Experimental carcinoma of liver in macaque monkeys exposed to diethylnitrosamine and hepatitis B virus.

The effects of treatments with diethylnitrosamine (DENA) and hepatitis B virus (HBV) on macaque monkeys were investigated by virus serology and by light and electron microscopy. The experimental groups comprised 43 newborn or juvenile cynomolgus and rhesus monkeys of both sexes. HBV neither had a carcinogenic effect nor increased the oncogenic effect of DENA. However, HBV given to juvenile primates before treatment with DENA resulted in subsequent gross and microscopic alterations consistent with mild hepatitis and postnecrotic cirrhosis; multifocal liver carcinoma apparently developed within these cirrhotic nodules. The pathologic findings in the experimental animals were strikingly similar to those observed in liver cancer patients.

Methoxyflurane hepatitis.

A veterinarian's assistant who sniffed methoxyflurane as a euphoriant developed fulminant hepatitis and died of hepatic failure. The abuse of fluorinated anesthetics has been reported and may be more frequent than is commonly believed. It is suggested that fluorinated anesthetics should be considered as a causative agent in a case of fulminant hepatitis, especially in patients who have easy access to such drugs.

Nuclear and nucleolar ultrastructure of Sézary cells.

Sézary cells were studied in the peripheral blood and characteristic skin lesions of the Sézary syndrome and mycosis fungoides by transmission electron microscopy to obtain more information on their nuclear and nucleolar ultrastructure. Sézary cells contain nucleoli with nucleolonemas or ring-shaped nucleoli similar to those of lymphoblasts and mature lymphocytes. "Maturation asynchrony" of the nucleolus and cytoplasm was evident in some cells that contain large numbers of ribosomes and ring-shaped nucleoli and in other cells that contain nucleoi with nucleolonemas and few ribosomes. The maturation asynchrony of the nucleolus and the cytoplasm, the presence of mitochondrion-like inclusion bodies in the nucleus, and fusion of mitochondria with the nucleus in Sézary cells are ultrastructural abnormalities of this neoplastic lymphocytic variant. The presence of the intranuclear "mitochondrion-like" inclusion body and nuclear rodlets in Sézary cells were exceptional findings.

Electron microscopy in the diagnosis of giardiasis.

Biopsy specimens of intestinal mucosa from two patients with malabsorption syndrome were examined with routine light and electron microscopy. Both showed intact intestinal mucosa, but by electron microscopy, the presence of Giardia lamblia in the intervillar spaces was identified. Electron microscopy is useful for the identification of this parasite, since the stool examinations in up to 50% may be negative. The parasites appear in light microscopy as cellular debris and can be easily overlooked, while by electron microscopy the typical morphologic features of the parasite are diagnostic.

Zinc binding by glycosaminoglycans.

Seven different standard GAGs were shown to bind various amounts of 65Zn by gel filtration on Sephadex G-25 at pH 4.0 and pH 7.0. "Molar" zinc-binding ratios, calculated on the basis of molar bound 65Zn per hexuronic acid and sulfate of each GAG, were significantly different for C4S, C6S, DS, HA, HS, at the two pHs; however, there was no difference for HP and KS. Moreover, while the binding ratios of C4S, C6S, and DS were higher, those of HA and HS were lower at pH 4.0 than at pH 7.0. The order of increasing zinc-binding ratios was (i) HS, KS, HA, C6S, C4S, DS, HP at pH 4.0 and (ii) C6S, KS, C4S, DS, HS, HP, HA at pH 7.0.

Effect of routine treadmill testing on the serum enzymes.

A number of serum enzymes increase after strenuous exercise. However, there is no information concerning these enzymes after routine treadmill testing, SGOT, CPK and LDH were determined before, immediately after, at 4 and 24 h postexercise in 100 subjects undergoing this test. When the group was considered as a whole, only the SGOT showed a slight but statistically significant elevation immediately after exercise. The mean values of the SGOT at 4 and 24 h, as well as the mean values for the CPK and LDH at all times after exercise, were not significantly increased. SGOT increased more significantly immediately after exercise in those patients who exercised more than 6 min. This group of patients also showed modest but statistically significant elevations of their CPK immediately after completion of the exercise test. Analyses of the data in relation to the presence or absence of ischemic ST shifts and presence or absence of documented coronary obstruction, failed to show any significant correlation. The effect of conditioning on the serum enzyme changes postexercise was not demonstrable.

Ultrastructural study of pulmonary bleomycin toxicity.

Ultrastructural manifestations of bleomycin A2 toxicity in the human lung were studied in three patients. In addition to the appearance of nucleolar fibrillar centers, an increase in membranous, beaded, and granular nuclear bodies was found in nuclei of type 1, type 2 alveolar epithelial cells, and interstitial fibroblasts in all treated patients. Few such nuclear bodies were found in specimens of untreated patients.

Primary rhabdomyosarcoma of the cerebrum.

A primary cerebral rhabdomyosarcoma is described in a 48-year-old Negro man. At autopsy two metastatic nodules were present in the liver. A review of the literature indicates that this is a rare and highly malignant tumor with no 5-year survival. The multipotentiality of the mesenchymal tissue in the leptomeninges is considered the histogenetic origin of intracranial rhabdomyosarcomas.

The usefulness of electron microscopy in the diagnosis of human tumors.

Ultrastructural features of neoplastic cells can provide clues for correct diagnosis when light microscopy fails. Secretory granules are characteristic in the following tumors: mucin granules in poorly differentiated adenocarcinomas, zymogen granules in acinic cell carcinomas, lysosomal granules in prostatic carcinomas, melanin granules in malignant melanoma, carcinoid, islet cell tumors, pheochromocytoma, and neuroblastoma granules in the corresponding neoplasms. Among cytoplasmic organelles, rough surfaced endoplasmic reticulum characterizes adrenocortical, ovarian, and hepatocellular carcinomas and plasmacytomas. Tonofibrils are characteristic of squamous cell carcinomas. Glycogen deposits distinguish Ewing's sarcoma from lymphoreticular neoplasms. Intercellular relationships and membrane specialization are important features in the differential diagnosis of various undifferentiated tumors. The frequent resolution of difficult diagnostic problems by electron microscopy outweighs the disadvantages of this technique, such as the expense and time required.

Cytoplasmic Granules

Spontaneous, nontraumatic gangrene due to Clostridium septicum.

Clostridium septicum is a major cause of spontaneous, nontraumatic gas gangrene. Unlike Clostridium perfringens, C. septicum is relatively aerotolerant and thus appears to be more capable of initiating infection in the absence of obvious damage to tissues. Six cases illustrate the clinical setting and fulminant nature of spontaneous gangrene caused by C. septicum. A lesion in the colon such as carcinoma is often present and is presumed to serve as a portal of entry to the bloodstream. Diabetes and leukopenia are also common predisposing conditions; compromise of vital host responses may facilitate proliferation of those organisms that settle out in the tissues. Acute lymphoma or leukemia during a course of chemotherapy is accompanied by damage to bowel mucosa and granulocytopenia, thus predisposing to spontaneous clostridial gangrene. Infection progresses in a fulminating manner; the majority of patients die within 24 hours of onset. Characteristic symptoms and signs include excruciating pain (although a sense of heaviness may be the only early symptom), swelling of tissues, crepitance, and bulla formation. A hallmark of C. septicum infection is the absence of acute inflammatory cells in involved tissues or in bulla fluid. A series of laboratory investigations demonstrated that fluid obtained from a bulla adversely affected the viability, morphology, and function of polymorphonuclear leukocytes (PMNs), which may explain the paucity of PMNs in involved tissues and may in part contribute to the fulminant progression observed in infection due to this organism.

Endogenous DNA polymerase-positive core particles from hepatitis-infected hepatocytes.

The purification is described of liver-derived hepatitis B core antigen (HBc Ag) from human hepatocytes demonstrating only intranuclear particles by electron microscopy. The purified preparation contained 5 x 10(11) particles per ml. The particles were mono-dispersed and relatively free of background material. DNA-dependent polymerase activity was present and had greater activity than an equal number of cores derived from Dane particles isolated from plasma. Specificity of the polymerase reaction was confirmed by precipitation of the activity with specific anti-HBc antiserum. A proportion of the liver-derived core particles was nonreactive for DNA polymerase activity. The polymerase-positive population of particles had a larger size than the polymerase-negative population of liver-derived cores as evidenced by gel filtration in Sepharose 4B.

Mirror image aneurysms of the ulnar artery in the palm.

A case of sequential bilateral ulnar artery aneurysms in the palm is reported. Thirty-five unilateral true aneurysms in the hypothenar area are reported in the literature and most have been associated with repeated trauma to the palm. The diagnosis is easily established from physical examination. Definition of the anatomic relationships of the aneurysm is by selective arteriography. Surgical excision or ligation of the communicating arteries is curative.
