Ioversol in ascending phlebography--a clinical trial.

In this controlled randomised double-blind parallel group study of the use of ioversol-240 and ioversol-320 in venography all studies were considered diagnostic with comparable quality in the two groups. Patient tolerance was high with mild heat observed in 7 patients in the ioversol-320 group and 1 patient in the ioversol-240 group. Assessment of pain was also comparable (2 patients in the ioversol-240 group and 1 in the ioversol-320 group). Both strengths of the contrast agent produced no clinically significant, drug related, changes in vital signs or laboratory parameters and there were no significant clinical adverse reactions.

Rapid analysis of triacylglycerols using high-performance liquid chromatography with light scattering detection.

This paper details a high-performance liquid chromatography (HPLC) method for the separation of triacylglycerols, using a 3-micron, 15 cm x 4.6 mm I.D. Spherisorb ODS column and gradient elution with dichloromethane and acetonitrile. The triacylglycerols are detected using a light scattering detector (mass detector). Separations of a number of different edible oils and fats are reported. The procedure offers a possible method for determining cocoa butter equivalents and the adulteration of edible oils and fats by other non-generic fats and oils.

Preoperative embolisation in the management of juvenile nasopharyngeal angiofibroma.

Thirteen cases of juvenile angiofibroma were treated surgically with preoperative embolisation. One case occurred in a female. Embolisation significantly reduced blood loss and allowed complete surgical removal of the tumour in all cases. No significant complications occurred. Preoperative embolisation should be employed routinely in the surgical management of these patients.

Multiple cholesterol emboli syndrome complicating angiographic techniques.

Three cases of widespread microembolisation of cholesterol crystals following angiography are described. The multiple cholesterol emboli syndrome has been well described as a spontaneous phenomenon and it is surprising that its occurrence during angiography appears rare. Only 19 cases could be found in the literature and these are reviewed. Dissemination of particulate cholesterol material produces irreversible organ ischaemia when a threshold 'dose' is reached. Males and patients with clinical evidence of widespread atherosclerosis are at increased risk. Prognosis is poor and available therapy unsatisfactory.

Nonfunctioning parathyroid carcinoma: a case history.

Parathyroid carcinoma is an uncommon endocrine tumor. Its usual clinical presentation is that of primary hyperparathyroidism with elevated serum calcium and parathormone levels. Nonfunctioning carcinoma of the parathyroid gland with normal serum calcium levels is very rare. There are 12 reported cases in the literature. This paper is a case report of a nonfunctioning parathyroid carcinoma and a review of the literature.

Angiography in the diagnosis of renal allograft dysfunction.

We reviewed retrospectively 75 renal transplant arteriograms done during a 7-year period. Acute rejection and vasomotor nephropathy were not differentiated. Generalized cortical ischemia was diagnosed correctly in 23 of 30 cases but there were 7 falsely negative results. Renal artery stenosis was found in 7 of 17 cases in which the main indication for arteriography was hypertension. We conclude that the major role of transplant arteriography is in the diagnosis of larger vessel disease.

Peripheral pooling of bronchographic contrast material: evidence of its relationship to smoking and emphysema.

Sixty-six subjects, mainly derived from various occupational groups and one-third of whom admitted to dyspnoea on exertion, have been grouped according to the appearance of their peripheral airways at bronchography with oily propyliodone. Eleven subjects showed marked peripheral pooling of radiographic contrast material, 22 showed mild or moderate pooling, and in 33 peripheral pools were absent. Pooling was not seen in non-smoking subjects. In the group of subjects without pooling, pulmonary function in non-smokers and subjects with a history of smoking was similar. Subjects with marked pooling had a significantly lower pulmonary diffusing capacity (transfer factor) and evidence of loss of pulmonary elastic recoil when compared with subjects with absent peripheral pooling. These results indicate that bronchographic peripheral pooling is associated with the physiological changes of panacinar pulmonary emphysema and suggest that a causal relationship may exist between the organic bronchiolar lesion of pooling and the peripheral parenchymal lesion of panacinar emphysema.

Metastatic melanoma simulating subdural hematoma. Case report.

Metastatic melanoma tends to be found in subcortical white matter and this localization may produce clinical or radiological features that are unusual in metastatic disease. A case is described that closely simulated a subdural hematoma both clinically and radiologically.
