in coronary disease diagnosed by coronarography].

OBJECTIVE: To evaluate the clinical usefulness of leukocyte elastase concentration for diagnosis of coronary artery disease. BACKGROUND: Recent research has shown the important role elastase, a proteolytic enzyme released by neutrophils, in the pathogenesis of coronary atherosclerotic disease. METHODS: 95 patients underwent coronary angiography during investigation of chest pain and/or heart valve disease; 38 had normal coronary arteriograms (Group I) and 57 had coronary lesions (Group II). The patients were characterized as regards presence or absence of stable or unstable angina, family history of coronary artery disease, smoking, diabetes mellitus, hypertension, leukocyte counts, plasma lipid and elastase concentrations. Among Group II patients, those with simple atheromatous plaques were distinguished from those with complex plaques. RESULTS: Elastase concentrations were greater in the Group II than in the Group I (41 +/- 21 vs 27 +/- 14 micrograms/L) (p < 0.001), and greater among complex plaque patients than among simple plaque patients (53 +/- 27 vs 33 +/- 12 micrograms/L) (p < 0.001). Logistic regression analysis showed than elastase concentration, angina, age and sex had independent value for prediction of coronary artery disease and that the risk increased by 7% for every 1 microgram/L increase in elastase concentration. Among Group II patients, the risk of complex plaque was greatest for those with unstable angina and high elastase concentration, increasing by 6% for every 1 microgram/L increase in elastase concentration. CONCLUSIONS: Peripheral blood leukocyte elastase concentration is a sensitive diagnostic marker of coronary artery disease. High values suggest the presence of complex atheromatous plaques.

[Nitrendipine in the treatment of mild-moderate essential arterial hypertension. Ambulatory study in Primary Care].

A non-comparative open clinical pharmacological surveillance study was conducted, in which 427 patients from both sexes with diagnosis of light to moderate essential arterial hypertension were included. All patients were treated with nitrenpine, a dihydropyridinic calcium antagonist during a 28-week period. The initial dosage was 10 mg/day, increasing such dosage to 20 and 40 mg/day and associating it with a thiazidic diuretic in those patients showing no antihypertensive response (diastolic arterial pressure (DAP) of 90 mmHg or a reduction of 10 mmHg in baseline figures). A significant decrease was observed in systolic arterial pressure (from 159.3 +/- 12.6 to 149.2 +/- 12.2 mmHg) and in Dap (from 94.8 +/- 9.6 to 86.7 +/- 10.5 mmHg), p < 0.001, with effectivity in 66.3% of patients (65% receiving monotherapy; 10.3% combined therapy: nitrendipine and diuretic). No modifications were observed in heart rate nor in the biological-metabolic parameters under study. Side effects had a low incidence (15.9% of patients, with a 4.7% of drop-outs) and were well-tolerated in most cases. Given these results, we can conclude that nitrendipine is an effective and safe drug for the treatment of light to moderate arterial hypertension.

[Pediatric morbidity in a health care center].

We have evaluated the characteristics of the pediatric demand in a health center during one year. Data were obtained by statistical sampling. High-frequency consultation motives were administrative procedures and medical examinations. Acute respiratory tract infections are the most common acute pathology. Acute childhood diarrhoea, traumatisms and tuberculosis are high incidence pathologies.
