Outcome of postpartum disorders: a 10 year follow-up of hospital admissions.

OBJECTIVE: To determine readmission and recurrence rates of women admitted to psychiatric hospital with their babies, and index factors predicting subsequent course. METHOD: A total of 66 women were followed up after 10 years. Information was obtained from structured interviews or other personal contact, and from general practitioners and hospital notes. Psychiatric diagnoses were made according to Research Diagnostic Criteria (RDC). Outcome and predictive factors were investigated using survival analysis. RESULTS: A recurrence rate of 87.2%, and a readmission rate of 63.3% were found. Readmissions were more common after index schizophrenia than other diagnoses. The strongest predictor of readmission was previous psychiatric history. Among 27 subsequent pregnancies, puerperal recurrence was high among those with index psychoses (75-80%), but lower among those with index depression (27.3%). CONCLUSION: The recurrence rate is high, with a moderate readmission rate predicted by past psychiatric history. Continuing clinical vigilance over the long-term is required for these patients.

Douglas House. Seven years' experience of a community hostel ward.

Of 24 residents of a ten-bed, community-based hostel ward suffering chronic psychiatric illness, nine have been resettled in the community, with four more expected to follow them. Five residents have made Douglas House their home but another six have manifested behavioural disturbance necessitating return to hospital wards. We found community discharge to be associated with illnesses having good prognostic features, while organic illness militated against such placement. Indicators of a prior history of behavioural disturbance seem to predict difficulties in managing a patient in this environment.

'Ecstasy' psychosis and flashbacks.

We report three cases of flashbacks and one case of recurrent psychosis experienced by three MDMA users. We draw the attention of clinicians to the combination of adverse effects observed with this drug of abuse and to the advisability of prolonged treatment should they occur.

Naloxone applied to conjunctiva as a test for physical opiate dependence.

Significant pupillary dilatation in response to conjunctivally applied naloxone was demonstrated with polaroid photography in 46 opiate addicts on maintenance methadone prescription. No pupillary response was seen to an identical procedure in 17 non-addict subjects who had been given an opiate premedication before elective surgery. This pupillary response to naloxone could be used as a test to identify the physically dependent opiate user.
