Efficacy and safety of ebastine 20 mg compared to loratadine 10 mg once daily in the treatment of seasonal allergic rhinitis: a randomized, double-blind, placebo-controlled study.

BACKGROUND: Few randomized studies have compared loratadine to ebastine in the symptomatic treatment of seasonal allergic rhinitis (SAR) patients. METHODS: This double-blind, placebo-controlled, randomized, parallel-group, comparative trial compared the efficacy and safety of ebastine 20 mg (E20), loratadine 10 mg (L10) and placebo (P), administered once daily, in the control of SAR symptoms over a 2-week period. An additional 2-week treatment period was included in order to check sustained efficacy and tolerability. RESULTS: A total of 703 patients were enrolled: 282 patients in the E20 group, 279 in the L10 group and 142 in the P group. E20 showed a greater decrease from baseline in the main efficacy variable (mean daily reflective total symptom score) than L10 (p = 0.0018) or P (p = 0.0024), whereas the difference between L10 and P was not significant. Moreover, reductions from baseline in all composite/individual daily reflective rhinitis symptom scores were significantly larger in patients receiving E20 than in patients receiving L10 or P. Most significant differences between E20 and L10 or P were maintained after 4 weeks of treatment. Overall, all treatments were safe and well tolerated. There was no significant difference in the percentage of patients who reported one or more adverse events (AEs) between the groups, and most AEs were mild to moderate (89.9%). CONCLUSIONS: E20 given once daily for 2 weeks was more effective in the treatment of SAR symptoms than L10 or P. E20 also showed a sustained efficacy after 4 weeks of treatment, and overall was well tolerated and proved safe.

A randomized, double-blind, placebo-controlled study comparing the efficacy and safety of ebastine (20 mg and 10 mg) to loratadine 10 mg once daily in the treatment of seasonal allergic rhinitis.

Few randomized studies have compared the H1-receptor antagonists loratadine and ebastine in seasonal allergic rhinitis (SAR) patients. The objective of this study was to compare the efficacy and safety of ebastine 20 mg (E20), ebastine 10 mg (E10), loratadine 10 mg (L10), and placebo (P), once daily, in controlling symptoms of SAR over a 4-week period. This was a double-blind, placebo-controlled, randomized, parallel-group study. Efficacy was assessed in 749 patients (12 to 70 years old) by SAR symptom scores (nasal discharge, congestion, itching, sneezing, and total eye symptoms) entered on diary cards every morning and every evening over the previous 12 hours (reflective score) and at the time of recording (snapshot score). The E20 group showed greater reductions from baseline compared with the L10 group in 2 daily reflective composite scores (nasal index [with or without congestion]) and in all 4 daily snapshot composite scores. E10 and L10 groups showed no significant differences in either the daily reflective or snapshot scores overall although E10 showed a greater improvement of nasal discharge snapshot score than L10. The efficacy of E20 at controlling the symptoms of SAR was well sustained during the fourth week of treatment, with significant differences over placebo in 22/36 total rhinitis symptom scores, followed by E10 (6/36), whereas L10 showed no differences (0/36). Patient and physician global evaluations at the final visit were not statistically significant for any treatment group compared with placebo. There was no significant difference among all groups in the number of patients who reported adverse events. In conclusion, ebastine 20 mg given once daily for 4 weeks in the treatment of SAR showed larger mean reductions from baseline in most rhinitis symptoms scores than loratadine 10 mg. Sustained efficacy was most frequently observed with ebastine 20 mg over placebo, whereas loratadine 10 mg did not provide a statistically significant improvement in any individual or composite symptom score at the end of the fourth week. Both ebastine 20 and 10 mg were well tolerated and proved safe in the treatment of SAR.

Efficacy of ebastine in the control of nasal congestion associated with allergic rhinitis.

Three similarly designed, multicenter, double-blind, randomized, placebo-controlled, parallel-group comparative studies were carried out in the United States in a total of 1,881 patients to evaluate the efficacy of ebastine 20 mg (E20), ebastine 10 mg (E10), loratadine 10 mg (L10), and placebo (P), all given once daily, in controlling the symptoms of ragweed-induced rhinitis over a 4-week treatment period. Efficacy was assessed, among other means, by nasal congestion symptom scores entered by patients on diary cards in the morning and before bedtime over the previous 12-h period (reflective score, R) and at the time of recording (snapshot score, SS). Mean value of both morning and evening score changes from baseline were analyzed in each study and for each treatment. E20 was more effective than placebo in all studies, in both R and SS symptom scores (6 of 6 scores), while E10 was effective in 4 of 6 scores (2 R and 2 SS). In contrast, L10 was effective in only 1 of 6 scores (1 R). In conclusion, the comparative analysis of the results from these three trials shows that ebastine is efficacious in the reduction of nasal congestion associated with seasonal allergic rhinitis. This symptomatic effect of ebastine may be accounted for by its ability to reduce inflammatory markers, as shown in preclinical studies, in addition to its primary effect of antagonizing histamine H1 receptors.

The reactivity of N-coordinated amides in metallopeptide frameworks: molecular events in metal-induced pathogenic pathways?

The amino acid derived tertiary amide ligand tert-butoxycarbonyl-(S)-alanine-N,N-bis(picolyl)amide (Boc-(S)-Ala-bpa, 1) has been synthesized as a model for metal-coordinating peptide frameworks. Its reactions with copper(II) and cadmium(II) salts have been studied. Binding of Cu2+ results in amide bond cleavage and formation of [(bpa)(solvent)Cu]2+ complexes. In contrast, the stable, eight-coordinate complex [(Boc-(S)-Ala-bpa)Cd(NO3)2] (5) has been isolated and characterized by X-ray crystallography. An unusual tertiary amide nitrogen coordination is observed in 5; this gives rise to significantly reduced cis-trans isomerization barriers. Possible implications for metal-induced conformational changes in proteins are discussed.

Olefin metatheses in metal coordination spheres: versatile new strategies for the construction of novel monohapto or polyhapto cyclic, macrocyclic, polymacrocyclic, and bridging ligands.

The broad applicability of the title reaction is established through studies of neutral and charged, coordinatively saturated and unsaturated, octahedral and square planar rhenium, platinum, rhodium, and tungsten complexes with cyclopentadienyl, phosphine, and thioether ligands which contain terminal olefins. Grubbs' catalyst, [Ru(=CHPh)(PCy3)2(Cl)2], is used at 2-9 mol% levels (0.0095-0.00042 M, CH2-Cl2). Key data are as follows: [(eta5-C5H4(CH2)6CH=CH2)Re(NO)(PPh3)-(CH3)], intermolecular metathesis (95 %); [(eta5-C5H5)Re(NO)(PPh3)(E(CH2CH=CH2)2)]+ TfO (E=S, PMe, PPh), formation of five-membered heterocycles (96-64%; crystal structure E = PMe); [(eta5-C5Me5)Re(NO)(PPh((CH2)6CH=CH2)2)(L)]n+ nBF4-(L/n = CO/1, Cl/0), intramolecular macrocyclization (94-89%; crystal structure L= Cl); fac-[(CO)3Re(Br)(PPh2(CH2)6CH=CH2)2] and cis-[(Cl)2Pt(PPh2(CH2)6CH=CH2)2], intramolecular macrocyclizations (80-71%; crystal structures of each and a hydrogenation product); cis-[(Cl)2Pt(S(R)(CH2)6CH= CH2)2], intra-/intermolecular macrocyclization (R=Et, 55%/24%; tBu, 72%/ <4%); trans-[(Cl)(L)M(PPh2(CH2)6CH=CH2)2] (M/L = Rh/CO, Pt/C6F5) intramolecular macrocyclization (90-83%; crystal structure of hydrogenation product, M=Pt); fac-[W(CO)3(PPh((CH2)6CH=CH2)2)3], intramolecular trimacrocyclization (83 %) to a complex mixture of triphosphine, diphosphine/ monophosphine, and tris(monophosphine) complexes, from which two isomers of the first type are crystallized. The macrocycle conformations, and basis for the high yields, are analyzed.

A meso-helical coordination polymer from achiral dinuclear [Cu2(H3CCN)2(micro-pydz)3][PF6]2 and 1,3-bis(diphenylphosphanyl)propane-synthesis and crystal structure of 1 to infinity Cu(mu-pydz)2][PF6 (pydz=pyridazine).

Reaction of achiral [Cu2(H3CCN)2(mu-pydz)3][PF6]2 (1) (pydz = pyridazine) with bidendate 1,3-bis(diphenylphosphanyl)propane (2) in acetonitrile at room temperature in a 1:1 ratio yielded the mononuclear copper(I) complex [Cu[CH2(CH2PPh2)2]2][PF6] (3) together with new one-dimensional coordination polymer 1 to infinity[[Cu(mu-pydz)2][PF6]] (4). Air-sensitive single crystals of 4, suitable for X-ray structure determination, were grown from a mixture of dichloromethane/ hexane [crystal system: monoclinic; space group: C2/c: a = 21.910(3), b = 12.130(2), c = 25.704(3) A,beta = 110.08(10) degrees, V = 6416.65(16) A3]. The one-dimensional coordination polymer 1 to infinity[[Cu(mu-pydz)2][PF6]] (4) exhibits as outstanding feature the rare structure of a meso-helix.

Ligand-to-metal ratio controlled assembly of tetra- and hexanuclear clusters towards single-molecule magnets.

A simple template-mediated route, starting from triethalolamine 1, sodium hydride or caesium carbonate, and iron(III) chloride led to the six- and eight-membered iron coronates [Na c [Fe6[N(CH2CH2O)3]6]]+ (2) and [Cs c (Fe8[N(CH2CH2O)3]8]]+ (3). In the reaction of N-methyldiethanolamine 4 (H2L1) or N-(2,5-dimethylbenzyl)iminodiethanol 6 (H2L2) with calcium hydride followed by addition of a solution of iron(III) chloride, the neutral unoccupied coronands [Fe6Cl6(L1)6] (5) and [Fe6Cl6(L2)6] (7) were formed. Subsequent exchange of the chloride ions of 7 by bromide or thiocyanate ions afforded the ferric wheels [Fe6Br6(L2)6] (8) or [Fe6(NCS)6(L2)6] (9), respectively. Titration experiments of solutions of dianion (L1)2- with iron(III) chloride in THF revealed interesting mechanistic details about the self-assembling process leading to 5. At an iron/ligand ratio of 1:1.5 star-shaped tetranuclear [Fe[Fe(L1)2]3] (11) was isolated. However, at an iron/ligand ratio of 1:2, complex 11 was transformed into the ferric wheel 5. It was shown, that the interconversion of 5 and 11 is reversible. Based on the mechanistic studies, a procedure was developed which works for both the synthesis of homonuclear 11 and the star-shaped heteronuclear clusters [Cr[Fe(L1)2]3] (12) and [Al[Fe(L1)2]3] (13). The structures of all new compounds were determined unequivocally by single-crystal X-ray analyses.

Self-assembly and crystal structure of a novel octadecametallic square box composed of 52 single components.

Reaction of divalent hexacoordinate metal ions with either tetradentate N-(2-piridylmethyl)iminodiethanol (3; H2L4) or tridentate N-(2-chlorobenzyl)iminodiethanol (5; H2L5) resulted in the formation of two completely different products. Starting from nickel(II) acetate and tetradentate 3, linear trinuclear complex [[Ni(HL4)]2-Ni(OAc)2(HCO2)2] (4) was formed. However, when tridentate 5 was allowed to react with cobalt(II) acetate, the novel octadecametallic square box [Na2-(Co4(HL5)3(OAc)5]4(HCO2)2] (6) was isolated. Chiral 6 has D2-molecule symmetry and crystalizes with both enantiomers in the unit cell. The structures of all new compounds were determined unequivocally by single-crystal X-ray analyses.

Magnetic anisotropy of a cyclic octanuclear Fe(III) cluster and magneto-structural correlations in molecular ferric wheels.

The magnetic anisotropy of the cyclic octanuclear Fe(III) cluster [Cs subsetFe(8)[N(CH(2)CH(2)O)(3)](8)]Cl was investigated. Based on a spin Hamiltonian formalism and the consequent use of all symmetries, the magnetic anisotropy could be calculated exactly to first order, i.e., in the strong exchange limit. Experimentally, the magnetic anisotropy was investigated by magnetic susceptibility and high-field torque magnetometry of single crystals. The field and angle dependence of the torque at 1.7 K could be accurately reproduced by the calculations with one single parameter set, providing accurate results for the coupling constant and single-ion zero-field-splitting. These magnetic parameters are compared to those of several related hexanuclear ferric wheels and are discussed with respect to magneto-structural correlations for both coupling constant and single-ion anisotropy.

Copper(I) complexes, copper(I)/O(2) reactivity, and copper(II) complex adducts, with a series of tetradentate tripyridylalkylamine tripodal ligands.

Copper(I) and copper(II) complexes possessing a series of related ligands with pyridyl-containing donors have been investigated. The ligands are tris(2-pyridylmethyl)amine (tmpa), bis[(2-pyridyl)methyl]-2-(2-pyridyl)ethylamine (pmea), bis[2-(2-pyridyl)ethyl]-(2-pyridyl)methylamine (pmap), and tris[2-(2-pyridyl)ethyl]amine (tepa). The crystal structures of the protonated ligand H(tepa)ClO(4), the copper(I) complexes [Cu(pmea)]PF(6) (1b-PF(6)), [Cu(pmap)]PF(6) (1c-PF(6)), and copper(II) complexes [Cu(pmea)Cl]ClO(4).H(2)O (2b-ClO(4).H(2)O), [Cu(pmap)Cl]ClO(4).H(2)O (2c-ClO(4).H(2)O), [Cu(pmap)Cl]ClO(4) (2c-ClO(4)), and [Cu(pmea)F](2)(PF(6))(2) (3b-PF(6)) were determined. The oxidation of the copper(I) complexes with dioxygen was studied. [Cu(tmpa)(CH(3)CN)](+) (1a) reacts with dioxygen to form a dinuclear peroxo complex that is stable at low temperatures. In contrast, only a very labile peroxo complex was observed spectroscopically when 1b was reacted with dioxygen at low temperatures using stopped-flow kinetic techniques. No dioxygen adduct was detected spectroscopically during the oxidation of 1c, and 1d was found to be unreactive toward dioxygen. Reaction of dioxygen with 1a-PF(6), 1b-PF(6), and 1c-PF(6) at ambient temperatures leads to fluoride-bridged dinuclear copper(II) complexes as products. All copper(II) complexes were characterized by UV-vis, EPR, and electrochemical measurements. The results manifest the dramatic effects of ligand variations and particularly chelate ring size on structure and reactivity.

A new family of chelating diphosphines with a transition metal stereocenter in the backbone: novel applications of "chiral-at-rhenium" complexes in rhodium-catalyzed enantioselective alkene hydrogenations.

The title compounds are accessed by sequences starting with racemic and enantiomerically pure [(eta5-C5H5)Re(NO)(PPh3)(CH3)]. Reactions with chlorobenzene/HBF4, PPh2H, and tBuOK give the phosphido complex [(eta5-C5H5)Re(NO)(PPh3)(PPh2)] (3). Reactions with Ph3C+ BF4-, PPh2H, and tBuOK give the methylene homologue [(eta5-C5H5)Re(NO)(PPh3)(CH2PPh2)] (9). Treatment of 3 or 9 with nBuLi or tBuLi and then PPh3Cl gives the diphosphido systems [(eta5-C5H4PPh2)Re(NO)(PPh3)((CH2)nPPh2)] (n = 0/1, 5/11). Reactions of 5 and 11 with [Rh(NBD)Cl]2/AgPF6 (NBD = norbornadiene) give the rhenium/rhodium chelate complexes [(eta5-C5H4PPh2)Re(NO)(PPh3)((mu-CH2)nPPh2)Rh(NBD)]+ PF6- (n = 0/1, 6+/12+ PF6-; 30-32% overall from commercial Re2(CO)10). The crystal structures of 6+ PF6- and 12+ PF6- are compared to those of 3 and 9, and other rhodium complexes of chelating bis(diphenylphosphines). The chiral pockets defined by the PPh2 groups show unusual features. Four alkenes of the type (Z)-RCH=C(NHCOCH3)CO2R' are treated with H2 (1 atm) and (R)-6+ PF6- or (S)-12+ PF6- (0.5 mol%) in THF at room temperature. Protected amino acids are obtained in 70-98% yields and 93-82% ee [(R)-6- PF6-] or 72-60% ee [(S)-12+ PF6-]. Pressure and temperature effects are defined, and turnover numbers of > 1600 are realized.

Solution and solid-state structure of (1-methoxy-8-naphthyllithium. THF)(2)

The structure of (1-methoxy-8-naphthyllithium.THF)(2), (2.THF)(2), determined by single-crystal X-ray diffraction (crystal data: monoclinic, a = 8.1816 (5), b = 21.9649 (14), and c = 8.2345 (3) A, beta = 117.969 (3) degrees; V = 1306.97 (12) A(3); space group P2(1)/n; Z = 4) reveals a centrosymmetrical dimer with a twist angle of about 63 degrees between the naphthyl rings and the Li-C(ipso)-Li plane, representing an intermediate between perpendicular and planar tetracoordinated C(ipso). NMR studies at low temperature indicate that 2 is dimeric in THF-d(8) solution under these conditions.

Effectiveness of low doses (50 and 100 microg b.i.d) of beclomethasone dipropionate delivered as a CFC-free extrafine aerosol in adults with mild to moderate asthma. Study Group.

The objective of this study was to evaluate the efficacy and safety of low doses (50 and 100 microg b.i.d.) of hydrofluoroalkane-134a (HFA) beclomethasone dipropionate (BDP) extrafine aerosol in improving asthma control. Reformulation of BDP in a new chlorofluorocarbon (CFC)-free propellant (HFA) has produced an extrafine aerosol with increased delivery of the drug to the airways of the lung. The study population comprised 270 steroid-naive patients with mild to moderate asthma (mean baseline forced expiratory volume in 1 sec [FEV1] as a percentage of predicted normal of 65%-85%). This was a 6-week, blinded, placebo-controlled, multicenter study. Patients were randomized to receive 50 or 100 microg b.i.d. HFA-BDP or HFA-placebo. Treatment with either 50 or 100 microg b.i.d. HFA-BDP resulted in a significantly greater improvement compared with placebo in FEV1 (mean change from baseline as percentage of predicted normal of 6.7%, 8.6%, and 0.4%, respectively; p < or = 0.01 active treatment groups vs. placebo), with a significant trend toward increasing improvement with increasing doses (p < or = 0.0001). Treatment also resulted in significantly greater mean changes from baseline in morning peak expiratory flow compared with placebo (29.5, 33.8, and 5.0 L/min, respectively; p < or = 0.01 active treatment groups vs. placebo). All other pulmonary function and asthma symptom measures supported these data. The study treatments were well tolerated. These results show that low doses of HFA-BDP extrafine aerosol effectively improve asthma control in adult patients with mild to moderate asthma. However, it is important that inhaled corticosteroid therapy is still given at a dose high enough to control airway inflammation as well as asthma symptoms.

Dose ranging study of mometasone furoate (Nasonex) in seasonal allergic rhinitis.

BACKGROUND: Topical nasal corticosteroids are rapidly gaining acceptance as first-line therapy for seasonal allergic rhinitis, but there is a desire for effective corticosteroids with an improved safety profile over existing products. OBJECTIVE: A multicenter, double-blind dose ranging study was conducted to compare the activity and tolerance of four doses of mometasone furoate nasal spray (tradename Nasonex) and placebo in adult patients with seasonal allergic rhinitis. METHODS: Four hundred eighty patients with seasonal allergic rhinitis were enrolled and randomized to receive mometasone furoate nasal spray 50 micrograms (n = 96), 100 micrograms (n = 95), 200 micrograms (n = 98) or 800 micrograms (n = 95), or placebo vehicle (n = 95) once daily for 28 days. RESULTS: All of the doses of mometasone furoate nasal spray showed activity in reducing the severity of rhinitis. The 200-microgram dose reduced total nasal symptom scores and total symptom scores throughout the study (P < .05 versus placebo vehicle). The 50-microgram dose and the 100-microgram dose showed less consistent activity at early timepoints (days 3 and 7), while the 800 microgram dose did not provide significant additional benefits over the 200-microgram dose. All dose levels were well tolerated CONCLUSION: The results of this trial indicate that 200 micrograms once daily is the optimum dose of mometasone furoate nasal spray for the treatment of seasonal allergic rhinitis.

A double-blind, controlled trial to assess the safety and efficacy of azelastine nasal spray in seasonal allergic rhinitis.

BACKGROUND: Azelastine solution is a topically (nasal) administered antiallergy drug with a preclinical profile suggestive of efficacy in patients with allergic rhinitis. OBJECTIVES: The study was designed to compare the effectiveness and safety of two dosages of azelastine nasal spray (2 sprays per nostril once daily and twice daily) with that of placebo in the treatment of patients with symptomatic seasonal allergic rhinitis. METHODS: Two hundred fifty-one patients (12 years of age or older) were randomized to treatment in this 2-week, double-blind, parallel-group study. Primary efficacy variables were Major Symptom Complex (nose blows, sneezes, runny nose, itchy nose, watery eyes) and Total Symptoms Complex (Major Symptom Complex plus itchy eyes/ears/throat/palate, cough, postnasal drip). RESULTS: Patients treated with azelastine had mean percent improvements in Total and Major Symptom Complex scores that were consistently superior to placebo at each evaluation point. Overall, improvements were statistically significant (p < or = 0.05) in the Total Symptoms Complex for both azelastine groups and in the Major Symptom Complex for the twice daily group with a trend toward statistical significance for the once daily group. Azelastine was superior to placebo in improving all individual rhinitis symptoms. Adverse experiences in the azelastine groups were minor and infrequent. CONCLUSION: The results support the efficacy and safety of azelastine nasal spray in the treatment of seasonal allergic rhinitis.

Fluticasone propionate given once daily is as effective for seasonal allergic rhinitis as beclomethasone dipropionate given twice daily.

Fluticasone propionate was compared with beclomethasone dipropionate for the treatment of allergic rhinitis in a multicenter, double-blind, randomized, placebo-controlled study during the mountain cedar (Juniperus ashei) pollination season in central Texas. Adults (n = 313) with moderate to severe symptoms were treated with fluticasone propionate aqueous nasal spray 200 micrograms once a day or beclomethasone dipropionate aqueous nasal spray 168 micrograms twice a day or placebo for 2 weeks. Fluticasone propionate administered once daily and beclomethasone dipropionate administered twice daily were equally effective as assessed by clinician- and patient-rated scores for nasal obstruction, rhinorrhea, sneezing, and nasal itching throughout the treatment and follow-up periods. Both regimens were more effective than placebo. Adverse events were related to topical administration and were similar in frequency and nature in all three treatment groups. Fluticasone propionate and beclomethasone dipropionate displayed a similar safety profile that did not differ from placebo. We conclude that fluticasone propionate aqueous nasal spray administered as 200 micrograms once daily in the morning is as safe and effective as beclomethasone dipropionate aqueous nasal spray administered as 168 micrograms twice daily for seasonal allergic rhinitis.
