Blood transfusion and survival following surgery for renal carcinoma.

The effect of peri-operative blood transfusion on survival after surgery for renal carcinoma was studied in 201 patients. In addition to blood transfusion, several other factors were included in a multivariate analysis. Using Cox's proportional hazards model, transfusion of more than 4 units of blood was found to be an independent prognostic factor in addition to tumour stage, erythrocyte sedimentation rate and macrohaematuria.

Indomethacin in the treatment of ureteral colic: is fluid restriction necessary?

The effect of fluid load on pain was estimated in 60 patients with acute ureteral colic treated with 50 mg. intravenous indomethacin. One group received 3 l. fluid intravenously, while in another group all fluids were withheld for 6 hours. Pain was assessed with a visual analogue scale. No intergroup difference in regard to pain was found after 1, 3 and 6 hours of observation.

Acute ureteral colic and fluid intake.

In the treatment of acute ureteral colic opinions differ regarding the consequences of fluid intake. This study was undertaken to evaluate a possible relationship between fluid load and the pain. One group of patients received 3 liters of fluid intravenously, while in another group all fluids were withheld. No intergroup differences as regards pain was found after 6 hours of observation.

Distribution of the renal cortical blood flow in man.

The distribution of the radioactivity following an interaarterial injection of (-131-labeled macroalbumin (MA) was observed in 7 human kidneys and compared with the renal distribution of MA and microspheres in the dog. In the renal cortex the radioactivity per gram of dry tissue in the juxtamedullar and middle cortex was 20.5 and 44.2 per cent of that observed in the outer cortex. The relative radioactivity in the juxtamedullary cortex was significantly (P is less than 0.01) greater in the human than in the dog kidney. In the renal columns the radioactivity was similar to that observed in the middle and outer cortex with incresing values towards the renal hilus. Only traces of radioactivity were observed in the renal medulla. The findings indicate that the distribution of the renal cortical blood flow is similar in the human and the dog kidney with the exception of a greater relative blood flow in the juxtamedullary part of the human kidney.
