Pelvic cellulitis: a life-threatening complication of hemorrhoidal banding.

Rubber band ligation is a commonly employed office procedure for the eradication of symptomatic internal hemorrhoids. Since 1980 increasingly frequent reports of an often fatal complication--"pelvic cellulitis"--have appeared. Death has been avoided for some of these patients by early recognition and treatment. One such survivor is reported here, in a case report that illustrates the value of early recognition of symptoms and appropriate diagnostic and therapeutic intervention.

Visceral neoplasia presenting at the umbilicus.

An umbilical mass as the presenting and only symptom of an underlying visceral carcinoma is an infrequent occurrence. The implications of such a finding are illustrated by four examples, with primary tumors in pancreas, colon, and ovary. Adenocarcinoma in the umbilicus represents a metastatic deposit until proved otherwise. Further diagnostic evaluation to localize the primary tumor should be based upon therapeutic implications. The need for abdominal exploration must be individualized.

Pharyngeal pseudodiverticulum in an adult.

Posterior perforation of the cervical portion of the esophagus may present deceptive clinical and roentgenographic pictures. In the neonate, the clinical picture resembles that of esophageal atresia; in the adult, the roentgenographic appearance may resemble Zenker diverticulum. In either instance, treatment should be no different from that employed for more commonplace esophageal perforations, namely, prompt, aggressive surgical intervention.
