P-domains as shuffled cysteine-rich modules in integumentary mucin C.1 (FIM-C.1) from Xenopus laevis. Polydispersity and genetic polymorphism.

A new frog integumentary mucin (FIM-C.1) has been discovered by molecular cloning. This mucin contains at least six typical P-domains as cysteine-rich modules. Shuffled P-domains have previously been detected in FIM-A.1, and they also form the basis of various P-domain peptides, which presumably have growth-modulating activities. Furthermore, FIM-C.1 contains at least three threonine-rich clusters, which consist of semi-repetitive cassettes. Various polydisperse transcripts have been characterized. They originate from two genes only and differ by deletions/insertions that are congruent with the semi-repetitive cassettes. Thus, polydispersities are probably generated by alternative splicing. Southern analysis revealed genetic polymorphism between different individuals. Furthermore, a specific antiserum was generated against a synthetic peptide deduced from the COOH-terminal end of FIM-C.1 and used for Western analysis.

xP2, a new member of the P-domain peptide family of potential growth factors, is synthesized in Xenopus laevis skin.

Similarly to epidermal growth factor (EGF) and EGF-like repeats, the "P-domain" represents a cysteine-rich module that has been detected in the past in a variety of polypeptides, as well as in high molecular weight proteins. Here, a precursor for a secretory polypeptide (xP2) is characterized that consists of two P-domains. xP2 has been discovered in Xenopus laevis with the help of the polymerase chain reaction. In contrast to all other P-domain peptides, it is synthesized in the skin but not in the stomach or the pancreas. By this and other criteria, it cannot be considered simply as the X. laevis homologue of recently described P-domain peptides, viz. the spasmolytic polypeptides (PSP/hSP/mSP). Furthermore, a polyclonal antiserum was generated against the deduced C-terminal end of xP2. Due to its immunoreactivity with granular glands, as well as with the epidermis, the possibility of a growth factor activity for xP2 in the germinal layer is discussed.

The polymorphic integumentary mucin B.1 from Xenopus laevis contains the short consensus repeat.

The frog integumentary mucin B.1 (FIM-B.1), discovered by molecular cloning, contains a cysteine-rich C-terminal domain which is homologous with von Willebrand factor. With the help of the polymerase chain reaction, we now characterize a contiguous region 5' to the von Willebrand factor domain containing the short consensus repeat typical of many proteins from the complement system. Multiple transcripts have been cloned, which originate from a single animal and differ by a variable number of tandem repeats (rep-33 sequences). These different transcripts probably originate solely from two genes and are generated presumably by alternative splicing of an huge array of functional cassettes. This model is supported by analysis of genomic FIM-B.1 sequences from Xenopus laevis. Here, rep-33 sequences are arranged in an interrupted array of individual units. Additionally, results of Southern analysis revealed genetic polymorphism between different animals which is predicted to be within the tandem repeats. A first investigation of the predicted mucins with the help of a specific antibody against a synthetic peptide determined the molecular mass of FIM-B.1 to greater than 200 kDa. Here again, genetic polymorphism between different animals is detected.

xP1 and xP4. P-domain peptides expressed in Xenopus laevis stomach mucosa.

Two different precursors for secretory polypeptides from the stomach of Xenopus laevis have been characterized by cDNA cloning. Both mature polypeptides are potential candidates for gastrointestinal growth factors. One, xP1, is the X. laevis homologue of the pS2 gene product consisting only of a single P-domain, whereas the second, xP4, is a novel polypeptide formed by four P-domains arranged in tandem. Northern analysis detected both transcripts in the stomach but not in the skin or the brain. In situ hybridizations localized the expression of both precursors in surface mucous cells of the gastric mucosa. With an antibody generated against the deduced C-terminal end of xP4, the mature polypeptide was investigated by Western analysis revealing N-glycosylation of xP4.

[Analysis of mortality time series in malignant lung tumors and its future trends].

Time series of standardized mortality from malignant neoplasms of the trachea, bronchi and lungs in 1965 to 1986 were examined by a group of programmes for computer supported prognoses. Czechoslovak data were compared with those from Austria, Sweden and England and Wales. Extrapolation of time series revealed conclusively that the development in Czechoslovak men is alarming. While in all compared countries there was in recent years a declining mortality, in Czechoslovakia the mortality is rising despite the fact that it is already highest among the compared countries. In women the position is less adverse, as due to the substantially lower absolute values in all compared countries, the values of standardized mortality in Sweden and England rise in an exponential manner, while in Czechoslovakia the increase is linear. From the prognosis ensues thus quite unequivocally that the development in Czechoslovakia in men is absolutely and relatively very unfavourable and in women relatively favourable.

Prognosis of manpower requirements to ensure specialist diabetological care in the Czech Republic.

The submitted prognosis of need of personnel to ensure specialist diabetological care in the Czech Republic is based on an analysis of the development of the number of diabetic patients and on analysis of activities which must be provided to diabetics in order to maintain the selected standard of health care. The estimate of the future number of diabetics is assessed by the method of demographic projection for one-year age groups, separately for men and women. The analysis of activities in diabetological care was elaborated from the results of an anonymous survey among diabetologists in the South Moravian region. Reflections on different variants of the standard of provided care respect also the analysis of the hitherto recorded trend in the number of health workers in diabetology and their available time. The estimate of needs is objective and can be applied in specialized diabetological services as well as in a different organization of care of diabetics, e.g. by the general practitioner. The contemporary organization of care of diabetics cannot ensure the required standard of care. The prognosis submits several possible ways how to achieve this standard. The prognosis deals in particular with the number of working hours of doctors but does not omit other categories of health workers.

[Prognosis for requirements for oral antidiabetic agents in Bohemia].

The authors elaborated the prognosis of requirements of oral antidiabetics (PAD) in the CSR, based on the estimate of the number of diabetics, using the method of demographic projection and the estimate of the number of diabetics treated by this therapy. The respective numbers of diabetics are circulated for five-year age-groups, separately for men and women. The need of PAD is derived from the mean daily dose per diabetic patient treated with PAD, separately for biguanides and derivatives of sulphonyl urea. The prognosis respects the basic factors which influence the need of PAD, i.e. the development of the diabetic population, incl. the evolution of its structure, and the development of the attitude to PAD therapy. The application of various assumptions on the future development leads various to variants of the prognosis. Comparison with the actual development is important for evaluation of applied therapeutic methods and for planning purposes. The results of the prognosis indicate that in 1995 the need of PAD in the CSR will vary between 140 and 190% of the consumption in 1980, however, as a result of an uneven development the annual increment could be as high as 19% or there could be a slight drop.

Expression of spasmolysin (FIM-A.1): an integumentary mucin from Xenopus laevis.

In the past, a unique type of precursor for a secretory protein was discovered. It contains a central repetitive domain rich in threonine residues and terminal cysteine-rich domains. Due to striking homologies of these terminal domains with pancreatic spasmolytic polypeptide, originally the name "prepro-spasmolysin" was proposed. Here we show that the mature protein has a MW of about 130 kDa, consisting of about 70% carbohydrate and 30% protein. Similar O-linked glycoproteins have been found in mucins from human intestine. For this and numerous other reasons we decided to rename this glycoprotein "frog integumentary mucin A.1" (FIM-A.1). Furthermore, analysis of the protein with specific antibodies against the predicted C-terminal end indicates that FIM-A.1 is probably not processed at pairs of basic residues. In situ hybridization as well as immunofluorescence studies revealed that FIM-A.1 is expressed and stored exclusively in mature mucous glands of Xenopus laevis skin. Only cone cells at the proximal part of these glands do not synthesize FIM-A.1. In contrast, all other physiologically active peptides from X. laevis skin investigated so far are synthesized in granular glands. A hypothetical function of FIMs for defense against microbial infections is discussed.

[Experiences with a combination therapy of methylprednisolone and troleandomycin in severe bronchial asthma requiring high-dose corticoids].

Although treatment comprising a combination of methylprednisolone (MP) and troleandomycin (TAO) has been employed to treat cases of severe bronchial asthma requiring high doses of corticosteroids, for about 20 years, now, it has always been associated with major adverse reactions (1). A new protocol avoids these adverse effects by a rapid reduction in the dose of MP to alternating administrations and low TAO dosage (250 mg). Nineteen patients were treated using this protocol, 16 of whom for more than two months (maximum 21, minimum 4 months). Three patients were taken out of the study as early non-responders in the initial phase. Five patients must be considered to be late non-responders, or were discharged from the study on account of pathological liver parameters. In 8 patients who showed good tolerance, an appreciable reduction in the dose of steroids, with stabilisation of the asthma, was observed.

[Prognosis for insulin requirements in Czechoslovakia].

The submitted prognosis of insulin requirements in the CSR is based on the estimated number of type 1 diabetics and the estimated number of insulin treated type 2 diabetics. For the purpose of the prognosis these numbers are calculated for five-year age groups, separately for men and women. In the first approximation the authors assume that in the treatment of type 1 diabetics four typical therapeutic regimes are used which are consistent with conventional therapy and its intensified modification. In type 2 diabetics in the first approximation it is assumed that another four variants of therapeutic regimes will be mostly used which are typical for these patients. The sum of daily doses needed by all patients gives an estimate of the total requirements of the four basic types of insulin. This prognosis respects the basic factors which influence insulin requirements, whereby the basis is the development of the health status of the population. Comparison of different variants of the prognosis of requirements with the actual consumption makes it possible to formulate some conclusions for planning production and for evaluating the used and perspectively foreseen insulin therapy. From the results of the prognosis ensues that in 1995 the insulin consumption in the CSR will vary between 150 and 185% of the consumption in 1980.

Simulation of CFU-s kinetics after irradiation.

A simulation model of the CFU-s population is used to interpret data from experimental studies of bone marrow recovery after irradiation. The model includes an original hypothesis that the proliferation rate in the CFU-s population depends on the number of DNA-synthesizing CFU-s. It is assumed that the DNA-synthesizing CFU-s produce a factor in the presence of which CFU-s enter the resting state G0 after mitosis and remain there for prolonged periods of time. The model can adequately reproduce complex CFU-s kinetics observed after severe damage caused by irradiation with a unique set of parameters.

[Right ventricular hemodynamics and lung function following amrinone injection].

UNLABELLED: Amrinone is a positive inotropic agent which has been widely used for treatment of left heart failure. As this drug has a relaxing effect on smooth muscle cells, we considered that hemodynamics and ventilatory mechanics could be simultaneously improved in patients with right heart failure caused by obstructive lung disease. Swan-Ganz catheters were used in 11 patients. Following injection of 1.5 mg/kg Amrinone, cardiac output increased from 6.1 to 6.9 l/min (p less than 0.05). Stroke volume showed only a slight increase; pulmonary vascular resistance was reduced from 407 to 308 dynes . s . cm-5 (p less than 0.01). Body plethysmographic examination revealed a decrease of specific airway resistance from 33 to 24 cm H2O . s (p less than 0.01). CONCLUSION: In patients with right heart failure due to obstructive lung disease Amrinone application can result in an improvement in both hemodynamics and ventilatory mechanics.

Analysis of the effect of hydroxyurea on stem cell (CFU-s) kinetics.

Hydroxyurea induces profound changes in the pluripotential haemopoietic stem cell (CFU-s) kinetics. The main feature of these changes is a synchronous entry of resting G0 CFU-s into the cell cycle. The analysis of the passage of the CFU-s cohort through the cell cycle has been largely based on the examination of the fraction of CFU-s which synthesize DNA in the S phase of the cell cycle. This analysis has, however, been hampered by the fact that both the sensitivity of the S phase CFU-s to hydroxyurea and their sensitivity in the [3H]thymidine suicide technique vary as the cells pass through the S phase. Methods which overcome these difficulties have been used in the experiments presented in this paper. It was demonstrated that hydroxyurea kills only about 80% of the S phase CFU-s. The sensitivity to hydroxyurea gradually decreases as the cells approach the middle part of the S phase and increases again as the cells enter the late portions of the S phase. The degree of CFU-s synchrony at the point of entry into and exit from, the S phase has been established. Mathematical analysis of the available data suggests that CFU-s pass through the S phase with a mean transit time of 4.79 hr (standard deviation, 1.45 hr). Hydroxyurea, administered in vivo, blocks CFU-s in the late G1 phase. The duration of this G1-S block, induced by a dose of 1000 mg of hydroxyurea per kg body weight, is approximately 2 hr. The CFU-s in the middle of the S phase, which survive hydroxyurea administration, are also blocked in their passage through the S phase. These cells, however, seem to finish the S phase with a delay of approximately 2 hr.
