Up-to-date aspects of meningoencephalitis. Anatomo-clinical researches.

The anatomo-clinical study of the deaths caused by meningoencephalitis at the Infectious Diseases Clinics in Bucharest in 1986 and 1987 showed the increase of morbidity, 194 lethal cases being registered. The most affected age group was up to 1 year, followed by older persons, then by children between 1 and 10 years, the male sex prevailing. The most incriminated pathogenic agents were meningococcus, pneumococcus, as well as negative Gram germs, Escherichia coli, whereas the favouring factors were especially malnutrition, premature birth, dystrophies, focal infections. The lesional pictures are described, underlying the frequency of vascular lesions, ventriculitis cerebral abscesses, as well as the role of demyelinizations and of the glial reactions in pathogenesis.

Anorectal melanoma. Problems of histopathologic diagnosis.

The present study discusses, in the light of the comparatively restricted data in the speciality literature, two cases of anorectal melanoma emphasizing the histologic diagnostic difficulties linked to appraisal of the limits of the benign naevus formations, predominance of the achromatous zones, alternation of the benign and sarcomatous fascicular proliferations and those of the anaplastic carcinoma type. Stress is laid on the importance for the diagnosis of the histologic examination and of serial sections, in all pedunculate formations in the anorectal region as well as of the tests applied for detecting melanin.

Hodgkin's disease with predominant epitheloid cells.

The present paper reports on a 38-year-old patient with left supraclavicular adenopathy, clinically suspect of tuberculosis. Histologic examination showed disappearance of the lymph node structure due to diffuse, nodular hyperplasia of epithelioid and Sternberg-Reed cells. The particularities of this rare form of Hodgkin's disease, described by Lennert, are discussed, as well as problems of differential diagnosis.

Therapeutic effect of intralesional interferon (Roferon) in squamous cell carcinoma.

Recombinant alpha-2 interferon (IFN)--Roferon--100,000 IU/ml was intralesionally administered in 8 cases of squamous cell carcinoma (SCC) three times a week during 4-6 weeks in inoculations of 1 ml each. The therapeutic effect was scored as major--more than 60% reduction of the tumor size, moderate--30-60% reduction of the tumor mass and, nonreactive--less than 30% reduction of the tumor size. Three cases showed a major reduction, three showed a moderate reduction and two patients showed no reduction of the tumor volume. Histopathological examination of the surgically removed tumors after completion of the Roferon administration confirmed the clinical diagnosis of squamous cell carcinoma and revealed that an intense leukocyte, mainly lymphocytes, infiltration can be observed along with necrotic centers, progressively surrounding and reducing in size the tumor islets, thus proving an intense activation of the immune effector reactions against tumor cells.

Experimental investigations in acute myocardial ischaemia. Note II. Studies concerning morphophysiologie alterations of the ischaemic myocardium under vascularization conditions.

The biology of the myocardial fiber was studied under experimental conditions similar to angina and the premonitory syndrome of myocardial infarction. The morphophysiologic data obtained particularly concerning the mitochondria demonstrated that affection of the myocardium by progressive ischaemia induced less severe lesions than the abrupt onset of ischaemia, also lowering the frequency of ventricular fibrillations.

Histopathological aspects of pulmonary carcinoid tumors.

Pulmonary carcinoid tumors are considered low grade malignant tumors, arising from neuroendocrine cells from bronchial mucosa. The small cell proliferation is arranged in small nests or trabeculae, the nuclei are round to oval with finely dispersed chromatin, indistinct nucleoli, small amount of cytoplasm, indistinct borders. Problems of differential diagnosis could appear in distinction with others malignancies like adenocarcinomas, squamous cell carcinomas, lymphomas and others neuroendocrine tumors, especially in the different prognosis and therapeutic approach. We described 15 cases of pulmonary carcinoid tumors diagnosticated by morphologic and immunohistochemical methods.

Morphological and immunohistochemical features of tumor infiltrating lymphocytes (TIL) in carcinomas. Note I.

Some malignant tumours like testicular seminomas and ovarian dysgerminomas or medullar carcinoma of the breast, present an unusually lympho-histiocytic (TIL) rich stroma. Many reports have concluded that the prognosis for these patients is correlated with the intensity of TIL. Recently, some analyses consider that tumour-host interactions have a significant prognostic role in many other neoplasms. The presence of TIL may be a sign of less aggressive behaviour of some epithelial neoplasm like gastro-intestinal, pulmonary, mammary, urinary or cutaneous carcinomas (Wilson et al.). The aim of this study is a morphological and immunohistochemical (IHC) characterisation of the lympho-histiocytic populations of TIL in some gastro-intestinal and mammary carcinomas. The two localisation were chosen for their different contact with the exogene antigens and their possible different type of host's immune response.

Human leukocyte interferon treatment associated with IL-2 in the non-AIDS related Kaposi's sarcoma.

Patients with classical European Kaposi's sarcoma have been treated by intra- and peritumoral injections of human alpha-leukocyte interferon (IFN) (12 cases) or alternatively with IFN and naturally synthetized IL-2 (8 cases). All the patients were HIV negative their tumour appearing at least six months before. In each patient one tumour received 1 ml (50,000 IU) IFM alone or associated, alternatively with 1 ml IL-2, twice a week during 4-6 weeks, whereas another nodule situated 8-10 cm apart was considered as control and remained uninjected. The clinical follow-up revealed that in the same patient in the same anatomical area the treated nodule was cured in all the investigated cases while the untreated nodule was not. These data undoubtedly prove that IFN is the responsible factor for the involution and final cure of the Kaposi tumours treated by perilesional inoculations. Association with IL-2 (and certainly also other interleukins) increases the beneficial clinical effect activating the tumour involution. The histological examination showed that important histopathological changes occur in the treated nodules, i.e., complete disappearance of the Kaposi's aspect; fibrosclerous modifications progressively replacing the abundant fibroblastic cells characteristic of Kaposi's tumour; abundant infiltrations of leukocytes especially lymphocytes; necrotic patches often appearing along with hemorrhagic centers. IL-2 association seems to frequently induce especially this last type of histological phenomena.

The role of tumor angiogenesis factor (TAF) in the tumor growth. Pathological aspects.

This work was carried out on control and immunosuppressed A2G mice inoculated with murine tumour cells (Ehrlich ascites) and human ones (KB cell line). The growth of the tumours was conditioned by the growth of the fibrovascular stromas as a response of the host organism to the TAF secretion. This process is a nonimmunological response, the differences between control and immunosuppressed mice being of no statistical significance.

Intralesional human leukocyte interferon treatment in the non-AIDS related Kaposi's sarcoma.

Twelve patients with classical European Kaposi's sarcoma have been treated by intra- and peritumoral injections of human alpha leukocyte interferon (IFN)--Ginterferon ("V. Babeş Institute", Bucharest). All the patients were HIV negative their tumour appearing at least six months before. In each patient one tumour received 1 ml (50,000 IU) IFN twice a week during 6-7 weeks whereas another nodule considered as control remained uninjected. After treatment biopsies of both the IFN-treated and the untreated nodules were performed in 8 patients and histological examinations were carried out. The clinical follow-up revealed: The skin colour progressively changed from purple red to dark reddish or even brown in all the IFN-treated tumours and only in 3 of the untreated ones (p less than 0.001). The consistency of the nodules decreased in all the treated tumours and only in 2 of the untreated ones (p less than 0.001). The tumour thickness decreased in 9 of the treated tumours and in none of the untreated ones (p less than 0.001). The surface of the lesions decreased in 4 of the treated nodules but not in the untreated ones (p less than 0.05). The IFN-inoculated tumours were gradually cured even in the four cases in which during the treatment some other tumour progressed or even new lesions appeared. The histological examination showed that: all the IFN-treated tumours were either predominantly sarcomatous or predominantly angiomatous. The typical Kaposi aspect disappeared totally in 2 of the cases examined, and in 3 a fibrosclerotic massive change was obvious.(ABSTRACT TRUNCATED AT 250 WORDS)

Leiomyoblastoma of the lesser omentum sac. A case report.

Localization of primary solid tumors on the omentum is rare and of leiomyoblastoma exceptional. A 57-year-old patient presented on the omentum an encapsulated, lumpy tumoral formation, 9 x 4.5 x 2.5 cm, with whitish-grey and hemorrhagic cystic zones. Predominant histopathologically were images of leiomyoblastoma infiltrated with fascicular and pleomorphic zones, recent thrombosis and hemorrhage. The postoperative evolution was satisfactory after three years.

Sweat gland tumors with stromal haemangiomatous component: an undescribed type of skin tumors. Report of four cases.

An undescribed type of skin tumour, discovered in four cases, is reported. It is characterized by sweat-gland tumours (benign or malignant), with a common stromal haemangiomatous component which confers to it a peculiar pattern. Histogenetic possibilities are discussed. A collision mechanism between a pre-existent hamartomatous component (haemangioma) and a recent sweat-gland growth may be responsible for the appearance of this type and we think that this picture is more frequent but unidentified because, to our knowledge, it was not described up to the present paper.
