Degenerative deformations of the acromioclavicular joint in the elderly.

The authors performed radiological and locomotor examinations of the acromioclavicular joint in relatively high numbers of unselected elderly patients. Data are presented on the incidence, distribution by age, localization and clinical symptoms of arthrosis. Finally the gerontological importance of this particular region is stressed.

[Results of orthopedic and radiological investigations on the knee joint of old age patients (author's transl)].

Orthopedic and radiological investigations were performed on the knee joint of 200 old age patients. The majority of them had no or minute signs, and were free of symptoms. The X-ray characteristics of the degenerative changes of the old age patient's knee joints, and their relationship with the clinical findings are discussed too. The authors point to the relevance of prearthritic conditions in the pathogenesis of those serious osteo-arthritic changes, which become evident in advanced ages.

[The significance of tomography in the x-ray diagnosis of coxarthrosis (author's transl)].

Authors have made besides routine roentgenograms tomograms too in 50 patients with coxarthrosis of various severeness. For an assessment of the diagnostic value of tomography the routine roentgenograms and the tomograms were analysed comparatively. The investigations have shown that the tomography may give in certain cases great help in the judgement of progression and in the correct indication of operative methods in hip surgery.

[Arthrosis of the distal radio-ulnar joint in workers using motorised saws (author's transl)].

Vibration-induced changes in the distal radio-ulnar joint were analysed by comparing workers using motorised saws (978 cases) and a control group. The following are the essential findings: 1. Exposure to vibration causes characteristic changes in the distal radio-ulnar joint, particularly on the left, consisting of osteophytosis, deformity and cystic rarefaction. 2. The frequency of the arthrosis is related in a statistically significant manner to a) length of exposure, b) age. 3. The subjective complaints of numbness, weakness and pain can, to some extent, be related to the radio-ulnar arthrosis. 4. Vibration is not the only factor in the genesis of the degenerative process of the distal radio-ulnar joint; ordinary use and stress can cause similar changes, although to a much less marked extent. The aetiological role of local vibration as a factor in causing the arthrosis has been confirmed.

On the basis of the examination on a rich old-age patient material it may be established that. 1. Radiomorphologically the Forestier's disease essentially differs from the banal spondylosis. 2. Age is of particular importance in its genesis -- it is primarily a disease of old age men. 3. It is localized for the most part on the dorsal vertebral part. 4. As compared to the multiple radiomorphological signs clinical symptoms are, in general, neglectable. 5. Additional search for the relationship with diabetic metabolic disturbances in gerontological patients seems advisable.

Gerontological aspects of degenerative changes of the shoulder-joint. (Radiological and clinical studies).

In a series of radiological studies of both shoulder joint of 505 aged persons considering themselves healthy, the authors have systematized and statistically analysed the changes detected. Based on an increasing frequency as a function of age, on the identical ratio of occurrence by sexes and sides, on simultaneous occurrence, and on the identity of bilateral phenomena, the authors interpret these radiological changes as the consequence of a primary degenerative process. In the course of detailed clinical examinations and tests made in 100 cases taken at random, the authors have found no evaluable number and degree of limitations of motion, nor any evaluable difference in respect of the right and left shoulder joint of males and females.

Gerontological aspects of hyperostosis frontalis interna.

Authors discuss hyperostosis frontalis interna observed in a large number of aged persons, on the basis of age and sex distribution as well as its clinical and roentgenomorphological analysis. In various forms of the ossification of the frontal bone no significant difference was found between the localisation of hyperostosis and the clinical symptoms. On other hand, there is a direct correlation between the extension and severity of hyperostosis and the frequency of occurence of the associated symptoms (obesity, hypertension). They found the aetiological classification more adequate than the morphological categorization of Moore. Their cases are discussed 1. as partial phenomenon of the Morgagni's syndrome; 2. as independent alteration, showing no other symptoms; 3. as transitionary forms inserted between the two groups mentioned above. They discuss also the question of senile, compensatory hyperostosis frontalis interna. On the basis of the study of a large autopsy material they support the opinion that there is a direct connection of this form with old age.

[The Paget's disease--a gerontological study (author's transl)].

Authors found Paget's disease in cases among 1500 patients who considered themselves as healthy. They established their diagnosis on the basis of radiological and laboratory examinations. The incidence rate of Paget's disease was 0,7%, however it must be more frequent in the old groups. The familiar incidence of this illness is not frequent, thus it is rather intriguing that three persons i.e., 2 sisters and 1 brother is one family were found to have Paget's disease showing similar clinical and radiological symptoms. After three years the comparative examinations of two returning aged women showed a progress of Paget's disease. This manifested in exacerbation of osteopathy and increased the activity of the serum-alkaline-phosphatase. The cases of Paget's disease were reported by the authors because of the radiological interest, familiar incidence and the gerontological-geriatrical aspects.

[Arthrosis of the first ray of the hand in geriatric patients (author's transl)].

The frequency of arthrosis in the first ray of the hand increases with the age of the patient, particularly in the saddle joint of the thumb and in the joint between the trapezoid bone and the navicular bone of the hand. The finding is frequently bilateral and is accompanied by kinesalgia, periarticular swelling and restricted movement. A relationship between arthrosis of the first ray of the hand and certain occupations can be assumed. While the arthrosis in the saddle joint of the thumb demonstrates the usual radiological symptoms of arthrosis, the sclerosing of the joint surfaces between the os naviculare and the ossa multangula is the most pronounced.
