[Diagnosis of Alzheimer's disease in old age].

A diagnosis of Alzheimer's disease in persons at the age of 75-89 years is rarely established by clinician as well as by pathologist without aimed attention. As usual it is mixed up with cerebral arteriosclerosis or marasmus senilis. More than 10 drusen were found in a low-magnification field [400X] in hippocampal cortex and in area 21 and 39 according to Brodman of 19 persons in a group of 26 autopsies. Diagnosis of Alzheimer's disease based on this commonly used limitation was quite frequent, then.

[Diagnosis of Alzheimer's disease in the aged. II. Quantitative aspects].

A quantitative aspect was instrumental in the distinction of late Alzheimer's disease from senile brain lesion in 9 cases from a group of 10. Eight to ten plaques in one microscopical field [magnification 400 X] were a decisive finding in Brodmann's area 10 or 21. Neuronal decrease in nucleus basalis was a less reliable feature [7: 10], neuronal frequency in locus coeruleus was the same as in control group. Another criterion seems to be necessary when border line cases of histological Alzheimer's disease with dementia and without dementia are to be distinguished. Subclinical forms of Alzheimer's disease in old age can be supposed because of the finding of plaques in neocortex and neuronal decrease in nucleus basalis in several cases of the control group.

[Visceral giant cell arteritis similar to polyarteritis nodosa].

Biopsy from the small and large intestines of a 60-year old female patient with intestinal tract ulcerations helped to reveal angiitis similar to polyarteritis nodosa but with progressive proliferation of giant multinuclear cells in the inflammatory infiltrate rich in eosinophils. As much as 20-31% eosinophils were found in peripheral blood. Postmortem examination also confirmed the presence of angiitis in the kidneys, liver and thalamus. The position of arteritis between polyarteritis nodosa and visceral giant cell arteritis is discussed.

[Tularemic lymphadenitis].

Three cases of granulomatous purulent colliquative lymphadenitis showed histologioal similarity with caseous tuberculous lymphadenitis as well as with some other granulomatous lymphadenites. It was only a positive serological test that suggested the diagnosis of tularemia.
