[Biological markers of alcoholism].

Diagnosis of alcoholism is very important, given its high prevalence and possibility of influencing the disease course. For this reason, the so-called biological markers of alcoholism are useful. These are analytic parameters that alter in the presence of excessive alcohol consumption. The two most relevant markers are the gamma-glutamyltranspeptidase and carbohydrate deficient transferrin. With this clinical comment, we aim to contribute to the knowledge of these tests and promote its use in the clinical practice.

Vitamin B6 status in uremia.

We have studied vitamin B6 status in 26 uremic patients, 18 on maintenance hemodialysis and 8 nonhemodialyzed. The vitamin B6 status was estimated by an assay of erythrocyte aspartate aminotransferase and coenzyme stimulation. Hemodialyzed uremic patients were found to have vitamin B6 deficiency. Patients were treated with 150 mg pyridoxine hydrochloride daily for 4 weeks. Erythrocyte aspartate aminotransferase increased significantly in both groups of uremic patients, the increase being greater in hemodialyzed patients. In vitro pyridoxal phosphate stimulation produces an erythrocyte aspartate aminotransferase activity greater than that obtained before pyridoxine hydrochloride administration. After cessation of pyridoxine hydrochloride treatment, erythrocyte aspartate aminotransferase decreases in hemodialyzed patients, while it remains elevated in nonhemodialyzed patients. The data obtained appear to indicate that vitamin B6 administration to patients with chronic renal insufficiency must be appraised not only for correcting the deficit but also for increasing the intracellular pyridoxal phosphate concentration, which could modify the possible functional impairment at the level of apoenzymes that use pyridoxal phosphate.

Carpal tunnel syndrome and vitamin B6.

Twelve patients with carpal tunnel syndrome were studied. Clinical and electrophysiological data were obtained and an estimation of vitamin B6 (pyridoxine) status by an assay of erythrocyte aspartate aminotransferase and coenzyme stimulation assay were done. None of the patients was found to have vitamin B6 deficiency. Patients were treated with 150 mg of pyridoxine daily for 3 months. Erythrocyte aspartate aminotransferase increased significantly (p less than 0.001) in all the patients. In 6 patients there were clinical and electrophysiological improvement and erythrocyte aspartate aminotransferase increased more than in the other 6 patients. The data obtained appear to indicate that although vitamin B6 deficiency is not common in carpal tunnel syndrome patients, pyridoxine supplementation can be recommended as adjuvant treatment in those patients undergoing surgery.

[Round pneumonias].

Two cases of pneumonia in which the initial radiologic sign was the solitary nodule are presented. The symptoms associated permitted the infectious origin. The clinical and radiological evolution after antibiotic therapy supported the diagnosis of rounded pneumonia. We highlight the need to discard benign lesions causing the solitary nodule, before trying to diagnose other diseases.

[Interstitial pulmonary disease in plaster workers].

Two cases of diffuse interstitial lung disease in plaster workers with clinical, histological and immunological data suggestive of hypersensitivity pneumonitis are presented. Special remark is made on the immunological results, both cellular (increase in activated T cells and Natural Killer cells) and humoral (immunoglobulins) from the bronchoalveolar lavage. We tried to detect the antigen although we did not succeed in its characterization and we discuss the relationship between these two cases and other hypersensitivity pneumonitis. We discuss the importance of the immunological study of cells obtained from bronchoalveolar lavage for the diagnosis of alveolitis with a lymphocytic predominance. Finally, we suggest that 99 Tc-DTPA gammagraphy can be usefull to monitor the evolution of the alveolitis in these patients.
