Vulvar pseudolymphoma.

We report a 31-year-old patient with longstanding complaints of vulvar itching and painful intercourse treated with many antimycotic drugs. On examination a small tumor, clinically not differentiable from a malignant lymphoma, on and near the clitoris was found. After excision biopsy the lesion proved to be a pseudolymphoma. Six months after excision of this rare vulvar tumor the patient was completely free of complaints. Although pseudolymphomas are small benign tumors they may case diagnostic problems and major complaints. To our knowledge this very rare disorder has not been reported in the gynecological literature until now.

The pliable obstetrical vacuum cup: application and opinions in The Netherlands.

A questionnaire was sent to all hospital obstetric units in The Netherlands to obtain information about the use of soft cups for vacuum extraction. Over 90% of the 156 units responded. Vacuum extraction was used twice as often as forceps for instrumental vaginal delivery. In 12 units the flexible cup was applied in the majority of vacuum extractions and without specific restrictions. In 47 units a minority of vacuum extractions was performed with a flexible cup: its use was generally restricted to anticipated easy low outlet extractions. It is concluded that in The Netherlands the flexible cup is an accepted instrument used in about 13% of vacuum extraction deliveries.

A case of hidradenitis suppurativa treated with gonadotropin-releasing hormone agonist and by total abdominal hysterectomy with bilateral salpingo-oophorectomy.

We report on a patient with hidradenitis suppurativa with premenstrual exacerbations, who was treated successfully with total abdominal hysterectomy and bilateral salpingo-oophorectomy combined with estrogen substitution. As a trial, she was initially treated successfully with a gonadotropin-releasing hormone agonist and subsequently a gonadotropin-releasing hormone agonist in combination with estrogen substitution.

A modified technique to create a neovagina with an isolated segment of sigmoid colon.

The creation of a functional vagina in patients with congenital vaginal aplasia or male transsexualism is a challenging problem. A group of 40 patients, including 23 male transsexuals, in whom a neovagina was created using a sigmoid transplant, is reported. The technique, a modification of Kun's "colocolpopoiesis," is described in detail. Ten patients showed some direct postoperative complications and five were readmitted the first six weeks postoperatively for a variety of reasons. No extensive complication occurred. Thirty-two patients were evaluated at the routine six week postoperative check-up. Four patients had had intercourse at that time and an adequate vagina was found in 21 other patients. It is concluded that this modification of Kun's technique, known as colocolponeopoiesis, has had, at short term in the majority of patients, functionally good results and an acceptable complication rate.

Errors in microsphere determination of cardiac output: a computer simulation in fetal sheep.

Through use of a compartmental model, we simulated the measurement of cardiac output and distribution by means of radioactively labeled microspheres in fetal lambs with weights between 0.5 and 3 kg. A systematic error in measured cardiac output caused by artifactual changes in blood volume caused by the injection and withdrawal of fluids during the procedure was less than 5% for fetal weights greater than or equal to 1 kg but increased for fetal weights less than 1 kg and when hypovolemia was simulated at all fetal weights. Sensitivity analysis disclosed no significant effect of changes in vascular resistance. We examined the effects of recirculation of microspheres and found no significant increase in error in the measured value of cardiac output due to 20% recirculation of all spheres entering a single isolated organ system; however, errors between 7 and 14% were observed with simulations of 20% recirculation in more than one compartment simultaneously. Recirculation also introduced significant errors in the measured distribution of cardiac output in certain cases. The effect on the measured cardiac output of a temporary change in the true cardiac output was dampened by the artifactual blood volume changes mentioned above and the fact that the measurement is a time-weighted average. We also evaluated four different experimental designs. We conclude that the microsphere technique provides a remarkably reliable means of quantifying cardiac output and individual organ flow in the fetus. The nonrandom errors inherent in the procedure examined in this study are of the order of 10%, which is likely to be less than the moment-to-moment variation in the true cardiac output.

Hypocalciuria in hypertensive disorder in pregnancy and how to measure it.

We studied the urinary calcium excretion in 41 patients in the third trimester of pregnancy. There was a significant decrease in 24-hour calcium excretion in hypertensive and pre-eclamptic patients. The 24-hour calcium excretion correlated well with the calcium to creatinine ratio of a single voided urine sample. We conclude that measuring urinary calcium excretion may be valuable in the care and the study of pregnant patients with hypertensive disorders, and that the 24-hour urinary calcium excretion can be estimated from a single voided urine sample.
