Onychauxis is a common nail disorder, particularly among the geriatric and debiliated populations. The hypertrophic nail plate proves unsightly and uncomfortable. Surgical options--intended to offer permanent correction--and nonsurgical modalities--designed to provide symptomatic relief--are reviewed.
A second case of compartment syndrome affecting the dorsal aspect of the foot has been presented. The syndrome was arrested by fasciotomy. There were no permanent neurologic, vascular, or musculoskeletal sequelae. Although techniques exist to obtain quantitative measurements of intracompartmental pressure, the diagnosis rests heavily on clinical assessment. Practitioners who encounter trauma victims must recognize the symptoms of the condition and be familiar with the procedures necessary to interrupt it.
Brachymetatarsia describes the condition of an abnormally short metatarsal. Although the condition has been recorded since antiquity, surgical options to correct the deformity have been available for only two decades. Most published procedures involve metaphyseal lengthening with autogenous grafts from different donor sites. The author discusses one such surgical technique. In addition, the author proposes specific criteria for the objective diagnosis of brachymetatarsia.
Tumefactive or nodular amyloidosis represents an uncommon variant of the disease. Localized tumorous masses, which are lardaceous in appearance, rubbery in texture, and approaching several centimeters in diameter, typify the presentation. Such lesions pose unique problems when situated on the weight-bearing regions of the plantar foot. The author discusses the use of accommodative weight-dispersing insoles as an alternative to surgical excision for the treatment of such lesions.
Compartment syndrome presents an emergency situation to the surgeon. Inordinate delay in performing a fasciotomy poses potential loss of limb or irreversible neuromuscular damage. Recognizing those injuries that may cause a compartment syndrome and careful monitoring of the at-risk patient during the early post-traumatic period are mandatory.
