Reconstruction of the nasal tip using nasalis myocutaneous sliding flaps.

An alar island subcutaneous sliding flap is described which, when analyzed, is actually myocutaneous, based on the lower portions of the nasalis muscle. In a series of 47 patients, tip defects 1.25 cm in diameter were reconstructed with a unilateral flap, and defects 2.0 cm in diameter were reconstructed with a bilateral flap. The advantage of the flap lies in aesthetics, which are so important in tip surgery. Not only are the incisions at or parallel with the edges of the lateral crus of the lower lateral cartilage, but also chronic edema and "dog-ears" so common after rotation pedicle flaps are avoided.

The experimental value of silicone-sheet, dacron felt spacers in prevention of capsular contractures.

An experimental study is recorded that compares the use of soft gel silicone implants with, and without, a silicone sheet Dacron felt spacer, using the scalp of the guinea pig as a test site. Capsular contractures were produced in 45 percent of the control animals without the spacer. This was documented histologically to be related to a migration of muscle fibroblasts and could be prevented by the fixation of the Dacron felt to the pocket floor peripheral to the implant.

Thumb reconstruction with a sensory flap from the dorsum of the index finger.

The dorsal skin of the index ray is very useful (1) for a one-staged thumb lengthening procedure after amputation, (2) for covering the stump of an avulsed thumb with sensory skin, and (3) for expanding the first web space. The flap may be transferred as a rotation flap, or the dorsal vasculature and nerve supply to the index may be carefully dissected free as a pedicle to permit its use as a neurovascular island flap. We believe that considerably more sensory skin can be transferred by this flap than by the ring finger neurovascular island flap, and that the technical risks and surgical time are less with the index finger flap.
