Succinylcholine-induced ventricular arrhythmia during halothane anesthesia.

A case of ventricular tachycardia in a healthy child subsequent to onset of a succinylcholine infusion is described. A 9-year-old male patient was scheduled for an adenoidectomy and bilateral tonsillectomies. There was no history of cardiac arrhythmia. Anesthesia was induced by mask with nitrous oxide and halothane. Immediately after the infusion of 20 mg of succinylcholine, the heart rate was 150 beats/min and then the electrocardiograph (ECG) revealed a short run of ventricular tachycardia. The patient was given 1 mg of propranolol, and enflurane and 100% oxygen were administered. Soon after the propranolol was given, normal sinus rhythm returned. Ventricular tachycardia may occur after a single dose of succinylcholine in a healthy anesthetized child without predisposing disease.

Affinity of chromatin constituents for isopeptidase.

Isopeptidase is a novel eukaryotic enzyme that cleaves a structural chromatin protein, A24, stoichiometrically into H2A and ubiquitin. To understand the rapid turnover of ubiquitin in mitosis as well as the high specific activity of the enzyme associated with metaphase chromosomes, attempts were made to determine chromatin constituents that show high affinity for this enzyme. Endogenous protease-free isopeptidase was prepared from calf thymus and applied to a Sepharose 4B affinity column on which histones, DNA, NHCP and ubiquitin were respectively immobilized. The enzyme proved to bind only histones. To further determine which of the histone fractions is involved, affinity columns with each histone fraction were also used. The enzyme showed affinity for all histone fractions. However, the strength of affinity varied in the order H2A greater than H3 greater than H2B greater than or equal to H4 much greater than H1, being inversely correlated with the ratio of basic/acidic amino acids in these molecules. These results suggest that the turnover of A24 in mitosis is controlled, at least in part, by the affinity of enzyme for histones, and also that such affinity is caused by a mechanism which cannot be explained simply by the electrostatic interaction between negatively charged enzyme molecules and positively charged histones.

Nuclear ribonucleoprotein particles in the cellular slime mold Dictyostelium discoideum.

The nuclear ribonucleoprotein (RNP) particles containing rapidly labeled RNA were isolated from interphase cells of the cellular slime mold Dictyostelium discoideum and characterized. The size of the isolated RNP particles was small (10S to 50S) in comparison with that of nuclear RNP particles found in higher eukaryotes. These small RNP particles do not seem to be artifacts due to degradation during the preparation of nuclear extracts. The rapidly labeled RNA of the nuclear RNP particles was heterogeneous in size and a considerable amount contained polyadenylic acid sequences. Synthesis of RNA in the nuclear RNP particles was resistant to a relatively high concentration of actinomycin D. The protein component of the RNP particle consists of at least four proteins with molecular weights of 80,000, 66,000, 60,000, and 42,000. Thus it is suggested that almost all of the nuclear RNP particles containing rapidly labeled RNA in interphase cells are RNP complexes consisting of Heterogeneous nuclear RNA and several protein species.

Competitive inhibition of hexokinase isoenzymes by mercurials.

A difference in the mode of inhibition of hexokinase [EC 2.7.1.1] isoenzymes by p-chloromercuribenzenesulfonate was confirmed with respect to glucose between two Type I isoenzyme preparations purified from the kidney and spleen of rat. Essentially the same difference was observed when galactose was used as the substrate in place of glucose, as the kidney Type I isoenzyme was inhibited in a competitive manner while the spleen counterpart was inhibited in a non-competitive manner by sulfhydryl inhibitor. Both the Type I isoenzymes, however, were competitively inhibited by other mercurial sulfhydryl inhibitors, methyl and butyl mercuric chlorides. On the other hand, the Type II hexokinase isoenzymes purified from the muscle, heart, and spleen were all inhibited competitively by p-chloromercuribenzenesulfonate with respect to glucose. The mechanism of competitive inhibition of the hexokinase isoenzymes by sulfhydryl inhibitors was discussed in view of the difference in the mode of action of the mercurials with different isoenzymes.

Somatosensory evoked potentials in rats with hypocalcemia.

The effects of hypocalcemia on somatosensory evoked potentials (SEPs) were studied in rats. The SEP in awake rats consisted of an unstable, large, negative wave (Nmax) preceded by a small positive one (P1). The P1 appeared to be a subcortical potential and Nmax appeared to represent the function of the cerebral cortex. With the decrease in calcium concentration caused by parathyroidectomy, as well as by administration of EDTA, an augmentation of amplitudes of the Nmax and a reduction in the SEP recovery functions were observed. The latency of P1 was hardly changed. The decrease in the SEP recovery functions possibly reflects a reduction in synaptic efficiency, and the increasing amplitude of the SEP probably reflects the cortical hyperexcitability caused by hypocalcemia.

Somatosensory evoked potentials in rats with acute uremia.

The effects of acute renal failure (ARF) on somatosensory evoked potentials (SEPs) were studied in rats. With the increase in blood urea nitrogen caused by the ligation of bilateral ureters, both a significant augmentation of amplitudes of the SEP and an increase in the SEP latencies were observed. The nerve conduction velocities of peripheral nerves were unchanged. Thus a kind of cortical irritability and a lesser degree of damage in the peripheral nervous system are characteristics in ARF.

[A case of cephalic tetanus presenting with opisthotonus].

We report a case of cephalic tetanus presenting with opisthotonus. A 49-year-old man was admitted because of repeated convulsions. The patient was a garbage truck driver known to be alcoholic, who fell down to suffer an injury of the left face two days before the onset of convulsion. Intravenous administration of diazepam and phenytoin partially relieved the convulsions. Anti-tetanus human immuno-globulin was also administered despite absence of typical clinical sign. Six hours later, however, the patient became unable to open the mouth, i.e. lockjaw developed, and the diagnosis of tetanus was made. Additional anti-tetanus human immunoglobulin of 3,000 units and 4,500 units on the next day rapidly relieved the lockjaw, convulsion, and general muscle rigidity without sequalae. The patient showed transient bilateral facial palsy and rotatory nystagmus during the course. Cephalic tetaus is characterized by a history of an injury of the head and a short latency before developing generalized tonic convulsion or opisthotonus. While a typical case presents with lockjaw, our case presented with opisthotonus, presumably because of early systemic lymphatic spreading of tetanus toxin. Early diagnosis and treatment is important to prevent generalized convulsions which are more frequent and sometimes lethal in cephalic tetanus than the common form.

Somatosensory evoked potentials in Huntington's disease--studies with paired stimulation.

The somatosensory evoked potentials (SEPs) and recovery functions of the SEP with paired stimulation were studied in patients with Huntington's disease (HD). A significant decrease in amplitude of the SEP was observed in 4 of the 5 patients. In addition, with paired stimulation, the second SEP amplitudes were normalized at intervals of about 10 ms. These findings possibly suggest that functional block rather than organic change in the thalamus may play an important role in the reduction in SEP amplitude in HD.
