[Radicular artery thrombosis after cervical anterior approach for cervico-brachial neuralgia (author's transl)].

An anterior cervical approach completed by a bone graft is performed on a man suffering from cervico brachial neuralgia which did not improve by medical treatment and rest. During four months, the post operative course is excellent, with complete disappearance of pain and regression of the pre-existing neurological deficit. At the end of this period, the appearance of a major deficit within the field of the concerned root led to a selective brachial angiogram which revealed the existence of a radicular artery thrombosis.

[Anterior sacral meningocele. Report on 2 cases treated by the abdominal approach].

Anterior sacral meningocele is a rare affection (150 cases published) that is frequently misdiagnosed with resulting inappropriate, always dangerous treatment. Diagnosis should however be simple to establish on the finding of a fluid hypogastric mass attached to the sacrum with associated radiologic anomalies of the sacrum. Two cases are reviewed and diagnostic and surgical therapy features discussed.

Doppler spectrum analysis and vascular imaging in the diagnosis of extracranial carotid artery disease.

Ultrasound methods are well suited for the exploration of the carotid system. High resolution echography can visualize atheromatous defects on the vessels walls with a resolution better than one mm, whereas Doppler examination detects haemodynamic disturbances due to these plaques. Conventional Doppler examination can reveal moderate or severe stenosis (greater than 60%); the recent development of frequency analysis of the Doppler audio signal makes now possible the detection of very light haemodynamic disturbances due to low grade stenosis even as small as 15%. Spectral distribution abnormalities have been classified in four grades according to their amplitude. The combination of the morphological and functional data provided by echography and Doppler spectrum analysis has drastically increased the diagnostic possibilities of the ultrasound methods in the field of the extracranial vascular pathology. With a new duplex system which combines echography and C.W. Doppler examination (duplex probe) it is possible to detect, and quantify carotid stenosis (in percentage of the lumen reduction), and to evaluate blood flow volume in ml/mn.

Detection of Legionella pneumophila antigen by ELISA in urine or experimentally infected guinea-pigs.

Guinea-pigs were experimentally infected with Legionella pneumophila. L. pneumophila antigen was detected in urine samples by a double antibody sandwich ELISA. Urinary antigen was present from the beginning of the acute phase of the disease. In two out of five cases, this antigen was found with no antibody detectable in sera by indirect immunofluorescence staining.

[Plea for a unifying concept of the cavernous sinus and the trigeminal cavity].

The object of this study is a scientific research in human and compared anatomy of the cavernous sinus and Meckel's cave. The observations made in the foetus and human adult are compared to these made in non human primates and domestic mammals, the cavernous sinus and the Meckel's cave contribute to realize a entirety that we call "a morphological and functional anatomical system". The human cavernous sinus and Meckel's cave are described as an indissociable parasellar space representing a heavy traffic area for vascular and nervous structures. In the human and non human primates, the authors observe a parasellar space which agreed with the concept of "trigeminal-cavernous anatomical system". In the cat, the same observations are made and an osseous outline closing the roof of the parasellar space is observed; this is the evidence of a more ancient osseous or cartilaginous wall. The authors demonstrate in the last part of this study that the morphogenesis of this trigeminal-cavernous system is in relation with the phylogenic development of its morphological and functional environment, that we call the "externation". This study is of interest: to a best understanding of the tumors involving the cavernous sinus, to a semantic point of view: an attempt to a review of the terminology applied to the cavernous sinus and Meckel's cave.

[Radiological consequences of lumbar disk surgery. Evaluation by MRI after 3 months].

Radiologic effects of the surgery for lumbar disc herniation: a study with MRI at the end of the third post-operative month. The results of patients operated for lumbar disc herniation are good in 83%, fair in 10% and poor in 7. The poor results are considered to be the consequence of post operative fibrosis. A prospective study in 67 patients has been performed to study the post operative appearance of the operated lumbar spine, three months after surgery. 45 days after surgery the recovery of patients was assessed by the score of Staufer and Coventry and patients were entered in the study independently of their clinical state. The MRI was performed at the end of the third month postoperatively and patient recovery assessed according to the score of Senegas and Lavignolle. All the clinical and radiologic data were statistically analysed... the chi-square test, a p. value less than 0.05 was considered statistically non significant. The correlation between the clinical recovery and the MRI was realized.
