[Rupture of a coronary artery aneurysm in mucocutaneous lymph node syndrome (Kawasaki syndrome). Demonstration of the coronary aneurysms by 2-dimensional echocardiography].

A further case of mucocutaneous lymphnode syndrome (MCLS) in infancy with multiple aneurysms both of the coronary arteries and systemic arteries is presented. Aged four months the male infant died of rupture of an aneurysm of the right coronary artery causing a hemopercardium. The aneurysms both of the coronary and systemic arteries were diagnosed intra vitam by 2-dimensional echocardiography.

[Vitamin K deficiency hemorrhages in 4 exclusively breast-fed infants 4 to 6 weeks of age].

Haemorrhages were observed in four wholly breastfed infants beyond the neonatal period. These infants were observed within a period of 8 weeks and showed the following characteristics: 1. Onset of bleedings was unexpected and without prior indication. 2. They were of a serious nature and involved the CNS in two children. 3. In all cases infants between 4 and 6 weeks of life were affected. 4. All infants had been wholly breastfed. 5. All were male. 6. There was a prompt improvement after administration of vitamin K or after blood or blood derivatives. Although preliminary own investigations do not indicate general lowering of vitamin-K-dependent coagulation factors in wholly breastfed infants in the postneonatal period, these 4 cases observed within a short time confirm the necessity to consider vitamin K deficiency in haemorrhages in infants in the postneonatal period. Diagnostic steps have to be initiated immediately.

Concentrations of prostaglandin E2 and F2 alpha in the cardiovascular system of infants with persisting patent ductus arteriosus.

The distribution of prostaglandin E2 and F2 alpha was examined in the peripheral veins and in several positions of the cardiovascular system before and after the blood had passed through the lungs in 37 infants. Prostaglandin E2 varied from 0.25 +/- 0.09 ng/ml to 0.44 +/- 0.09 ng/ml when measured in the pulmonary artery, the ductus arteriosus, the right atrium, the right ventricle, the left atrium, the left ventricle, the inferior vena cava and the descending aorta. Prostaglandin F2 alpha was much higher in these positions of the cardiovascular system. The range was 0.99 +/- 0.36 ng/ml to greater than 2.0 ng/ml. The vascular tissues produced virtually identical high amounts of prostaglandin E2 and F2 alpha, but there were no significant differences in prostaglandin E2 and F2 alpha, concentrations, in venous blood as well as in systemic arterial blood. The results suggest that prostaglandin E2 is not responsible for the persisting patency of the ductus arteriosus in infants. There is no explanation for the increased prostaglandin F2 alpha concentrations in these patients.

[Diverticulum of the brachiocephalic trunk--angiographic image and embryologic significance].

A diverticulum of the brachiocephalic trunk is described in 5 children with unilateral absence of the pulmonary artery, in a child with tetralogy of Fallot, and in an other child with Bland-White-Garland syndrome. The diverticulum is a remnant of a contralateral ductus arteriosus, which cLosed after birth and represents the origin of the distal part of the sixth aortic arch.

Isolated atrioventricular discordance. Report of two surgical cases with isolated ventricular inversion.

Isolated ventricular inversion is a term used for a congenital heart malformation with the segmental arrangement atrioventricular (AV) discordance and ventriculoarterial concordance. It describes a condition which from a physiological point of view resembles complete transposition of the great arteries. We have recently seen two patients with this anomaly. Both underwent intracardiac repair by means of a Mustard operation at 10 years and 10 months of age, respectively. Associated lesions in the first patient were an AV septal defect with two AV orifices (partial AV canal, ostium primum defect), partial anomalous pulmonary venous return, and anomalies in the systemic venous drainage. A perimembranous ventricular septal defect complicated the condition in the second patient.

Absent pulmonary artery. A hint to its embryogenesis.

Cardiac catheterization was performed in nine patients with unilateral absence of a pulmonary artery. Aortography revealed a diverticulum of the innominate artery in five of six patients in whom the aortic arch and absent pulmonary artery were on opposite sides. It is suggested that the diverticulum indicates fetal systemic blood supply to the affected lung through the distal part of the sixth aortic arch, which--consisting of ductal tissue--obliterated after birth, leaving the diverticulum where the ductus joins the innominate artery.

[Cardiotoxic effect of the tocolytic agent fenoterol (partusisten) in the neonate (author's transl)].

Forty-two children of mothers treated with beta-adrenergic drugs were investigated cardiologically during a period of 3 1/2 years. Eleven neonates were seen to suffer from severe toxic myocardiopathy associated with heart failure. The mothers had in part been given excessive fenoterol doses for 13 weeks on the average. One child died of regressive myocardial alterations as confirmed by autopsy. Suspicion of toxic myocardiopathy was suggested in another 16 children. Related to the very large number of mothers treated with tocolytic agents, fenoterol-induced myocardiopathy so far appears to be rare. In agreement with other reports, however, our observations suggest the possibility of severe, even perilous myocardial damage. Therefore, the indication and dosage of fenoterol should be subjected to more rigid criteria than so far applied.

[Hemostaseological problems with artificial heart-valves (author's transl)].

The risk of thrombo-embolic episodes in patients with artificial heart-valves has been reduced by means of improved material and of anticoagulation. Kinetic studies, however, indicate a correlation between the risk of thrombo-embolism and an increased platelet turnover. Therefore a therapy with platelet aggregation inhibitors has been recommended. In addition to an increased platelet turnover, other risk factors have been evaluated like the material, the number, and the position of the artificial valves, the time elapsed since surgery, cardiologic disease, and anamnestic criteria. We proposed a semiquantitative risk score taking in account all these risk factors to avoid unnecessary coagulation treatment and to recommend antiplatelet therapy, where necessary. This risk score could be a basis to evaluate controlled studies.

[Different ventilation of lungs - an indication for angiography of pulmonal-artery? (author's transl)].

The radiological symptom of different ventilation of each lung has various causes. There are birth defects and diseases. Different, invasive and non-invasive methods are necessary for diagnosis. The way of diagnosis is shown by an 11-year old girl with absence of the left pulmonal-artery. The different methods are discussed and various own cases of agenesie of the pulmonar-artery in combination with others dysplasias are added.
