Ultrastructure of the kidney of the marine teleost Sparus auratus: the renal corpuscle and the tubular nephron.

The ultrastructure of the renal corpuscle and tubule of Sparus auratus is described. The parietal epithelium in Bowman's capsule is flattened with occasional cilia; podocytes are large with bundles of perinuclear microfilaments, a large vacuole and occasional cilia; a filtration slit membrane can sometimes be identified; mesangial cells are placed peripherally and among the walls of the capillaries. The neck segment is short and ciliated; it lacks the mucous cells which appear in some teleosts. The first proximal segment has columnar cells with a well developed brush border, and some cilia, large light vacuoles and many lysosomes appear in the apical zone; the second proximal segment has taller cells than the former, which appear with a less dense brush border, containing numerous multivesicular bodies; the third proximal segment, which has cells similar to the previous ones, possesses a less developed brush border and numerous mitochondria scattered all over the cytoplasm. No distal tubule is present. There is a collecting tubule with columnar cells with few microvilli and some apical mucin granules which empty into the collecting duct.

Cowden's disease: familial goiter and skin hamartomas. A report of three cases.

Multiple hamartoma syndrome (Cowden's disease) consists of characteristic skin lesions of the face, mucous membranes and distal extremities in association with a variety of benign and malignant internal tumors, especially of the thyroid and breast. We describe a family in which the father, daughter and son were found to have goiter associated with the skin lesions of Cowden's disease. Review of the 40 reported cases of this syndrome indicates that thyroid disease occurs in two thirds of patients with Cowden's disease and most often presents as goiter at an early age. Thyroid cancer has occurred in only three (7.5%) of the patients.Surgical removal of the large goiter of the son showed that it was composed of multiple encapsulated follicular adenomas and a few areas of lymphocytic thyroiditis. Studies of the thyroid tissue showed that peroxidase activity was decreased, the thyroglobulin had a reduced content of thyroxine and triiodothyronine (perhaps due to the therapeutic suppression of thyroid-stimulating hormone) and thyroxine 5'-monodeiodinase was greatly increased; increased outer ring monodeiodinase activity may be a characteristic of human follicular adenomas.

The pulmonary artery segment in normal and in valvular pulmonary stenosis.

1. The length and arrow or the PAS of 130 normal cases were measured, respectively. The BSA (m2) ranged between 0.30--2.05, and the ages between 6 months and 40 years. 2. Seventy-three cases VPS were divided into four groups according to the peak SPRV: Group 1, less than 50 mm Hg; Group 2, from 50--75 mm Hg; Group 3, from 75--150 mm Hg; and Group 4, with more than 150 mm Hg. The length and arrow of the PAS was measured. The age of the patient ranged from 4--38 years. 3. In normal cases, the PAS was a straight line or presented a slight concavity in 30% of the cases. 4. In cases of VPS all of them had a slight or marked bulging of the PAS, except one in Group 1 (6.6%); two in Group 2 (11%); three in Group 3 (10%); and one in Group 4 (8.3%). Marked convexity (more than 10 mm of arrow of this segment) was found one time in Group 1 (6.6%); three times in Group 2 (16%); three times in Group 3 (10%); and twice in Group 4 (16%). 5. In normal cases the length and the arrow of the PAS tend to have greater values when the BSA is higher. When the arrow of the segment is above 5 mm and the length higher than 50 mm, irrespective to the patient's age, we must be facing an abnormal finding and all those diseases capable of giving an abnormal PAS should be considered. 6. We want to stress the importance of obtaining a proper roentgenogram to assess, with accuracy, the quantitative and qualitative data needed to do a correct interpretation of the cardiac silhouette.

Acute phase reactants and clinical stages in multiple myeloma.

Serum levels of seven specific proteins mostly acute phase reactants (APR) have been studied (transferrin, alpha 2-macroglobulin, alpha 1-antitrypsin, haptoglobin, C3, ceruloplasmin, orosomucoid) in 14 healthy subjects and in 55 patients with multiple myeloma. The alpha 2-macroglobulin and transferrin are significantly decreased in the myeloma group compared with healthy controls whereas the remaining proteins under study are elevated, significantly only orosomucoid and ceruloplasmin. The APR in the IgG3 myelomas seem to show differences from those of the IgG1 and IgG2 subclasses the levels of the negatively changed proteins being lower, whereas the positive APR higher in the IgG3 myeloma. In the IgA myeloma, however, decreased levels of haptoglobin and alpha 1-antitrypsin have been found in spite of being positive APR. Transferrin and haptoglobin serum level can be included as a new parameter in regression equations for calculation of IgA myeloma cell mass.

Partial reconstitution of active ribosomes and 50S subunits.

Escherichia coli ribosomes and their 50S subunits disassembled by LiCl treatment can be reconstituted into structurally completed but inactive particles. However, peptidyltransferase and polyphenylalanine synthesizing activity can be partly recovered by the addition of methanol to the reconstitution system. Furthermore, entirely active ribosomes and 50S subunits are reconstituted when methanol is present during the initial treatment with LiCl to disassemble the ribosomal components. The presence of methanol (10% v/v) during this treatment diminished the release of some proteins but does not affect the separation of the 5S RNA.

Postoperative hypocaloric parenteral nutrition. A study in patients without neoplasm.

Hypocaloric peripheral parenteral nutrition during the postoperative period aims to reduce or minimize protein loss and avoid use of central venous catheters. The efficacy of such nutrition in regard to postoperative outcome and nitrogen balance was evaluated in comparison with standard fluid therapy. The study was performed on 49 well-nourished patients who underwent major surgery for non-neoplastic conditions. Group I received 1 g amino acids + 1 g xylitol and 1 g sorbitol/kg body weight, while standard fluid therapy was given in group II. No significant intergroup difference was found in serum levels of albumin, total protein, prealbumin, transferrin or retinol-binding protein. The blood urea, nitrogen excretion and nitrogen balance were significantly higher in group I. The postoperative outcome was similar in both groups, as were the observed complications. Thus although the nitrogen balance was superior in group I, no intergroup difference was clinically evident.
