The riddle of femininity: the interplay of primary femininity and the castration complex in analytic listening.

This study elucidates the application of an analytic attitude to questions of gender and sexuality. The paper reports on a study group's exploration of the relative heuristic use of two important organising concepts in analytic work with female analysands: primary feminity and the phallic castration complex. A tendency to cling to one position over the other skews analytic listening. Two cases are presented of women struggling to consolidate positive feminine identifications and, to that end, working through conflicting feminine identifications and defences against a resolution of the awareness of gender differences. Analytic listening requires a view of each psychic construction as a layer to be understood in its own right yet as a cloak soon to reveal the next layer--a different construction. The study includes observations on perverse fantasies in women.

Radiation keratosis associated with exposure to a gold ring.

A radiation keratosis occurred in a woman who wore a radioactive gold ring forty-three years ago. Clinicians should be aware that not all "warty lesions" on the hands are actinic keratosis, seborrheic keratosis, or warts. Radioactive gold rings still exist. Diagnosis requires a high index of suspicion, since patients may no longer be wearing the offending ring.

[H2O2 low temperature plasma sterilization. New possibilities for use with eye surgery instruments].

The H2O2-low-temperature-plasma-sterilization (STERRAD 100) works with a temperature below 50 degrees C (140 degrees F). This system is appliable for thermostabile materials as well as for thermolabile materials. The efficancy of this new system is shown by a biological test with Bacillus pumilus spores. 5 typical ophthalmic surgical instruments were contaminated. After sterilization the numerical reduction of the microorganisms had to be at least 6 log levels. Corrosion caused by hydrogene peroxide was excluded after exposing steal with a high quantity of this substrate. Electromicroscopy analysations of the surfaces of stainless steal after LTP, steam sterilization and hot-air sterilization are compared. Options and limitations of this new sterilization technique are discussed. A newly developed operating system with a complete instrumental box (OP-Set) will be introduced.

Palisading cutaneous fibrous histiocytoma. An immunohistochemical study demonstrating differentiation from dermal dendrocytes.

The palisading cutaneous fibrous histiocytoma is a unique benign spindle cell tumor that often occurs in an acral location. Histologic findings feature nuclear palisading reminiscent of a schwannoma. Previous studies have suggested that this tumor may be a variant of a dermatofibroma. We performed immunohistochemical staining with a panel of antibodies including neurofilament, S100 protein, factor XIIIa, epithelial membrane antigen, Leu-7, desmin, and vimentin. We found positive staining with factor XIIIa and vimentin, suggesting connective tissue differentiation for this tumor.

IgA pemphigus foliaceus. Report of two cases and a review of the literature.

The cases of two patients with vesiculobullous lesions were diagnosed clinically and histopathologically as pemphigus foliaceus; unexpectedly, both revealed intercellular IgA, but not IgG, in the upper epidermis by direct immunofluorescence. Such histologic and immunofluorescence findings have been reported in eight other cases. In our cases no circulating IgA or IgG intercellular antibodies could be detected; in four of eight other reported cases IgA antibodies showed intercellular staining like that of pemphigus antibodies. Subcorneal acantholytic lesions occurred in both our cases; of the other cases reported, five had essentially identical histopathologic findings. The clinical and histopathologic features of pemphigus, as well as the recent findings of circulating IgA intercellular antibodies alone or with IgG antibodies, appear to place this disease into the spectrum of pemphigus. The 10 IgA pemphigus cases reported to date fall into one of two groups, the IgA pemphigus foliaceus (including our two cases) and IgA pemphigus of the intraepidermal neutrophilic type, which seems to be less common.

[Suitability of thermal selection for the primary culture of Pseudomonas aeruginosa].

It could be shown by quantitative methods that growing P. aeruginosa suspensions at 42 degrees C without any loss is possible only in a medium without any inhibitory additives--when suspensions of unimpaired organisms are used. The addition of inhibitors e.g. malachite green without influence to the growth rate of P. aeruginosa at 37 degrees C leads to a marked reduction of growth at temperatures only slightly higher e.g. 38 degrees C. This can also be observed when poor media like Drake's medium 10 are used. The method of thermic selection was the most inappropriate for the culture of P. aeruginosa organisms impaired by chlorine. Germs which showed good growth at 37 degrees C could be grown only at a very reduced rate at 42 degrees C even though a rich medium free of inhibitory additives was used. Thermic selection is not suited to the cultivation of P. aeruginosa from samples of water which as a rule is chlorinated e.g. swimming pool water.

Iatrogenic myelomonocytic leukemia following melphalan treatment of scleromyxedema.

We report the case of a patient with scleromyxedema with abnormal monoclonal gamma globulin of the kappa type who was treated with melphalan (Alkeran), an alkylating agent. After nine and one-half years of therapy, the patient showed myelomonocytic leukemia. The risk of secondary malignancies must be considered when weighting the benefits and risks of melphalan treatment.

Anti-endomysial antibodies. A serologic marker of dermatitis herpetiformis.

Direct immunofluorescence (IF) studies of skin biopsies are of value in the diagnosis of most, but not all, cases of dermatitis herpetiformis (DH). Similarly, histologic studies are of help but may be questionable or completely nonspecific. Serologic studies for the presence of IgA-class anti-endomysial antibodies are very specific and are found in 70% of patients with DH and in all untreated patients with celiac disease. The titers of these antibodies are directly associated with the degree of gut disease in these patients. Thus, the presence of these antibodies even in the absence of classic direct IF and histologic findings are diagnostically important. We encountered three cases in which both direct IF and histologic studies were equivocal toward confirming the clinical diagnosis of DH. Serologic studies for the presence of IgA-class anti-endomysial antibodies provided evidence for the diagnosis of DH, and, in each case, results were confirmed by further direct IF studies. Since these antibodies are disease specific for DH and celiac disease and are found in most active cases of DH, they may be considered an adjunct to the direct IF and histologic studies of the skin.

Cicatricial pemphigoid. A case of onset at age 5.

Cicatricial pemphigoid is a chronic subepidermal bullous dermatosis which primarily involves the mucous membranes. It is a disease found almost exclusively middle-aged and elderly persons. This report describes a case of cicatricial pemphigoid with onset at age 5 and involving primarily the mucous membranes of the mouth and eyes. The patient cleared well on 40 mg of Prednisone and has been maintained on a regimen of 7.5 mg of Prednisone every other day.

[Culturing of chlorine-damaged E. coli bacteria using the membrane filteration technic].

The number of chlorine-impaired E. coli organisms obtainable with the aid of the membrane-filtration-Endo-agar-method is so reduced that this method cannot be considered equivalent to liquid enrichment. Notwithstanding the well documented difficulties that arise from differences between batches of the same filters e. g. from problems to assure their standardization, the main obstacles do not stem from the questionable suitability of membrane filters but from the toxicity of the Endo-agar. The use of lactose-pepton (L.P.)-broth (German Standard Methods 1971) equally does not lead to satisfactory results if compared to the use of casein-soya (C.S.)-broth.

[An improved lactose-peptone medium for the cultivation of chlorine-damaged E. coli bacteria].

A description of a casein-soya-lactose broth (CSLB) for the cultivation of chlorine-impaired E. coli bacteria is given. In this liquid medium the recovery rates of chlorine impaired E. coli are superior or at least equal to recovery rates observed when casein-soya-broth (CSB) is used. Differences are regularly seen when lactose-pepton-broth (LPB) according the German Standards (DEV.K6) is used between direct inoculation into a liquid enrichment medium and inoculation into the same medium following membrane filtration are no longer found when CSSL-broth is used instead of LPB.

[Preparation and properties of chlorination-damaged P. aeruginosa suspensions].

The preparation of suspensions of bacterial cells impaired by the action of chlorine require a very accurate standardization of all the procedural detail of controlled experimental impairment by chlorine. All measures suitable to spread the kinetics of reduction of bacterial numbers in time will facilitate the reproducibility of results. The exact amount of hypochlorite--determined experimentally prior to the actual test--is transformed to chloramine at 0 degrees C then a defined quantity of bacteria is added. After different periods of contact samples are taken and stabilized by adding thiosulphate of sodium. When the bactericidal action is not abrupt but sufficiently drawn out a relatively high proportion of bacteria impaired by chlorine but still viable is obtained (50-90%). In suspension the proportion remains relatively stable during storage in the refrigerator (4 degrees -6 degrees C). Resuscitation experiments carried out with suspensions of organisms damaged as explained above showed resuscitation times of 3 h in casein soy broth not to grow as well as those after 24 h of resuscitation. Obviously the process of reparation after impairment by chlorine must be assumed to be relatively slow under these conditions.

Cutaneous signs of internal malignancies.

The more common paraneoplastic skin changes associated with internal malignancies (acanthosis nigricans, dermatomyositis, secondary ichthyosis) are discussed. The rare glucagonoma syndrome, consisting of alpha cell tumor of the pancreas, migratory necrolytic erythema, and diabetes mellitus, is described. The importance of recognition of these skin lesions may lead to early detection of the underlying cancer with possible cure.
