Umbilical vein blood flow measurement in nonimmune hydrops fetalis.

Twenty-five measurements of fetal umbilical vein blood flow were performed in 22 cases of nonimmune hydrops fetalis using real-time and pulsed Doppler duplex ultrasound. The umbilical vein diameter, blood velocity, and blood flow in fetuses with hemoglobin Bart's hydrops fetalis were usually higher than those in fetuses with hydrops fetalis from other causes. Umbilical vein blood flow measurement appears to be an effective technique for differentiating hemoglobin Bart's from non-Bart's hydrops in this series. This hemodynamic characteristic of umbilical vein blood flow may be helpful in determining the etiology of nonimmune hydrops fetalis.

Reduced doses of hepatitis B immune globulin in the prevention of perinatal transmission of hepatitis B.

From October 1982 to May 1983, newborn infants of 79 hepatitis B surface antigen (HBsAg)-positive women were enrolled in a study of the efficacy of hepatitis B immune globulin (HBIG) in the prophylaxis of perinatal transmission of hepatitis B virus (HBV) infection. HBIG 0.5 ml or 0.25 ml was given to the newborn within 15 minutes of birth and at 3 and 6 months. The mother-infant pairs were followed-up every 3 months for at least 9 months. Similar observations of untreated infants were used for comparison. Among infants of hepatitis B e antigen (HBeAg)-positive carrier mothers, the HBsAg carrier rates at 3 months were similar in the 0.5-ml and 0.25-ml HBIG dose groups. At 12 months the difference--17.7% of 17, 40% of 15--did not reach statistical significance, but the differences between these rates and that of the untreated control-85.7% of 35--did. Among infants of HBeAg-negative carrier mothers, HBV infection rates in both dose groups were similar to those of untreated infants. In the treated groups at 12 months about 45% of infants of HBeAg-positive mothers and 90% of infants of HBeAg-negative mothers were still negative for HBsAg and anti-HBs. Vaccination to induce active antibody is necessary to prevent postnatal infection and chronic carriage of HBV. To reduce the cost of combined passive and active hepatitis B immunoprophylaxis in children born to HBeAg-positive carrier mothers, 0.25 ml of HBIG could be used instead of the usually recommended 0.5 ml.

Percutaneous ultrasound-guided fetal blood sampling in the management of nonimmune hydrops fetalis.

Nonimmune hydrops fetalis can be caused by fetal chromosomal, hematologic, cardiac, or infectious conditions. A fetal blood sample obtained from percutaneous ultrasound-guided fetal blood sampling can offer vital information about the underlying cause of nonimmune hydrops fetalis, and perinatal management can be planned accordingly. Ten cases of nonimmune hydrops fetalis were investigated with percutaneous ultrasound-guided fetal blood sampling and subsequent fetal blood analysis. Nine were proved to be Bart's hemoglobin hydrops fetalis, with 78.2% to 99% of Bart's hemoglobin in the fetal blood. Terminations of pregnancy were undertaken immediately, resulting in a lowered maternal morbidity rate. Free cord loops or an intra-abdominal portion of the dilated umbilical vein can be used for percutaneous fetal blood sampling in nonimmune hydrops fetalis when the placental cord insertion is difficult to visualize. Percutaneous ultrasound-guided fetal blood sampling and subsequent fetal blood analysis will eventually play a key role in the management of the puzzling nonimmune hydrops fetalis if the safety of ultrasound-guided fetal blood sampling can be verified by further clinical studies.

The placental transfer and concentration difference in maternal and neonatal serum uric acid at parturition: comparison of normal pregnancies and gestosis.

The maternal uric acid (MUA) and neonatal uric acid (NUA) levels were measured simultaneously at parturition in three groups of pregnancies: group I - 83 cases of normal pregnancies, group II - 7 cases of mild gestosis and group III - 12 cases of severe gestosis, totaling 102 cases. Umbilical venous blood samples were taken in all of the cases. Maternal venous blood samples were obtained from 69 patients in group I, 6 cases in group II, and 12 cases in group III. The correlation coefficients of MUA and NUA values were 0.90, 0.91, and 0.95 (all p less than 0.01) in the three groups, respectively, and 0.93 in total series (p less than 0.0001). The high correlation and minimal concentration difference between MUA and NUA in either normal or gestosis suggested free transfer of uric acid via placenta in both directions. Moreover, not only MUA but also NUA levels were significantly different among normal and gestosis groups, and both MUA and NUA showed higher levels in accordance with the severity of gestosis. Both MUA and NUA had negative correlation with birth weight (BW), one-minute apgar score (AS-1) and five-minute apgar score (AS-5). It implied that the uric acid levels at parturition might provide as a reference index for fetal outcome in pregnancy with gestosis.

Antenatal measurement of fetal umbilical venous flow by pulsed Doppler and B-mode ultrasonography.

In this study, 163 normal fetuses of between 25 and 41 weeks' gestational age were examined sonographically using B-mode and Doppler equipment to measure umbilical vein diameter, umbilical venous flow amount (UVFA) and velocity (UVFV), and abdominal area (AA). A total number of 230 examinations were performed. The relationships between gestational age and umbilical vein diameter, UVFA, UVFV, and the UVFA/AA ratio were analyzed. Throughout pregnancy, the umbilical venous blood flow velocity remained relatively constant, whereas the umbilical venous blood flow amount increased steadily with gestational age and was proportional to abdominal area.

The outcome of pregnancy after chemotherapy for gestational trophoblastic disease.

Thirteen pregnancies in 11 women previously treated with cytotoxic chemotherapy for GTD were studied. There were 11 live full-term babies (84.6%), 2 blighted ova (15.4%), 1 placenta accreta (7.7%) and 1 fetal anomaly (7.7%). Unlike other series, 69% of the pregnancies occurred within 1 year after the termination of chemotherapy. The rate of successful pregnancy was not different from that reported by others. Prenatal elimination of abnormal embryos after conception rather than wastage of damaged oocyte before conception might explain this observed low incidence of abnormal birth in GTD patients treated with cytotoxic chemotherapy.
