Mammary hamartoma, a benign breast lesion.

Hamartoma or mastoma is a benign breast lesion which is unfamiliar to most clinicians. Five cases with this lesion are presented. It is a rather soft, sharply delineated lump which may cause a considerably asymmetry of the breasts. Radiologically it appears as a circumscribed tumour with homogenous or varying density and at operation the lump is easily enucleated. Histologically it contains normal or dysplastic mammary tissue. It is unilateral and benign. The lesion is now more frequently diagnosed because of increased use of mammography and can be mistaken for a neoplasm. It is therefore important that clinicians and radiologists are acquainted with it.

Renal revascularization after splenic artery implantation into the kidney. Studies on angiography ex vivo, microangiography and histology in dogs.

Twenty-eight mongrels were subjected to splenic artery implantation into the left kidney for a period of time varying from 7 days to 32 weeks. In 25 of the kidneys a renal artery stenosis was created. Angiography ex vivo, microangiography and histological examination showed that direct implantation of the splenic artery into the renal parenchyma regularly was followed by an initial thrombus formation in the implanted artery and an early recanalization of this thrombus. Newly formed vessels could be demonstrated in the vicinity of the intrarenal portion of the artery already during the second week after implantation. By way of these newly formed vessels three different types of communications were established between the implanted artery and the original intrarenal arteries.

Conization as only treatment of carcinoma in situ of the uterine cervix.

Knife conization was performed in 2,099 cases with abnormal vaginal smears. The frequency of complications was low. Carcinoma in situ was diagnosed in 1,500 cases and follow-up showed that conization was curative in 87%. The curative rate was depending on whether the resection margins were free of pathologic epithelium or not. If smears were repeatedly negative the first year after conization a new diagnosis of cancer was made in 0.4%. It was not possible to decide whether these lesions were residual changes or true recurrences. Treatment of carcinoma in situ by conization has so far reduced the frequency of invasive cervical cancer by 60%.

A retrospective study of 304 cases of malignant melanoma in Malmö 1952-71.

In the city of Malmö with about 250 000 inhabitants and only one hospital, all cases of malignant melanoma are referred to the departments of plastic surgery and radiotherapy. Altogether 304 cases were treated between 1952 and 1971 and all patients were accounted for at the follow-up. This figure does not include cases of superficial melanoma, Hutchinsons melanoma or eye melanoma. The incidence of melanoma increased from 3.4 to 8.0 per 100 000 inhabitants during the period. The treatment was the same during the years. The results of the treatment are presented in relation to the stage of disease, surgical procedure used, site of the tumour and sex. The overall 5-year survival rate without recurrence was 62%, while 6% were alive with recurrence. The corresponding figures for stage I cases were 71% and 7%. Total biopsy of the tumour before more radical surgery was not found to have any disadvantage. In view of the good results, we intend to continue the present method of treatment with addition of adjuvant chemotherapy in the "poor risk" groups.

Bilateral fibromuscular hyperplasia in the internal carotid arteries with aneurysm formation.

A patient with bilateral fibromuscular hyperplasia in the carotid arteries with development of aneurysms is reported. The patient had no symptoms except for a palpable mass on one side and a bruit on both sides. Surgery was carried out bilaterally, using direct end-to-end suture on one side and a reconstruction with an autologous vein on the other side. It is suggested that fibromuscular hyperplasia may be the cause of some of the internal carotid artery aneurysms reported as congenital or with uncertain etiology. Reconstructive surgery of the carotid artery is recommended due to the obvious hazards of the condition.

Carcinoid tumours. Frequency in a defined population during a 12-year period.

The frequency of carcinoids was studied in a 12-year-period (1958-1969) in Malmö, a town with a population of 220,000 at the beginning of the period and 250,000 at the end of it. Of all persons who had died in Malmö, 46 per cent were necropsied in the first year of the study and 70 per cent in the last (altogether 62.6 per cent). The series was examined in a uniform way at one department of pathology. Carcinoid tumours were found in 1.22 per cent (199 patients) of patients comprised in the entire necropsy series (16,294 autopsies). Bronchial carcinoid accounted for 0.1 percent, the remaining lesions were found in the digestive tract. About 90 per cent of the carcinoids were found incidentally at necropsy. During the same period, 44 carcinoids were diagnosed in surgical specimens examined in Malmö. The average annual frequency of carcinoid in the entire series was about 8.4 per 100,000 inhabitants, which is about 7 times as high as that recorded in the National Cancer Register applying to the whole of Sweden. The value of such country-wide reports is discussed. The carcinoid syndrome is extremely rare and was observed only once during the entire 12-year-period. The localization, frequency of metastases and sex-distribution of carcinoids are described and discussed in detail.

Plasminogen activators in ovarian tumours.

Ovarian tumours obtained at laparotomy were histochemically examined for their local fibrinolytic activity, and simultaneous fibrin/fibrinogen degradation products (FDP) were determined in the serum. The fibrinolytic activity was confined mainly to vessels of both malignant and benign tumours. A very close correlation was demonstrated between the fibrinolytic activity and the vascularity of the sections. FDP were found in the serum in 13 of 14 patients with malignant tumours, but in none with benign tumours. The difference in occurrence of FDP in patients with malignant and benign tumours might be due to the invasive growth of the former with the entrance of thromboplastic substances, fibrinolytic activators or locally formed FDP into the bloodstream.

Renal cortical necrosis induced in rabbits by local perfusion followed by systemic administration of endotoxin.

In an experimental model adapted to avoid severe systemic lesions, renal cortical necrosis was frequently induced in rabbits by repeated administration of endotoxin. The experimental model involves perfusion of the kidney in situ with endotoxin. Another dose of endotoxin was given intravenously 24-28 h before or after the perfusion. A significant effect of the perfusion or the endotoxin administered first was reflected by the pronounced unilateral morphological alterations appearing after the second administration of endotoxin. Some of the kidneys with cortical necrosis had band- or ring-shaped areas of preserved juxtamedullary tissue. Heavy infiltration of leukocytes at the borders of these areas called for commentation of the histological picture of infarcts, incorrectly described even in most modern textbooks.

Significance of increased factor VIII in early glomerulonephritis.

One hundred sixteen patients with early glomerulonephritis had factor VIII without impairment of renal function. They also had other coagulation and fibrinolytic components and acute phase reactants. The patients were followed for up to 4 years with respect to both coagulation pattern and renal function. Patients who made a complete recovery during follow-up had normal initial factor VIII levels, while those who developed persistent renal damage had high factor VIII levels. The other coagulation factors and the acute phase reactants were of no prognostic significance. Factor VIII has been shown to be synthesized in the vessel intima. The initial factor VIII level in early glomerulonephritis may reflect the degree of vascular affection of the glomeruli. The high factor VIII levels may be of significance for the development of fibrin deposits in glomeruli.

Subcutaneous mastectomy in 80 patients with breast tumours. Indications, surgical results and pathological findings.

Eighty patients were operated upon with subcutaneous mastectomy and reconstruction with prosthesis. Seventy operations were performed on both breasts. The indications used were carcinoma in situ in a previous biopsy, pronounced epithelial proliferations on at least two previous occasions or recurring fibrocystic disease after at least two previous biopsies. Since the autumn of 1971, the operation has been performed as a one-stage procedure and we have had few complications. The appearance of the breasts was evaluated at review 3-60 months after operation. Most patients found the results acceptable or good. The mamilla always lost its sensibility, but the sensibility of the remaining skin of the breast sometimes returned with time. Most of the patients were well satisfied with the procedure and thought it better than any other sort of treatment. The tissues removed were thoroughly studied. Changes in the breasts were often bilateral and pronounced epithelial proliferation and carcinoma in situ were fairly common. Invasive carcinoma was found in 2 instances. The importance of the histologic examination of removed tissues is stressed. A regular follow-up of these patients is important for assessing the value and the risks of subcutaneous mastectomy.
