Placental histology in 984 pregnancies (758 term and 226 preterm) occurring during the years 1995 to 1998 was investigated and amniotic cavity cultures were taken during 918 cesarean sections. Histology revealed placental infection in 20.3% of placentas. Chorioamnionitis was confirmed in 119 out of 758 term pregnancies (15.7%) while in 81 out of 226 preterm pregnancies (35.8%); the difference is highly significant (chi 2 = 26.6, p < 0.01). Bacteriological culture resulted in bacterial growth in 19.7% of all cases (181/918), its frequency was significant higher in premature births (54/133, 40.3% chi 2 = 25.2, p < 0.01) as compared with pregnancies carried to term. Recovery of any organism from the amniotic cavity was strongly associated with chorioamnionitis confirmed by histology. Comparison of the rates of indicating placental infection found in this study and a study performed between 1981 and 1983, showed that the program aiming at detection and treatment of silent intrauterine infection has only partially been successful.
This study was performed prospectively. Between 1. January 1995 and 31 December 1997, a modification of the screening-based strategy protocol was implemented. Antenatal screening cultures for GBS were performed at approximately 30-32 weeks of gestation. The protocol recommends the use of antibiotic prophylaxis to GBS positive women with any of the obstetric risk factors for early-onset of GBS disease. Our regiment for prophylaxis for patients in labor was ampicillin 2 g. intravenously then 1 g. i.v. every 4 hours until delivery. Before this study had started (1984-1994), there were 149 serious neonatal GBS infection (149/15,040 pregnancy, among them were 97 premature infants. Thirty-one infants suffered from connatal sepsis. We observed 29 lethal infection. Between January 01, 1995, and December 31, 1997, 4150 women participated in this investigation. The incidence of positive group B Streptococcus cultures from the vaginal samples was 11.6% (481/4150). During the study period (3 years), serious GBS infection was detected in 46 infants (1.1%). There were 9 cases of neonatal sepsis (0.2%), two of them suffered lethal infections (0.05%). There was no late onset of GBS disease and lethal outcome in the last two years. Our investigations bears clinical importance because we confirmed that group B streptococcal colonization is an important risk factor for neonatal infection. The selective intrapartum chemoprophylaxis is a safe and effective intervention to prevent early-onset severe GBS disease.
In Central Europe seven lps species are characterized by differences in morphology, structure of galleries, host specificity and aggressiveness. These species were analysed by allozyme markers and by sequencing 567 bp of the mitochondrial cytochrome oxidase I gene, in order to define their phylogenetic relationships. Orthotomicus erosus and Tomicus minor were taken as outgroup species. The data revealed high inter-specific and low intra-specific sequence divergence. Plotting the observed sequence divergence caused by transversions (Tv) and transitions (Ts) and the level of saturation for Ts and Tv of each codon position showed that the third positions were highly saturated by multiple substitutions. Maximum parsimony analysis produced two groups: (1) I. typographus, I. cembrae, I. amitinus, I. duplicatus and I. acuminatus; (2) I. mannsfeldi, I. sexdentatus and the two outgroups. In all analyses the species of the first cluster were put together and I. typographus and I. cembrae, and I. mannsfeldi and O. erosus emerged as sister pairs. The data do not support a common ancestor for the seven European lps species. The eight-spined bark beetles (except I. mannsfeldi) and I. acuminatus formed a monophyletic group. The close relationship of I. mannsfeldi and O. erosus supports the latter belonging to the genus lps as proposed by Wood (1982) and Escherich (1923). However, more genetic markers and more species of the genera Orthotomicus and Pityokteines have to be analysed to resolve the phylogenetic positions of I. sexdentatus I. mannsfeldi within the tribe lpinl.
A case of a 44 year old woman with antrum gastritis and H. pylori infection was reported. After unsuccessful treatment of the disorder with bismuth and tinidazole, an auto-vaccine was prepared from the bacterium in order to eliminate the infection. After the first injection of the vaccine a generalised urticaria was observed. In the development of the skin eruptions a type I, and a type IV allergic reaction could be demonstrated using the H. pylori specific RAST-test and leukocyte migration inhibition respectively. After eradication of the bacterium by amoxycillin treatment, the clinical signs of both the gastrointestinal and allergic diseases disappeared.
Since 1986 in the radiodiagnostic department of the District Institute of National Health in Lucenec 17 768 abdominal sonographies were made. In the total number of examinations by means of ultrasonography 22 aneurysms of the abdominal aorta were diagnosed, incl. one ruptured one. The high mortality rate on operation of ruptured aneurysms can be reduced in particular by elective resections of non-ruptured aneurysms. Examination of the abdominal aorta should be made as a matter of routine, independently on indication of abdominal sonographic examination, in particular in elderly patients.
The authors searched for Campylobacter pylori (CP) in gastric biopsies from 180 patients by means of microbiological culture. Warthin-Starry staining and urease activity determination. 50 patients with CP-positive antral gastritis were treated with bismuthsubsalicylate 2.4 g per day for 3 weeks, followed by a therapy-free interval of 7-10 days and then a control biopsy was performed. Combined results of bacterial culture and histology proved to be mostly reliable. The prevalence of CP in Hungary is similar to other European countries. CP-positivity was found at the following rates: endoscopically normal patients 30%; stump gastritis 30%; antral gastritis 75%; duodenal ulcer 89%. In all CP-positive cases chronic antral gastritis was seen, whereas normal antral mucosa was never CP-positive. There was no correlation between dyspeptic complaints and CP-positive chronic antral gastritis. Both decreased after bismuth therapy independently on the elimination of CP. CP-positivity in 2/3 of the control investigations points to fast recolonization. The clinical relevance of the CP-infection seems to be questionable.
The occurrence of Campylobacter pylori (CP) was studied in 180 patients referred for endoscopy. The bacterium was detected by culture, histology (Warthin-Starry staining) and urease test of antral biopsy samples. Patient groups were formed according to endoscopic diagnoses, clinical symptoms and antral mucosal histology. 50 CP positive patients with chronic antral gastritis were treated by bismuth subsalicylate (2,4 g/day) for 3 weeks. Positivity by culture and/or silver-stained histology proved to be the most reliable way for detecting CP. CP was proved in about 30% in patients with normal gastroduodenum (13/42) or with stump gastritis (4/15), in 75% with endoscopic antral gastritis (51/68) and in 89% with duodenal ulcer (49/55). A close relationship between CP and histological chronic antral gastritis could be demonstrated. No causal link between CP positive chronic active antral gastritis and non-ulcer dyspepsia could be verified. The decrease in histological activity of chronic gastritis and in dyspeptic complaints after bismuth salt therapy was found to be independent of CP elimination. The results of control investigations following a therapy-free interval of 7-10 days speak in favour of CP recolonialisation within a relativelly short period. It can be concluded that, despite the undeniable relationship between CP and chronic antral gastritis and duodenal ulcer, further studies are necessary to clarify the clinical relevance of the CP infection.
As a 3rd generation cephalosporin Cefobid monotherapy was applied during 1985-1986 with 16 hematological patients in immunodeficient, immunosuppressive states where the available aimed and combined antibiotic therapy failed to be effective for the treatment of bacterial infections of grave course and septic character. 4 g/day was the average I.V. dose of Cefobid, higher doses were applied only in especially grave septic states. The hematological patients tolerated well the Cefobid in monotherapy. Recovery form the septic state and excellent clinical effect was found with 9 patients, good effect with 4 and satisfactory effect with 1 patient. In 1 case the therapy had to be stopped owing to drug hypersensitivity. Cefobid is regarded as an antibiotic drug that is effective if used in monotherapy for treating grave, septic infections of hematological patients in immunodeficient--immunosuppressive--myelodepressive states having received earlier antineoplasmic polychemotherapy.
The authors report on the results of Mycoplasma hominis and Ureaplasma urealyticum cultivations performed within two and a half years in 1,421 pregnant women and gynaecological patients. Considering the obstetrical-gynaecological patients Mycoplasma hominis was identified in 7.25% and Ureaplasma urealyticum in 38.11% of cases. No significant difference was found in the incidence of Mycoplasma infection when comparing the results of cultivations of samples obtained from a clinically non-inflamed vagina or cervix with the results of cultivations of samples obtained from the female lower genital tract in the course of an inflammatory disease. In cases of endometritis, salpingitis or cophoritis Mycoplasma hominis could be isolated from the cervix significantly more frequently. The incidence of Mycoplasma hominis infection was also significantly higher in the examined 315 sterile women. The incidence of Mycoplasma hominis and Ureaplasma urealyticum infection did not differ when comparing the 267 symptom-free pregnant women to the other patients. The presence of the pathogenic organisms did not aggravate the unfavourable course of pregnancy. When examining the effectivity of Doxycycline capsule the drug proved to be effective in 78 patients for the treatment of genital Mycoplasma infections.
A 40-year-old female patient developed spontaneous right-sided chylothorax which after a single thoracocentesis did not relapse. Available examination methods did not reveal the cause of the disease. From the clinical course it may be assumed that the chyle drainage was impaired in the thoracic duct or in a minor lymphatic vessel by subclinical injury, whereby the damage obviously was small, capable of spontaneous closure and healing. The authors present some contemporary views on the pathogenesis, diagnosis and treatment of this rare disease.
