The role of danazol in the management of precocious puberty.

The management, investigation and treatment of 12 children with precocious puberty is described. Danazol, a synthetic derivative of ethisterone, has been used over a period of 3 1/2 years. Its clinical, endocrine and biochemical effects have been evaluated. Danazol ha a very useful clinical effect in controlling the development of secondary sexual features but does not modify the rapid linear growth and skeletal maturity seen in this condition.

Repeat lumbar puncture in the diagnosis of meningitis.

Meningitis may be difficult to diagnose. If it is suspected clinically and a first lumbar puncture is normal, a second cerebrospinal fluid (CSF) sample may be required within a few hours to confirm the diagnosis. A child is presented in whom CSF was normal 14 hours after the onset of illness, but who 14 hours later showed the characteristic changes of purulent meningitis. This case differs from those previously cited in the literature, in that the blood cultures taken at the time of the first lumbar puncture were also negative.

Surgical repair of atrial septal defect in a dog.

Atrial septal defect, a relatively rare canine congenital cardiovascular defect, caused syncope during excitement in a 5-year-old male Boxer dog. The defect was successfully repaired by open heart surgery.

Congenital anomalies of the diaphragm. Personal experience with thirty-five consecutive cases.

A personal experience of thirty-five consecutive patients with congenital anomalies of the diaphragm is reported. The anomalies included twenty-five Bochdalek hernias, seven diaphragmatic eventrations, and three esophageal hiatal hernias. Except for the patients with esophageal hiatal hernias, virtually all patients presented with respiratory embarrassment, twenty-seven of the thirty-five within 24 hours of birth. An especially high mortality is associated with this early onset of symptoms. The treatment was surgical with a thoracic approach used for right Bochdalek defects and eventrations, and an abdominal approach used for left Bochdalek hernias. The importance of proper preoperative and postoperative management in a well equipped neonatal intensive care unit is emphasized.

Watery diarrhoea and ganglioneuroma with secretion of vasoactive intestinal peptide.

A young girl presenting with diarrhoea and stridor was found to have a thoracic ganglioneuroma. The tumour, which was partially resected, contained an abnormally high amount of vasoactive intestinal peptide (VIP) as measured by radioimmunoassay. This decreased after the operation. In addition, analysis of urine showed the presence of abnormal amounts of catecholamines and catechol metabolites. The findings suggest that the diarrhoea in the syndrome of ganglioneuroma and bowel dysfunction is due to an excess of VIP secreted by the tumour.
