Splenectomy during primary maximal cytoreductive surgery for epithelial ovarian cancer.

Six patients with epithelial ovarian cancer underwent splenectomy as part of their extensive primary cytoreductive surgical management at the Johannesburg University Hospital. These patients represented 7.6% (6 of 79) of advanced-disease patients (stages III and IV) followed during the study period. Five splenectomies were performed because of metastatic disease confirmed on microscopy. One of these showed parenchymal involvement and represented only the third case in the world literature. The remaining patient required splenectomy for capsular avulsion injury and resultant bleeding. Late thrombotic complications occurred in 4 of 6 (67%) patients, resulting in death of one patient from massive pulmonary embolism 20 days after surgery. Three patients (50%) currently have no evidence of disease 14, 25, and 32 months after surgery. The remaining 2 patients died of recurrent disease 19 and 58 months following primary operation. This, the largest reported series in the English literature, indicates that splenectomy, albeit potentially hazardous, is justified when it permits optimal cytoreductive surgery.

Non-gynaecological metastases to the vulva and vagina.

A retrospective study of 1513 vulval and vaginal biopsy specimens was undertaken to determine the frequency of non-gynaecological metastases to the vulva and the vagina; 7 cases were found. These were reviewed and an attempt was made to determine the mode of spread.

Combined chemotherapy and irradiation therapy after radical surgery for leiomyosarcoma of the vulva. A case report.

Radical surgery failed to remove a leiomyosarcoma of the vulva completely, and residual tumour was left. Chemotherapy did not inhibit further local growth or the occurrence of secondary lesions in the lung. Irradiation therapy of the pelvis, including the area of the tumour, then successfully inhibited any further local growth, as evidenced by a marked decrease in size and eventual disappearance of the mass; however, the patient succumbed to distant metastasis.

Ruptured tubo-ovarian abscess late in pregnancy. A case report.

A pregnancy complicated in the late second trimester by a tubo-ovarian abscess, generalised peritonitis and a subdiaphragmatic abscess is reported. The fetus did not survive. The English-language literature is reviewed, as well as possible aetiological mechanisms and methods of management.

Experimental estimation of intrauterine fetal irradiation dosage in computed tomography using a Rando Phantom.

A Rando Phantom was used experimentally to estimate the dosage of intrauterine irradiation a fetus would be subjected to if a computed tomographic investigation were to be performed antenatally. Lithium fluoride thermoluminescent dosimetry chips (TLD chips) were placed in the assumed intrauterine fetal position in a Rando Phantom. A scanogram was then used to identify the location of the chips and a computed tomographic scan was performed of the assumed intrauterine fetal position. It was felt that the measurement obtained from the TLD chips would give a more realistic estimate of the total dose that a fetus may be subjected to, both due to the primary and the scattered irradiation of the computed tomographic examination.

Pregnancy in patients with prosthetic cardiac valves.

Thirty-seven pregnancies in 33 patients who had prosthetic cardiac valves inserted at Baragwanath Hospital are reviewed. The patients were divided into three groups, as determined by fetal and maternal outcome. There were 31 live births, and the patients seemed to deal well with the haemodynamic stresses of pregnancy. There were 2 maternal deaths, and 6 patients required a second valve replacement during the pregnancy under review. The problem of longterm anticoagulant therapy and its management is discussed.
