Predictive value of some biochemical indices in stone formers.

Search is under way to develop reliable tests for the prediction of stone risk. Several indices and ratios on the basis of urinary excretions have been suggested. In the present study the applicability of some risk indices and ratios in slum dwellers of Dharavi area of Bombay was examined. No significant difference was observed in IAP (ionic activity product) and CORI (calcium oxalate risk index) between stone formers (SF) and normal subjects (NS). We have suggested two more adjuncts, PIR (promoter/inhibitor ratio) and COQ (calcium oxalate quotient), and found them to be quite useful in the detection of risk. Pre-existence of risk factor(s) in the majority of the normal population suggests that triggering of stone formation should be a transient phenomenon in this population. No consistent pattern of relationship between various urinary parameters was observed.

The metabolism of 2,4-dibromo-17 alpha-ethynyl[6,7-3H]oestradiol in the rat.

1. The metabolism of 2,4-dibromoethynyloestradiol (2,4-DBEE2) in the rat was studied in order to determine the influence of ring-A substituents on the phase I biotransformations of oestrogens. 2. 2,4-Dibromo-17 alpha-ethynyl[6,7-3H]oestradiol was synthesized by the one-stage bromination of 17 alpha-ethynyl[6,7-3H]oestradiol (EE2) with N-bromoacetamide, and administered (30 micrograms/kg, i.v.) to anaesthetized male and female rats. 3. A single metabolite, identified as a glucuronide of 2,4-DBEE2, was rapidly and extensively eliminated in bile by male rats (83% of the dose over 6 h). Females excreted additional minor conjugated metabolites. Neither unchanged 2,4-DBEE2 nor EE2 was detected in bile. 4. The hepatic residues after 6 h (percentage of dose) were 2.7% and 3.4% in male and female rats, respectively, whilst less than 0.1% per organ(s) was found in kidneys, heart, spleen, lungs and brain. 5. 2,4-Dibromo substitution of EE2 effectively blocked all phase I biotransformations whilst not limiting glucuronylation in male rats, but did not entirely preclude phase I metabolism in females. The inertness of 2,4-DBEE2 to ring-A hydroxylation in male rats conforms with the insignificant debromination of 2,4-dibromoestradiol by hepatic microsomes.

High-dose, potentially myeloablative chemotherapy and autologous bone marrow transplantation for patients with advanced Hodgkin's disease.

Twenty three patients with Hodgkin's disease were treated with BCNU (carmustine), etoposide, and cyclophosphamide at doses of 450-600 mg/m2, 1500-2000 mg/m2, and 120 mg/kg respectively. Bone marrow refrigerated at 4 degrees C for 2-5 days or cryopreserved at -80 degrees C was used to reconstitute bone marrow function. The median age was 28 (range 16-48), and the median Karnofsky performance status was 70. Nineteen patients had progressive disease while on chemotherapy. The median number of prior regimens was three (1-7), and the median number of prior chemotherapy drugs was 10 (range 4-12). Ten patients had received at least two of the drugs used in this study and four had had all three. Indicator lesions included lung (10), peripheral lymph nodes (9), retroperitoneal nodes (8), liver (3), and chest wall masses (2). Ten patients achieved a complete remission (43.5%; 95% confidence limits 23-64%), and five patients had a partial remission (21.7%; 95% confidence limits 5-39%). The median duration of complete remission was 6 months (range 2-13+ months). Responses were shorter in duration for patients with primary refractory disease. Liver function abnormalities were noted in nine (39%) cases. Post transplant, the recovery time was 18 days (range 11-43) for WBC and 24 days (11-77) for platelets. Two patients died of septic episodes while neutropenic. The median number of RBC units used was seven (range 1-45). Ten patients had evidence of pulmonary dysfunction. In seven patients there was symptomatic improvement with steroid therapy, but three patients who were not treated with steroids died as a result of interstitial pneumonia. Future programs should consider bone marrow transplantation in patients with Hodgkin's disease earlier in the course of disease, at the time of minimal residual disease, and employ newer, potentially less toxic drugs.

Modulation of 5-fluorouracil toxicity by allopurinol in man.

Oxipurinol, the major metabolite of allopurinol, decreased the toxicity of 5-fluorouracil (5-FU) to human granulocyte colony-forming units in vitro by a factor of four. The ability of allopurinol to reduce 5-FU toxicity in vivo was studied in 23 advanced cancer patients during 42 courses of treatment. 5-FU was administered by continuous intravenous infusion for five days; allopurinol, 300 mg, po, every 8 hours was started 2 hours before and continued during and for 24 hours after 5-FU infusion. 5-FU was escalated from 1.5 to 2.25 g/m2/day on separate courses; the dose-limiting toxicity was mucositis which occurred at a level of 2.0 g/m2/day. At a 5-FU dose rate of greater than 2.0 g/m2/day 5-FU pharmacokinetics were nonlinear, reflecting saturation of catabolic pathways, and the steady-state 5-FU serum concentration was approximately 4 times that which was tolerable without allopurinol. At these concentrations of 5-FU oxipurinol significantly influenced the clearance of 5-FU. Thus concurrent allopurinol therapy permitted a doubling of the maximum tolerated dose of 5-FU and a four-fold increase in the tolerated concentration x time exposure to 5-FU.

A survey of blood groups.

A survey was conducted to investigate into the frequency of different blood groups in Punjab. A total of 1415 persons were included in this survey. The slide method was used for determination of ABO and AB blood groups as well as Rh factor. The frequency of blood group A was 21.20%; B, 36.16%; AB, 9.05% and O, 34.14%. Distribution of blood groups among various castes revealed the incidence of blood group A, 13.57% to 30%; B, 28.125% to 50%; O, 16.67% to 40%; and AB, zero to 25%. Only 2.76% cases were found to be Rh negative. Rh negative frequency was much higher in Baluchs, Awans and Gujjars than Rajputs, Jats and Arains.

A novel triple label method validates the double label technique of defining cell cycle kinetics in HL-60 cells.

HL-60 cells were sequentially labeled with the thymidine analogues iododeoxyuridine (IUdR) and bromodeoxyuridine (BrdU). The labeling index (LI), the duration of S-phase (Ts) and the total cell cycle time (Tc) were measured immediately. It was therefore possible to predict the next time when the single versus double labeled cells would re-enter the S-phase. In our study, the Tc was calculated to be 20 hours. The third label, tritiated thymidine (3HTdR), was introduced at the predicted time of 20 hours to confirm the validity of the previously calculated Tc. The actual percentage of cells which were labeled by (3HTdR) was very similar to the predicted value. We conclude, therefore, that the calculated cell cycle time correlated well with the actual cell cycle time, at least in a controlled in vitro culture system. This novel triple label method validates our double-label technique developed for cell cycle measurements.
