Training and generalization of expressive syntax in nonfluent aphasia.

Generalization of treatment effects remains a consistent goal of clinicians who treat aphasic adults. Specifically, various types of stimulus/response generalization designs are available, depending on the level of generalization desired. We have reviewed training matrices designed to elicit gestural subject-plus-verb targets and treatment studies designed to answer more global questions regarding treatment efficacy. As Kearns (1989) so eloquently states, "clinical accountability cannot, in fact, be fully achieved without documentation of communication skills trained in the clinic generalized to other settings and situations." Although the studies cited and data reviewed in this article did not meet this standard specifically, an attempt was made to provide a brief prelude to the data now emerging in the aphasiology literature. Additionally, with an increasing data base in the literature specific to generalization, more efficacious treatment paradigms will be made available. Clinicians treating aphasic adults thus should have the opportunity to choose the treatments shown to be most effective for a specific type and severity of aphasia.

Denial of visual perception.

An adult with the diagnosis of cortical blindness, complaining of a complete visual loss of 2 years in duration, was found to have a small preserved visual field and remarkably preserved visual abilities. Although denying visual perception, he correctly named objects, colors, and famous faces, recognized facial emotions, and read various types of single words with greater than 50% accuracy when presented in the upper right visual field. Upon confrontation regarding his apparent visual abilities, the patient continued to deny visual perceptual awareness, typically stating "I feel it." CT indicated bioccipital lesions sparing the left inferior occipital area but involving the left parietal lobe. The denial of visual perception evidenced by this patient may be explained by a disconnection of parietal lobe attentional systems from visual perception. The clinical presentation is described as representing "inverse Anton's syndrome."

Cueing verbs: a treatment strategy for aphasic adults (CVT).

This investigation demonstrated that the treatment protocol, "Cueing Verbs: A Treatment Strategy for Aphasic Adults" (hereafter referred to as CVT), was an effective rehabilitation technique. Specifically, this CVT was shown to be beneficial with marked/moderate to mild fluent aphasic adults. Additionally, the study addressed such issues as optimal treatment frequencies, treatment initiation and termination points, and criteria for success and failure. Finally, the six-tier hierarchy of treatment levels are presented with all variables operationally defined.

Speech and language disturbances.

A basic acquaintance with a variety of deviations in speech and language behavior that are encountered in a clinical setting is provided. Clinical differences among the various disorders are established, along with a glossary of frequently used terms.
