[Perilymphatic fistula. Diagnostic and therapeutic difficulties].

The symptoms and signs of perilymphatic fistulae (PLF) are very varied and frequently misleading. The operative demonstration of a perilymph leak is not always obvious and the therapeutic results are sometimes disappointing. The study of 26 cases demonstrates certain concepts concerning the diagnostic and therapeutic difficulties. Any atypical cochleovestibular symptoms, particularly occurring in a traumatic context, should suggest the possibility of PLF. Exploratory surgery must be performed according to a rigorous protocol in order to demonstrate the fistula. Search for a fenestra ovalis sign due to pressure on the membrane of the fenestra rotunda may help to reveal a lesion of the annular ligament. Simple filling of the breach is not always effective, especially at the level of the stapes. Stapedectomy frequently constitutes the best solution for fistulae of the fenestra ovalis.

[Aneurysmal cyst of the petrous bone. Apropos of a case].

Aneurysmal bone cyst (ABC) is a rare form of osseous dystrophy. A lesion of the petrous temporal bone appears to be extremely rare. It must be differentiated from a hemangioma which is a hamartoma affecting the geniculate ganglion or the internal auditory meatus. The case described involved a 23-year-old woman. The ABC presented with violent otalgia and severe endolabyrinthic-type perception deafness. It was associated with extensive bony destruction of the petrous temporal and occipital bones. Curettage only led to considerable through temporary regression of deafness and bone reconstruction within a few months. Thrombosis of the internal jugular, confirmed at operation, is an interesting contribution in terms of the pathogenesis of ABC, apparently linked to disturbances of venous pressure. Surveillance is required since this benign condition may be recur.

[Limited mandibular osteoradionecrosis. Early conservative surgical treatment].

Nine patients with early phase mandibular osteoradionecrosis received conservative treatment involving closure with mucosal and periosteal graft. Early intervention and the limited nature of the osteoradionecrosis are the two fundamental elements for a successful outcome to this conservative therapy, thus avoiding progression to an extensive form requiring interruption mandibulectomy.

[Indications for exofacial parotidectomy in treatment of mixed tumors (author's transl)].

Systematic total parotidectomy is still recommended by some authors for treatment of mixed tumors of the parotid. This attitude is based on the notion of multifocal involvement. In fact, multifocal involvement seems exceptional Very wide exeresis of the tumor is sufficient for efficient treatment, but the anatomical constitution of the parotid makes topographical division difficult. An attempt for systematisation of exeresis following the topography of the tumor is suggested, mainly for cases where parotidectomy is sufficient.
